Chapter XXIII: Yin and Yang Studies 4

The five elements of yin and yang explained in Chinese medicine textbooks stay at the philosophical level, but there is a difference between philosophy and science. How do we answer the question of yin and yang? If the theory of yin and yang is simply described, functionally speaking, yin and yang are physiological excitation and inhibition. What causes excitation and inhibition in the human body? It is mainly activated and inactivated at the level of proteins and nucleic acids, resulting in excitation and inhibition of cells and organs, so that people as a whole show excitement and inhibition, that is, the two states of yin and yang.

Traditional Chinese medicine believes that the human body uses qi, blood, essence and jin as the basic raw material, and uses meridians as channels, through the five internal organs of mutual coordination and mutual restraint, the movement and change of qi, blood, essence and jin liquid occur, which is the gasification of traditional Chinese medicine. Gasification activities are ultimately manifested as the excitement and inhibition of physiological functions, that is, yin and yang, thus completing life activities and reproductive activities. Western medicine believes that the human body is based on oxygen, sugar, lipid, protein, vitamins, trace elements as the basic raw materials, through the synergistic effect of eight major systems (movement, reproduction, nerves, respiratory, digestion, circulation, urinary, endocrine), the transformation of matter and energy occurs, and finally the function of cells and organs appears excited and inhibited, so as to complete life activities.

Therefore, the raw materials of Chinese medicine are qi, blood, essence and liquid, and Western medicine is oxygen and nutrients; Traditional Chinese medicine is based on the meridians as the channel, through the five internal organs to play a role, Western medicine is the eight major systems; What ultimately happens in TCM is the movement and transformation of qi, blood, essence, and liquid, that is, gasification; Western medicine is ultimately the transformation of material energy and information, that is, metabolism; Traditional Chinese medicine ultimately presents the two functional states of yin and yang, while Western medicine is the excitement and inhibition of physiological functions.

Western medicine has three main schools of understanding the human body: the first is the structural school, which mainly studies the structure of the human body through dissection, light microscope and electron microscope; The second is the functional school, which understands the function of the human body from the level of systems, organs, cells and proteins; The third is the informatics school, which mainly studies how biological macromolecules (such as nucleic acids, proteins, polysaccharides, etc.) transmit biological information. For example, nucleic acids are triplet encoding, which is a major molecule of information; Proteins transmit biological information through changes in quantity and phosphorylation; Polysaccharides also transmit biological information.

We can study yin and yang from three aspects: First, the material basis of yin and yang. Genes, proteins, including neuroendocrine and other aspects can show some characteristics of yin and yang, so yin and yang have a material basis. The material basis of yin and yang is not a single substance, but multiple substances all affect the physiological functions of the body, and the synergy is manifested as the characteristics of yin and yang. Second, the function of yin and yang. For example, to study the causes of excitation and inhibition in the body, we can study it from the protein, cell and overall level. Third, the information of yin and yang. It can be discussed from the level of physics, for example, yin and yang can be understood as the two states of matter, energy and information in the human body: yin formation is the self-organization process of matter, energy and information, thereby forming cells and organs, constituting "man", a self-organizing creature; We living people need to consume matter and energy, and yang qi is the process of dissipating matter and energy; Yin and yang secret is a natural and ordered state of the body, and these are the yin and yang research methods guided by the theory of dissipative structure. To sum up, the study of yin and yang, on the one hand, can find the material basis of yin and yang, on the other hand, we can find the functional characteristics of yin and yang. Only by organically combining structure and function can we better explain the essence of yin and yang.

1. Yang gas research. Malignant tumors are characterized by poor cell differentiation and loss of cell function. An immature cell does not have the full function of the cell, such as the poor ability of liver cancer cells to synthesize albumin. Functional activity refers to gasification, and Yang gas represents the functional activity of the human body. Wenyang drugs help to dissolve qi, can promote cell differentiation, improve the functional activity of cells, thereby reversing tumor cells and making them have the function of normal cells. My master's project was the induction of aconite polysaccharides into the differentiation of promyelomyeloid leukemia cells, and some effects were found through studies, but not obviously. The dose used clinically should be large, and it is necessary to use until the patient's fever and yang qi come and go, in order to see a better effect.

2. Yin formation research. Sputum drinking water belongs to the category of yin and is related to the formation of tumors. Yang deficiency and yin victory will cause cold phlegm to condense, so we study the use of warm cold phlegm to treat tumors. We found that the effect of using phlegm alone to treat phlegm and drink coagulation is not necessarily good. Because "sick phlegm drinkers, when warm medicine and harmony", when aconite is used in clinical practice with sputum dissolving drugs such as tubei, banxia, quercus and tiannanxing, the efficacy will be significantly increased. We also found that when aconite alone can promote tumor cell migration, sputum drugs such as soil fritillary and quercus alone can inhibit tumor cell migration, but when aconite is combined with quercus or earth fritillary, not only the role of aconite in promoting tumor cell migration is gone, but also can enhance the phlegm dissolving effect of soil fritillary and other drugs. "Sick phlegm drinkers, should use warm medicine and it", this sentence is very reasonable, that is, the treatment of tumors can not simply reduce phlegm, nor can it be simply warmed, we have done many similar studies.

3. and yin and yang research. Hyoscya (Tianxia) Wenli, ephedra temperature table, ephedra contains ephedrine to stimulate the sympathetic nerve, hyoscya contains hyoscyamine blocks vagus nerve excitation. Sympathetic nerve and vagus nerve are antagonistic in the body, showing functional activities similar to yin and yang in the human body, so we use ephedrine to stimulate the sympathetic nerve, and use hyoscyamine to block the vagus nerve, which can lead to changes in the expression of the second signaling molecule CAMP and CGMP, thereby blocking breast cancer cells in the G1 phase and ultimately inhibiting the growth of tumors.

