The film tells the story of Su Zhenghuai (Yin Zhaode), a low-level clerk of the Department of Health, who took his eldest daughter Su Ting (Xia Yiyao) and son Su Yuesheng (Chu Yuhe) to visit relatives in the provincial city, and met su Ling (Zhu Ya), the second daughter who had been missing for a long time, on the train. During the train was attacked by a swarm of rats, Su Yuesheng was unfortunately bitten by rats and fell into a coma, in order to save his young son and many passengers who were bitten, Su Zhenghuai and Su Ting decided to venture to Xiaobaihe Hospital to get medicine, thus starting a extreme escape.

During the Republic of China, Su Zhenghuai and his family boarded a train to the provincial capital. Shortly after the train started, the rats in the carriage struck, and the passengers began to run away in surprise. The passengers were attacked by the rats and fell one after another, and the younger son, Su Yuesheng, was also bitten by the rats and fell into a coma. Su Zhenghuai and others temporarily escaped from danger with the help of Du Dashan, a passenger in the same carriage. In order to save the lives of Su Yuesheng and the survivors, Su Zhenghuai and others decided to go to the town hospital to get medicine. Not wanting the whole town to be occupied by rats, the people went through hardships to bring back the medicine. Survivors also began to improve after injecting drugs. Just when everyone thought they were out of danger, the train suddenly stopped when it encountered obstacles. In a time of crisis, the survivors came together and managed to clear the obstacles on the railroad tracks. The train starts again. At this moment, the train driver found that the wrench in front of him was not pointing in the right direction, and Su Zhenghuai resolutely turned around and pressed the wrench that had fallen into disrepair with his hand until the train completely passed.
