Five and a half years ago, Jinping Miao and Yao Dai Autonomous County in Honghe Prefecture, Yunnan Province, was a state-level poverty-stricken county located in the southwest border of the motherland with a border area of 502 kilometers and an area of 99.7% in the mountainous area. Ethnic minorities in Jinping County account for 86% of the total population, and have been inhabited by ethnic minorities such as Miao, Yao, Dai and Hani for generations (including the most primitive Zhijiao ethnic groups, the Lahu and Mang people), which are the most ethnic groups in Honghe Prefecture and the county with the largest number of cross-border ethnic groups in the province.
There are the purest blue skies and white clouds, and there are also endless rugged mountain roads; there are the most swaying customs and customs, and there are many compatriots with very weak health concepts; there are the most colorful national characteristics, and there are also the dilemmas of lack of medical resources, language barriers, and different concepts...
In 2016, the first batch of medical teams from the Red House came to Jinping County People's Hospital, 2,640 kilometers away from Shanghai. In 2015, the local maternal mortality rate was 25.33/100,000, higher than the national maternal mortality rate of 20.1/100,000 that year. As the only county-level comprehensive public hospital in Jinping, the medical center of the whole county, and the "120" emergency station in Jinping County, Jinping County People's Hospital undertakes the health treatment tasks of 378,000 people of all ethnic groups in the county, some patients in neighboring counties, and Vietnamese border residents. For our medical team, to solve the safety problem of high maternal mortality in critical cases, to carry out new technologies in obstetrics and gynecology, and to protect the health of women in the frontier... It is an urgent but very difficult task.
During the assistance period, while the safety of childbirth was guaranteed and the delivery experience was improved, the local gynecological minimally invasive surgery was vigorously developed, the laparoscopic minimally invasive radical surgery for malignant tumors achieved a breakthrough of 0, and the hysteroscopic surgery was also from scratch to an average of 150 cases per year, and the medical team brought the local gynecological surgery into the "minimally invasive era", and the minimally invasive rate of gynecological surgery by the end of 2021 exceeded 90%. In order to completely solve the problem that the blood reserve of Jinping County People's Hospital is only 1000ml, the medical team also led the establishment of the first blood bank in Jinping County to provide blood source guarantee for the rescue of critically ill patients.
Today, Jinping County has long been lifted out of poverty, and the medical team has left, but what is left is the "technology, talent, and achievements" that cannot be taken away. With the help of the medical team, the framework for medical treatment of obstetrics and gynecology in Jinping County People's Hospital has been completed, and a critically ill maternal treatment center and a newborn treatment center have been established, which successfully passed the acceptance in June 2020 and the review and acceptance in July 2021. At the same time, the comprehensive medical capacity of the obstetrics and gynecology department of Jinping County People's Hospital, such as workload and technical difficulty indicators, is also in the forefront of the same level hospital in Honghe Prefecture, becoming the obstetrics and gynecology diagnosis and treatment center in the south of Honghe Prefecture, and setting a new benchmark for the health of women and children in the frontier.
5 years, is a day by day perseverance in the long-distance running relay
And all these results are the "long-distance running" jointly carried out by 11 medical teams and local medical personnel, with the original intention of rock-like, from scratch, turning one "impossible" into "possible" on the border of the motherland.
Coming to Jinping, the first thing the medical team had to solve was the "update breakthrough" of clinical technology.
In the face of the backward concept of local women's fertility concept and the weak level of hospital treatment, year after year, the medical team has gone from easy to difficult, step by step "passing, helping and leading". Tamponade balloon compression hemostasis, breechy inversion, cessation of the ascending branch of the uterine artery during caesarean section, cervical cerclage, caesarean section forceps and vaginal low forceps, painless delivery, neonatal endotracheal intubation... New obstetric technologies have been slowly carried out, building a series of guardian gates for mothers and children on the frontier, and only then have they achieved the change from medical team assistance to the present time without a single case of maternal death.
In terms of gynecology, the medical team is fully committed to helping the local gynecological team to achieve a "breakthrough of zero" in technology. Laparoscopic total hysterectomy, uterine fibroid removal, ovarian cyst removal, tubal plastic surgery, transabdominal extensive total hysterectomy (cervical cancer), transabdominal tumor body reduction (ovarian cancer), vaginal sacral fixation to preserve the uterus, congenital vaginal mold molding, old perineal iv degree laceration repair, standardized diagnosis and treatment of gestational trophoblastic disease. From simple to complex, the development of new gynecological technologies has allowed more women to seek medical treatment in the local area, solving the problem of difficulty in going out for medical treatment.
