I am an ordinary nurse, working in the West China Hospital of Sichuan University for nearly 9 years, and I am also a teacher and scientific researcher, and friends around me often ask "What do nurses do besides nursing patients?" My students often ask, "Teacher, what does it look like when we graduate?" Male students will also ask, "Teacher, what are the advantages of boys?" Can only care for the sick? ”...... In the face of these questions, I have also been thinking: What is a nurse? What are nurses going to do? What can a nurse do? What do nurses need to do? What is the role of male nurses? Although these questions have not yet been fully answered, in the process of exploration, I am also looking for a development path that suits me. After graduating from Sichuan University in 2013, while engaging in clinical nursing work, thinking about clinical problems, in the leisure time of clinical work, often thinking about the needs of clinical nursing, the lack of existing medical equipment and facilities, the advantages and disadvantages of medical care technology, and after verification, put forward new design or optimization programs, and slowly embarked on the road of nursing innovative product development and health technology evaluation.

The first department I worked in after graduation was the Department of Critical Care Medicine, and when I first left the campus, I had only two impressions of the Department of Critical Care Medicine: 1. Patients hovering on the death line; and 2. Various instruments. For the safety of patients and the need to quickly adapt to my position, I spend a lot of time and energy every day to understand the equipment, instruments, consumables, etc. in the intensive care ward to understand how they are used and how they work. With the gradual deepening of the work, gradually realize that a small accessory may affect the treatment of critically ill patients, the existing equipment and instruments are not perfect, and the emergence of new problems also requires corresponding technology to solve. Therefore, under the leadership and guidance of the Department of Critical Care Medicine and the pioneers of Nursing Innovation in West China, we began to innovate nursing technology for specific clinical problems, so as to design new high-efficiency, safe and inexpensive equipment, instruments and consumables. The subsequent quality and safety management work in the nursing department and the current experience in the emergency department made me realize that small improvements in clinical nursing work may change the outcome of patients and even the fate of a family, which also made me more determined to take the path of clinical nursing innovation.

Work for nearly 9 years, presided over and researched more than 20 patents, a number of products have been listed or preparing to be listed, of which the most impressive is the infusion bag, ventilator pipeline and medical positive pressure air supply hood 3 designs. The infusion bag is the first innovative product that I participated in designing as a principal researcher, and injection and infusion is the daily work of nurses. The relevant data of the Prospective Industry Research Institute's "China's Large Infusion Industry Development Prospect Forecast and Investment Strategic Planning Analysis Report" show that the mainland's large infusion production in the past decade has been more than 10 billion bags. The adverse reactions caused by intravenous drug infusion are the most common of all drugs, and there are obvious compatibility contraindications between most drugs, and whether many drugs cause damage to the human body after entering the human blood is not yet known. In some developed countries, hospitals even inject one dose of normal saline between each infusion to avoid compatibility contraindications or interactions between drugs to reduce efficacy, so it is often seen that a patient hangs multiple infusion sets and infusion bags, and the hospital is also jokingly called "disc filament hole". In order to solve the problem of compatibility contraindications between intravenous drugs, while reducing the use of infusion bags, infusion sets, saline, etc., to ensure the safety of intravenous infusions while improving patient satisfaction, comfort and reducing staff workload and medical costs, we have designed a "mother-daughter" infusion bag, equipping each bag of liquid with a normal saline sac that can flush the infusion, so as to ensure that a normal saline rinse can be carried out conveniently and quickly after each drug infusion. Thus avoiding the occurrence of compatibility contraindications between the drugs in two infusions. The scheme has successfully applied for 1 invention patent and a number of utility model patents, and the scientific research results have been transformed, and the design has also won the first prize of the Sichuan Nursing Society Innovation and Invention Competition.

The second most impressive and longest time design is the "ventilator pipeline", mechanical ventilation is an important means of treatment for critically ill patients, ventilator and ventilator accessories (including ventilator pipes and other consumables) is an important equipment and instrument to achieve mechanical ventilation, so how to ensure the effectiveness and safety of mechanical ventilation during mechanical ventilation has become the most concerned issue for medical staff. In clinical practice, we found that the biggest risk in the use of ventilator is the problems of ventilator line blockage, disconnection and ventilator-related infection, etc. In response to these problems, we have designed a series of patents related to ventilator lines, focusing on improving the series of problems of condensate in ventilator pipelines. The project is the first medical innovative product designed and developed by me as the person in charge, applied for 3 invention patents and a number of utility model patents, some of the patents have been transformed, and won many awards including the first prize of the Chinese Nursing Association Innovative Invention Competition, the third prize of the Chinese Nursing Association Innovative Invention Competition, the special prize of the Sichuan Nursing Association Innovative Invention Competition and the Sichuan University Nursing Landmark Achievement Award.

