It is difficult to evaluate the professional titles of medical staff, and many doctors have joked that "it is more difficult to be promoted from attending physician to chief physician than to win the lottery jackpot".

The difficulty in the promotion of medical personnel is mainly caused by the large number of additional conditions in the evaluation of health personnel's professional titles.

Educational requirements, years of experience, number of papers published, and time spent on continuing studies or going to the countryside to support are all factors that affect the promotion of medical staff.

To this end, the Ministry of Human Resources and Social Security has issued the Notice of the General Office of the Ministry of Human Resources and Social Security on Further Improving the Evaluation of Professional Titles, which has deepened the reform of "one-size-fits-all" and simplification of the "one-size-fits-all" and simplistic issues such as academic qualifications and the number of papers published in the evaluation of medical staff's professional titles (Central People's Government Network, 2022).

"Reasonable setting of requirements for papers and scientific research results. Health, engineering, art, primary and secondary school teachers and other professional titles with strong practicality do not take papers as the main evaluation index for professional title evaluation, and the evaluation criteria must not simply set up rigid requirements such as the number of papers and impact factors. For research series talents, focus on original achievements and high-quality papers, pay attention to the evaluation of original contributions, academic influence and research capabilities, and dilute the number of papers. Each title series gradually transforms the "mandatory" paper into "multiple choice" of achievements, and establishes a "menu-type" evaluation index system. Promote the representative achievement system, standard development, technology promotion, technical solutions, innovation breakthroughs, high-quality patents, achievement transformation, theoretical articles, think tank achievements, literary and artistic works, teaching plans, medical records and other performance achievements can be used as representative achievements to participate in the title review. Explore ways to improve the authority of representative achievement evaluations through methods such as academic committee identification, peer expert review, third-party institution evaluation, and international peer evaluation. Human resources and social security departments at all levels cooperate with relevant industry authorities to severely crack down on violations of discipline and regulations such as writing and distributing papers on behalf of others, false publication, etc., and encourage scientific researchers to publish high-quality papers in domestic scientific and technological journals. Academic misconduct such as plagiarism, plagiarism, and improper authorship is to be handled in accordance with the "Rules for the Investigation and Handling of Untrustworthy Conduct in Scientific Research" and other relevant provisions, and the obtained professional titles are to be revoked, and recorded in the creditworthiness archives for the application and review of professional titles, and where the circumstances are serious, the qualifications for professional titles and post promotions are to be revoked for a set period of time.

Reduce restrictive conditions such as academic qualifications and awards. Each title series only makes basic requirements for the applicant's academic qualifications, and if the applicant does not have the required academic qualifications but has significant performance and outstanding contributions, it can be recommended by more than 2 peer experts with senior professional titles to make an exceptional declaration, except as otherwise provided by laws and regulations. Except for series or majors involving public safety and personal health, if the major engaged in is inconsistent with the major studied, it may be allowed to declare the professional title according to the person's long-term professional engagement. Part-time academic qualifications and full-time academic qualifications, vocational college graduates and ordinary school graduates at the same level enjoy the same treatment in the evaluation of professional titles. Graduates of intermediate engineering classes, senior engineering classes, and preparatory technician (technician) classes of technical colleges can apply for corresponding series of professional titles according to technical secondary school, junior college, and undergraduate degrees. Scientific research projects, the number of funds, awards, the level of papers and journals, titles, titles, etc. shall not be used as restrictive requirements for the evaluation of professional titles. ”

The release of this policy by the Ministry of Human Resources and Social Security has enabled some medical staff who have performed well in their posts but failed to meet the requirements of academic qualifications and scientific research achievements to have the opportunity to be promoted to their professional titles, which has improved their enthusiasm for work.

However, there are still many difficulties in the promotion of medical personnel. Among them, the most criticized is the issue of job quotas.

With the continuous expansion of the medical system, the number of TCM physicians in the department is also increasing. However, there is only one department director in a department, and some departments have 1~2 deputy directors, and the number of positions is limited.

Even if the attending physician of many large hospitals meets all the conditions and has obtained the title of deputy chief physician, he may still be in the position of attending physician. This is because there are too many associate chief physicians, and attending physicians often have to wait for a vacancy in the promotion or departure of a physician at the next level before they can be hired.

In previous years, the title promotion standards stipulated that medical personnel must have been employed for a certain number of years in the previous level before they can apply for the next level of title promotion. In other words, many medical staff have obtained a job title, but they are unable to apply for a promotion to the next job title because they have not been hired for a position that matches that job title.

This led many doctors to complain, "It took 20 years to be promoted to deputy chief physician, and I am still the deputy chief physician after I retire." ”

Recently, the Liaoning Provincial Health Commission has put forward improvement measures in response to the above difficulties, bringing hope to countless medical personnel who are unable to be promoted due to the time constraints of their posts.

The "Notice" specifies that the personnel who apply for the deputy senior and senior titles must have been employed in intermediate and deputy senior positions, and the length of service can be calculated in "segments":

2023 and before can be calculated according to the time of obtaining the corresponding professional and technical qualifications. In 2024 and beyond, it will be calculated according to the time of appointment of the corresponding professional and technical positions. The two periods of time are combined to calculate the seniority time. Physicians who have completed their appointment before June 30 of each year can calculate their tenure for the whole year.

That is to say, if in 2023 and before, a doctor has obtained the title of deputy chief physician for 4 years and is hired as an associate chief physician in 2024. According to the previous standard, the doctor had to continue to work in the position of deputy chief physician for 5 years before he could apply for the chief physician. However, under the new requirements, the doctor has served for exactly 5 years in total, and he is eligible to apply for title promotion this year.

The "Notice" has also adjusted the conditions for serving the grassroots level.

Licensed physicians who are promoted to associate senior titles shall have at least one year of experience in providing medical and health services at medical and health institutions below the county level or with counterpart support.

Physicians who meet the "Notice on Issues Concerning Urban Doctors' Application for Senior (Associate High) Professional and Technical Qualifications to Serve in Rural Areas" (Liao Weihan [2014] No. 163) and other relevant regulations may be deemed to have completed the experience of serving at the grassroots level or are not required.

The "Notice" also introduced incentive policies for front-line medical personnel in epidemic prevention and control.

The work experience of medical personnel participating in the prevention and control of the new crown epidemic in the front line of prevention and control is regarded as the completion of the work experience of serving the grassroots level.

Front-line medical personnel for epidemic prevention and control may, on the basis of meeting the requirements for professional and technical qualifications, apply for the evaluation of a higher professional title one year in advance.

Front-line personnel in prevention and control who have made outstanding contributions and won commendations and awards at or above the provincial and ministerial level may directly apply to participate in the evaluation of higher-level professional titles in combination with the evaluation of their professional ability.

The "Notice" also refines the requirements in terms of performance results conditions.

The work performance implements the representative work system of results. Clinical cases, surgical videos, nursing cases, epidemiological investigation reports, emergency response reports, papers, final reports of scientific research projects, health standards, technical specifications, popular science works, technical patents, and transformation of scientific research achievements can all be used as representative works of performance achievements to participate in the review.

According to the different functional positioning of medical and health institutions at different levels, in addition to providing medical records or business work reports that can reflect their professional and technical level since they took up their current positions, they can also submit the landmark work performance that best represents their professional and technical ability and level since their current position in three categories: professional ability, teaching ability and scientific research ability.
