Zhang Zhili, deputy chief physician of the Department of Otolaryngology, who came to consult, noticed a different hint. In view of He Bo's condition, experts from the Department of Otorhinolaryngology of the First Hospital of Zhejiang University, together with a number of experts from neurosurgery, anesthesiology, operating room, ultrasound imaging department and other departments, conducted a detailed discussion.
In January 2022, Zhang Zhili, deputy chief physician of the Department of Otolaryngology, and Cao Fei, deputy chief physician of the Department of Neurosurgery, jointly conducted a major operation on the elderly to remove the tumor on the right skull base under the microscope.
Due to the complex anatomy of the base of the lateral skull, the lesion range is large, and the operation is carried out for a full 14 hours, covering important structures such as the internal carotid artery and the facial nerve. During the operation, the expert team exposed He Bo's temporomandibular joint, found dense granulation-like tissue inside, cut part of the tissue and sent it to the intraoperative frozen pathological examination, and the results suggested that it was squamous cell carcinoma! It was the tumor that continued to "nibble" from the ear to the deep part, invading important nerves such as He Bo's facial nerve, which caused him headaches, facial paralysis, swallowing difficulties and other discomforts. After the operation, Ho Bo's headache was relieved, but further radiation therapy was required.

Zhang Zhili, deputy chief physician, said that middle ear cancer tends to occur in 40 to 60 years old, and most middle ear cancer patients have a history of chronic purulent otitis media before the onset of the disease, which is manifested by repeated ear pus, bleeding, ear pain, hearing loss and various neurological symptoms.
Patients with otitis media should be extra vigilant against cancer
Deputy Chief Physician Zhang Zhili explained that chronic purulent otitis media is a common multi-morbidity in otolaryngology. The biggest hazards can be life-threatening in addition to affecting hearing. On the one hand, intracranial complications such as meningitis and brain abscesses may be induced. The other is cancerous, that is, middle ear cancer.
According to statistics, more than 75% of middle ear cancer patients have a long history of chronic otitis media. Therefore, patients with otitis media with recurrent chronic purulent infection should be given high attention. Persistent purulent infections and chronic irritation in otitis media can lead to the susceptibility of epithelial cells in the middle ear cavity to cancerous.
Experts emphasize that because the early symptoms of middle ear cancer are similar to the symptoms of chronic purulent otitis media and are easily confused with each other, and the appearance of middle ear cancer and the inflammatory granulation or polyps caused by otitis media are difficult to distinguish in appearance, early diagnosis is difficult, and it is easy to delay treatment. Early detection of middle ear cancer is directly related to the treatment effect and prognosis of patients.
The possibility of otitis media becoming cancerous should be highly alert when the following symptoms occur:
Chronic purulent otitis media that cannot be cured for a long time, and there are rapid recurrences after the granulation of the middle ear cavity that is prone to bleeding;
hearing loss, swelling in the ear, deep ear throbbing pain, dizziness, bleeding from the ear canal or bloody discharge;
Progressive persistent headache that is difficult to explain with inflammation;
The sudden appearance of facial paralysis, especially after several months of ear pain, is a powerful indication of middle ear cancer.