The third and first innovative medical product to be released is the medical positive pressure protective hood. On the eve of the Spring Festival in 2020, the outbreak of the new coronavirus pneumonia (hereinafter referred to as the new crown pneumonia) epidemic, medical personnel across the country threw themselves into the fight against the new crown pneumonia epidemic, and nearly 20,000 medical personnel from multiple medical teams rushed to Hubei. Although I have not been able to go to the frontline of the fight against the epidemic, I have always paid attention to the development of the epidemic and the various problems under the epidemic. Among them, I am most concerned about the personal protection of medical staff, and it is very unpleasant to see my colleagues and comrades-in-arms face with the indentation of protective masks and the sweat flowing from their backs after fading protective clothing. Subsequently, on the major websites to find whether there is more safe and comfortable personal protective equipment, positive pressure air supply hood is a good choice, but also found that the global shortage of medical protective equipment, and the mainland when the time did not have independent research and development of medical positive pressure air supply shield, the use of medical positive pressure air supply hood on the market for imported or domestic processing and production of products, in the serious epidemic situation simply do not have enough supply, Therefore, there are not enough medical positive pressure air supply hoods on the front line of the new crown pneumonia epidemic for medical personnel to use. Isn't this the problem of the neck of the card in the field of nursing? We want to design and develop our own medical positive pressure air supply hood to provide safe and comfortable personal protective equipment for medical personnel fighting in the frontline of epidemic prevention and control, so with the help of the "Medicine + Manufacturing" Center of Sichuan University, I successfully formed a multidisciplinary team including medical engineers, health technology evaluation experts, hospital infection prevention and control experts, respiratory experts, infectious disease experts, disaster medicine/nursing experts and artists. After a rapid analysis of the needs of the frontline of epidemic prevention and control and the key design of the medical positive pressure air supply hood, after many ideas, discussions and experiments, the preliminary design was finally completed. Safe filtration efficiency meets the requirements of the frontline of epidemic prevention and control, lightweight fuselage reduces the burden on medical staff, multi-gear air supply system meets different needs, and the integration of ergonomics and aesthetics provides comfort and elegance. After the successful research and development of samples, standard clinical randomized controlled trials were quickly carried out to test the effect of the design and propose the direction of optimization and improvement, and after the clinical trial, more consideration was given to the needs of the clinical front-line, the design was improved, and finally 1 invention patent, 1 appearance patent and 4 utility model patents were declared, and the medical device registration certificate and marketing license were declared at the same time. As soon as the design was launched, it won many awards such as the iF Design Award, the Silver Award of the Tianfu Baodao Design Competition of the Sichuan Provincial Economic and Information Department, the China Design Intelligent Manufacturing Excellence Award, the Most Investment Value Award of the 7th China Medical Industry Innovation Competition, and the second prize of the Science and Technology Award of the Chinese Nursing Association. At the same time in the design process, I also found that there is a disconnect between engineering and medical treatment in medical device products, industrial standards and manufacturing processes without medical staff participation and failed to fully consider the actual clinical situation, so we strive to participate in the formulation of the "medical positive pressure air breathing apparatus" industry standards, and medical device manufacturing institutions to participate in the formulation of industrial industry standards for such products, and propose that in the development of medical and health products, production and related standards should fully consider and respect the opinions of clinical experts. This is also the first time that medical staff have participated in the development of industry standards for the manufacture and production of medical devices.

Rich experience in clinical innovation and research and development has made me more and more aware of the importance of solving clinical problems in the research and development of clinical innovative products, and also fully realize the importance of clinical medical workers in the innovation and development of medical products. As a nursing team that pays more attention to medical products and has an active thinking, male nurses will play an extremely important role in the innovation and reform of nursing technology in today's era of iterative and rapid development of scientific and technological innovation. 2021 is the first year of the mainland's 14th Five-Year Plan, which adheres to the principle of "innovation-driven development and comprehensively shaping new advantages for development", of which facing the people's life and health is one of the important directions for adhering to the core position of innovation in the overall situation of the mainland's modernization construction. Although the research and development of nursing innovative products is under the large system of medical innovative product research and development, due to the particularity of nursing work, it also has its unique attributes, and nursing innovative products often have the characteristics of low single value, large volume, small improvement, strong applicability, short research and development cycle, and high cost performance.

Nursing technology and product innovation because of the need and development, because of the use of development, in the nursing technology and product innovation and development of male nurses play a greater role, what I do is what many male nurses are doing at present, I hope that we male nurses can be like a spark of nursing innovation and invention of the spirit of the whole field of nursing. Yan Liu, Nurse in Charge of the Emergency Department of West China Hospital, Sichuan University, holds a double master's degree in evidence-based medicine/disaster nursing. He is the chairman-elect of the Male Nurse Working Committee of the Chengdu Nursing Association and the secretary of the Male Nurse Working Committee of the Sichuan Nursing Association.