(Baidu Encyclopedia-莨菪: also known as "Heavenly Fairy". Bitter, cold, toxic, into the heart, stomach, liver three meridians, toxic, with epilepsy, pain relief effect, often used to treat madness, wind paralysis pain, wheezing, stomach pain, chronic diarrhea, rectal prolapse, carbuncle, vicious sores and other symptoms. 0.06~0.6g。 External use: frying and washing, powder compress or smoke. Tianxian is the best among hallucinogenic plants, first seen in the "Shennong Materia Medica", there is "eating too much makes people go crazy." Long service light body, walking and galloping, strong will, strength, and spirit." "Thousand Golden Fang" :(1) Cure stone carbuncles are as strong as stones, and do not make pus: vinegar and scopolamine seeds, put on the head. (2) Cure the sores: burn the scopola. (3) Treat the wound and broken, and there is bruising in the abdomen: the last hyosa is applied to the sore. (4) Treatment of dog poison: take seven scopola, one day. )

Chapter XXIII: Yin and Yang Studies 4

The use of ephedra to treat tumors comes from Yanghe Tang, and the "Shennong Materia Medica" records that ephedra can fight tumors and treat the accumulation of fistulas. The use of hyoscopola to treat tumors comes from the "General Record of Shengji", in which hyoscopa is used with jujube to treat "cold convulsive qi", which belongs to yang deficiency tumors.

"Typhoid Fever" uses ephedra aconite licorice decoction to treat oligoyin disease "but sleepiness", that is, sleepiness and sleepiness during the day, that is, the vagus nerve excitement mentioned in Western medicine; Huanglian ejiao soup cures insomnia at night, which is the sympathetic excitement mentioned by Western medicine. Why can ephedra cure enuresis? Because vagus nerve excitation leads to enuresis, ephedrine can antagonize the vagus nerve. Ephedra can cure allergies, such as scratches, and the skin is a white line, which is the only evidence of ephedra. How to judge yang deficiency? One of the simplest methods is the scratch experiment, the mechanism of Western medicine is vagus nerve excitation; Traditional Chinese medicine says that this is an indication for ephedra, and also because ephedrine can antagonize the vagus nerve. Sympathetic nerve excitation consumes the body, and vagus nerve excitation makes the body reserve, so sympathetic and vagus nerve are also related to yin and yang. By studying the anti-tumor effects of ephedra, we have a very clear understanding of what its mechanism is and what types of tumors it applies to.

4. Yin and yang should be studied as such. We have done studies on the relationship between seasons and ovarian cancer and found that the median survival of patients who relapsed from December to March was only 19 months; The median survival of patients who relapsed from April to November was 47 months, more than twice as good. The progression-free time for ovarian cancer to recur from December to March is only 8 months; From April to November, the recurrence is 20 months, which is more than twice as long. Why is that? Because ovarian cancer is a yang deficiency tumor. "Yang qi, if the sky and the sun, lose their place, then lose their life and do not show", Chinese medicine compares yang qi to the sun, that the cold of the human body is connected with the cold outside air, the weakest time in nature sunshine is winter, vitamin D3 levels are low, easy to lead to tumor recurrence, this is the mechanism of the corresponding view of heaven and man implemented on ovarian cancer.

5. Clinical research on the treatment of tumors by warm yang. "The foundation of life is based on yin and yang", "less yang and fire must not be moved rashly", "under the ruffian, there will be a fuyang", "those who drink phlegm should be reconciled with warm medicine", we must seriously consider the truth of these words. Our study found that patients with pancreatic cancer treated with the warm method had the shortest survival and did not survive more than 25 months. Clinically, there are many pancreatic cancer patients who show yang deficiency, such as Wumei pills. The use of Ume pills can significantly improve the symptoms of patients, but many patients' tumors are progressing, which shows that the advantages and disadvantages of dialectical treatment deserve further consideration.

Through the study of gastric cancer, we have also found that advanced gastric cancer patients treated by warm method have the shortest survival time, many of which are less than 12 months. In fact, after the clinical use of warm yang drugs, the symptoms of cold intolerance, fatigue, weight loss, poor appetite and abdominal distension improved rapidly, but most of them had a short survival period.

There are many tricks to treating tumors with the warm method. We have followed patients for many years, and after modern scientific research, our understanding of warm treatment of tumors has greatly improved. We have studied the mechanism of the warm method, and although the temperature method is also used to treat tumors, the prescription drug has changed significantly compared with the traditional warm method, and there is a lot of attention to the matching. We went from the clinic to the animal to the cell, and then we found the way and then went back to the clinic.

For example, the warm method can promote cell growth, but we use the warm method and chemotherapy drugs at the same time, first using ginseng to block the cells in the S phase, and then treating with chemotherapy drugs such as capecitabine. One promotes cell growth, the other kills cells, this is the idea of combining cold and temperature, and Chinese and Western medicine convergence. For example, a patient with ovarian cancer, the tumor was about four or five centimeters, and we used prescriptions such as rhubarb pills to stabilize the condition and control the growth of the tumor. She is also doing interventional therapy, so it cannot be said that it is completely the effect of Chinese medicine. Because she couldn't hang up, she found another Chinese medicine doctor for treatment, used ten complete tonic soup, less than 3 months, the tumor grew to 14~15 cm, the abdomen was as big as a drum, the whole body failed, and soon died. Some people will question: the patient's overall condition is very poor, why do you still use rhubarb insect pills? This question, we can think about.
