On October 24, 2023, the Shanxi Provincial Emergency Management Department released the investigation report of the accident. After investigation, the direct cause of the accident was that the truck platform was not formally designed and constructed as required, and the weight of the goods stacked and the body weight of the operator far exceeded the bearing capacity of the supporting columns and beams of the steel structure layered load platform, resulting in the bending and destruction of the supporting column and main beam of the steel structure layered load platform, which in turn led to the overall collapse of the steel structure layered load platform.

After investigation, the cause of the accident is that the rainwater infiltrates into the horseshoe kiln operation channel through the gap on the side of the cover plate of the coal pit, the structure is unstable after the soil layer seepage, the supporting force of the channel arch is insufficient, and the disturbance of the masonry operation makes the operation channel collapse. The villagers did not call the police at the first time, and risky rescue led to a secondary collapse, delaying the scientific and professional rescue time.

According to the investigation by the accident investigation team of the State Council, this is a major production safety liability accident caused by the illegal implementation of the renovation project by the hospital, the ineffective construction safety management, the chaos of daily management, the long-term existence of fire hazards, the illegal operation of the construction unit, the lack of on-site safety management, the ineffective emergency response, and the failure to implement the responsibilities of the local party committee and government and relevant departments.

After investigation, due to the narrow passage of the slurry operation and the lack of protective railings of the platform, Ji Mouliang accidentally fell into the collection pool (limited space) during the slurry operation, and the oxygen content in the collection pool was insufficient.