The medical team is also concerned about fellow villagers in remote areas. Jinping's green mountains and waters attract doctors from the eastern coastal cities, but the fickle weather also tests them. In 5 and a half years, cold and summer, winding roads, bad weather and even the danger of landslides cannot stop the pace of free clinics. The footprints of the Red House people cover many mountainous areas and villages, sending free clinics to Jinping and surrounding areas, benefiting more than 4,000 people.
In order to strengthen the physiological health education of adolescents in mountainous areas, during the assistance period, the hospital's "Red Pulpit" adolescent sex education team and the medical team went into the mountains to care for the growth and development of "Mountain Liwa". More than 20 lectures were held before and after, benefiting nearly 5,000 teachers and students, donating more than 3,500 books, and also forming the "Red Podium Golden Balance Detachment" and training 10 local lecturers.
It is better to teach people to fish than to teach people to fish, and at the beginning of the assistance, everyone's vision is to leave a medical team that cannot be taken away and continue to benefit local female compatriots.
In the multidisciplinary cooperation practice of many critically ill pregnant women's rescue; in the mode of repeated first-aid drills and discussion and learning of difficult and critical cases, the medical team has taught hand in hand, and has trained more than a dozen business backbones in the local area, and assisted ten local business backbones to come to Shanghai for further study. In addition to clinical and teaching work, the medical team also carried out various lectures, business learning and training, covering 4400 people before and after. 5 years is a deep memory of a critical moment
Speaking of the bits and pieces in Jinping, the members of the medical team are still vividly remembered, those warmth, friendship, touching, emergency, confusion, breakthrough... It is still remembered by everyone.
During the Dragon Boat Festival in 2016, an elderly woman with a large hematoma in the vaginal wall was transported by ambulance to jinping county people's hospital. Although the hematoma of the vaginal wall is a relatively rare complication of childbirth in obstetric clinics, if it is not found and treated in time, it can easily lead to postpartum hemorrhage and even cause the occurrence of hemorrhagic shock. 9 cm large hematoma, bleeding point is difficult to find, the hospital is not prepared for blood, life and death, in the face of unprecedented difficulties, the first medical team still withstood the pressure, handled properly, and saved the patients in time.
In May 2018, in order to rescue a rare and dangerous pregnant woman with acute fatty liver in pregnancy, the fourth medical team fought with the disease for 12 hours. There is no standardized advice, no excellent facility conditions, the condition is like a wild horse that has lost its reins, rampage to destroy the liver and kidney, coagulation, and metabolic functions of pregnant women, and at the critical moment of acidosis, hypothermia, coagulation dysfunction, and vaginal bleeding, the medical team still snatches back a life from the hands of the god of death.
In July 2018, the day after the fifth medical team reported to Jinping, they encountered a severe placental abrupt cesarean section in the obstetric department, and although the environment was not fully familiar and the teamwork was still a bit unfamiliar, they still won the "first battle". Soon after, a young girl with repeated irregular vaginal bleeding was about to take the college entrance examination but suddenly fainted, and after being treated by the medical team, she successfully entered the examination room; through a 20-minute life and death race, they also exchanged for the safety of the mother and baby for placental abruption, prenatal bleeding, and the first exposure of the umbilical cord for the pregnant mother...
On the day of the 2019 Dragon Boat Festival, a 16-year-old high-risk pregnant woman with placental abruption, severe anemia, coagulation abnormalities, early symptoms of shock, and fetal death was admitted to the hospital. The seventh team of medical teams worked together with local medical staff to save her womb and life for the young her. The postoperative multidisciplinary cooperation corrected the problems of hyperkalemia, metabolic acidosis, hypertension, and low platelets, and the young girl finally turned the corner.

Also going to Yunnan, on December 19, 2021, the obstetrics and gynecology hospital officially embarked on the journey to help build the medical team of Yongping County People's Hospital. This is the 12th batch of medical team members sent by Fudan University to Yongping County since 2016, and the five "post-80s backbone" medical team members of our hospital, Xin Weijuan, Gong Xiaohui, Gu Shouxin, Liu Danyang and Lu Yaojun, have carried out their counterpart construction work in Yongping.
