Recently, the Guangdong Provincial Government approved the investigation report on the "7.15" major permeability accident of the Shijingshan Tunnel of the First Bid Section of the Zhuhai Xingye Express (South Section), and determined that the "7.15" permeability accident of the Shijingshan Tunnel of the First Bid Section of the Zhuhai Xingye Express (South Section) was a major production safety liability accident.

After investigation, the direct cause of the accident was that when the tunnel passed under the Jida Reservoir, it encountered a water-rich granite weathering deep trough, and due to improper engineering measures, the palm face vault of the right line tunnel collapsed and permeated, pouring into the left line tunnel and causing the operators to drown.

The accident investigation report pointed out the problems existing in the relevant enterprises and the Zhuhai Municipal Government and relevant departments, mainly including: the implementation of the construction risk control measures of the second bureau and the third company of the China Railway And the third company was not effective, and the construction was not in strict accordance with the requirements of the design and safety specifications; the subcontracting behavior of the second bureau of the China Railway; the Zhuhai Xingdi Company did not perform the safety production duties of the supervision unit in accordance with the law; the Zhuhai Urban Construction Municipal Company did not organize the implementation of advanced geological drilling as required, and the on-site safety inspection organization of the agency construction project was not effective; the survey report of the Zhuhai Engineering Survey Institute was not true. Accurately reflect the geological situation, the rate of exploration and laying holes, drilling and core adoption does not meet the norms; Zhuhai Jixiang Blasting Company does not have the conditions for safety production permits, and carries out blasting operations in violation of laws and regulations; Fujian Zhuozhi Company does not construct in accordance with construction technical standards and does not implement the safety production responsibilities of labor subcontracting units; other relevant units include Zhuhai Highway Affairs Center, Zhuhai Urban Construction Group, Zhuhai Huaxin Company, Shanghai Municipal Engineering Design Institute, Zhuhai Zhengqing Company, Zhuhai Huayuan Blasting Company, etc. There are safety problems Zhuhai Municipal Bureau of Housing and Urban-Rural Development, Zhuhai Municipal Construction Engineering Safety Affairs Center, Zhuhai Xiangzhou District Housing and Urban-Rural Development Bureau, Zhuhai Municipal Water Affairs Bureau, Zhuhai Xiangzhou District Agriculture and Rural Affairs and Water Affairs Bureau, Zhuhai Municipal Public Security Bureau Gongbei Port Branch, Jida Police Station, as well as Zhuhai Municipal and Xiangzhou District Party Committees and Governments have certain problems in performing their safety supervision duties.

The public security organs took compulsory measures against the 12 responsible persons suspected of committing the crime in accordance with the law, and transferred them to the judicial organs for criminal responsibility in accordance with law. Specifically, they include: Liang Jie, member of the Party Committee and deputy general manager of China Railway Second Bureau And Three Companies, Liu Guang, manager of the project department of China Railway Second Bureau And Three Companies, Yang Jincheng, deputy manager of the project department of China Railway Second Bureau And Three Companies, Zhong Kui, chief engineer of the project department of China Railway Second Bureau And Three Companies, Chen Xiaolong, safety director of the project department of China Railway Second Bureau and Third Company, Lu Aiyong, legal representative and chairman of Zhuhai Xingdi Company, Hu Yunbin, project director of Zhuhai Xingdi Company, Nie Jinwei, project director representative of Zhuhai Xingdi Company, and Lu Ruimei, legal representative of Fujian Zhuozhi Company. Wang Changrong, the on-site person in charge of Fujian Zhuozhi Company, Chang Genzhao, the project technical leader of Zhuhai Jixiang Blasting Company, and Wang Kaiwen, project director of Zhuhai Huaxin Company.

The relevant departments imposed administrative penalties on the Second Bureau of China Railway, the Third Company of the Second Bureau of China Railway, the Zhuhai Urban Construction Municipal Company, the Zhuhai Engineering Survey Institute, the Zhuhai Xingdi Company, the Fujian Zhuozhi Company, the Zhuhai Jixiang Blasting Company, the Zhuhai Zhengqing Consulting Company, the Zhuhai Huayuan Blasting Company, and 16 relevant personnel in accordance with the law, and some enterprises were included in the "blacklist" management of joint disciplinary action.

The personnel and public employees of the enterprises involved such as the Second Bureau of China Railway shall be handed over to the relevant units for handling in accordance with the provisions of the party organization relationship and management authority.

The discipline inspection and supervision organs held a total of 27 people accountable for the party committees and governments of the areas where the accident occurred, public employees of relevant supervision units, and state-owned enterprise personnel, as follows:

Gu Shengjie, director of the Zhuhai Highway Affairs Center, gave a warning within the party and a punishment for excessive administrative records.

Xu Wei, deputy director of the Zhuhai Highway Affairs Center, gave the party a serious warning and a major punishment for the administrative secretary.

Zhang Heping, director of the First Department of Engineering Management of Zhuhai Highway Affairs Center, was given a one-year probationary period and dismissal from government affairs, and was demoted to a first-level section member.

Li Fuzhen, deputy director of the First Department of Engineering Management of Zhuhai Highway Affairs Center, was given a one-year probationary period and dismissal from government affairs, and was demoted to a second-level section member.

The accident investigation report put forward suggestions for accident prevention and rectification measures, emphasizing that local party committees and governments and relevant departments, especially Zhuhai City, should thoroughly implement the spirit of General Secretary Xi Jinping's important instructions, deeply absorb the tragic lessons of the accident, firmly establish the concept of safe development, effectively do a good job in the safety management of water-related underground projects, supervise and urge enterprises to implement the main responsibility for safe production, and strictly prevent the occurrence of major accidents.
