Thousands of miles away to listen to the "heart sound" issued an early warning, Hubei Provincial People's Hospital set up a cloud ECG monitoring network

Chutian Metropolis Daily, November 7 (reporter Yan Wen correspondent Du Weiwei photo: Liu Yu) "Jingle bells..." In the early morning of November 6, a rapid telephone call woke up Mr. Li, who was still asleep in Yicheng, Hubei Province. At the end of the phone is Shi Shaobo, a doctor at the "Hubei Provincial ECG Big Data Center" located in Hubei Provincial People's Hospital (Wuhan University People's Hospital). "Remote ECG Monitoring Warning: You have developed a serious heart block, please go to the nearest hospital as soon as possible." Fortunately, after timely diagnosis and treatment, Mr. Li got rid of the danger of sudden cardiac death.

This is one of the many scenarios of "construction and application of wearable ECG monitoring and early warning system based on big data" led by Professor Huang Congxin of the Department of Cardiovascular Medicine of the People's Hospital to benefit cardiovascular patients. In the past four years, the project has screened more than 10,000 patients and saved lives. On November 3, the project passed the on-site acceptance with "excellent" results in the major project of technological innovation organized by the Department of Science and Technology of Hubei Province.

Professor Huang Congxin introduced that there are as many as 290 million patients with cardiovascular diseases in China, and the number of sudden cardiac deaths every year is as high as 570,000. ECG information as an important diagnostic basis for screening, early warning and diagnosis of acute coronary syndrome, arrhythmia, sudden cardiac death and other major cardiovascular diseases, the analysis, accumulation, refinement and summary of big data are helpful to greatly improve the quality of cardiovascular disease diagnosis and treatment.

Launched in 2016, patients with cardiovascular disease can use a doctor-directed "wearable" device that weighs only 39 grams to enable remote, real-time Holter MONITORing. The patient's ECG information can be sent directly and remotely to the doctor's workstation of Hubei Provincial People's Hospital and permanently stored in the Internet cloud, avoiding the drawbacks of routine ECGs that are difficult to capture the patient's occasional abnormal ECG information and difficult to save. If poor ECG activity is detected, the "wearable" device will automatically alarm; if the patient feels unwell, he or she can manually "one-click alarm". Doctors' stations and big data center headquarters will automatically receive "critical value" reminders and manual alarms from patients, so that doctors can catch abnormal ECG activities in a timely manner and guide diagnosis and treatment.

Over the past three years, Professor Huang Congxin has led the members of the research group to go deep into dozens of counties, cities and townships in Hubei Province and Henan Province to build a remote ECG diagnosis and treatment network and train doctors in grass-roots hospitals in ECG diagnosis technology. At the same time, a set of efficient medical service processes have been established in Hubei Provincial People's Hospital, providing remote ECG diagnosis and treatment services for 38 medical institutions. Community and township primary hospitals that join this center, if the diagnosis of patients' ECG activity is not clear, they can also directly transmit patient information to the headquarters of "Hubei Provincial ECG Data Center" located in Hubei Provincial People's Hospital through this big data center. The 4 cardiovascular disease experts at or above the deputy senior post who take turns on duty will study and judge in a timely manner and issue reports to guide treatment, so that patients can enjoy the services of cardiovascular experts in top domestic hospitals without leaving the village and town community, and promote the implementation of graded diagnosis and treatment.

Professor Chen Yuanxiu, one of the members of the project team and director of the Department of Cardiac Function of Hubei Provincial People's Hospital, introduced that the project used 1 million + ecgrectronic data to develop arrhythmia recognition algorithm, equipped with ECG screening equipment, applied in more than 80 hospitals across the country, completed 270,000 in-hospital independent screening, and found and treated more than 10,000 cases of hereditary cardiomyopathy, hereditary arrhythmia diseases, idiopathic ventricular tachycardia, high atrioventricular block and other high-risk patients with sudden cardiac death, with a positive monitoring rate of 3.7%. At the same time, we continue to develop smarter and more convenient second-generation products, combined with telemedicine, to achieve a full-life cycle of closed-loop medical services for arrhythmia diseases from early warning, diagnosis, treatment to follow-up.
