Title Interpretation:

Tibia plateau collapse fracture: the conduction of force in a high fall injury is conducted upwards along the tibia of the foot, and the gravity of the trunk is transmitted downwards, working together on the knee, combined with a lateral torsional load, which can lead to a fracture of the medial or lateral platform of the tibia. Collapse beyond a certain range (2.0-3.0 mm) can lead to a serious mismatch between the joint surfaces, accelerate cartilage wear and tear in daily activities, and produce traumatic arthritis, which seriously affects the patient's life.

Minimally invasive internal fixation: Tibial plateau collapse fracture is a reduction fixation method based on the principles of minimally invasive surgery, and the traditional method is to pry the window under arthroscopic monitoring to reduce the internal fixation of the loose bone tension screw of autologous bone graft. Balloon dilation reduction calcium sulfate bone cement filling combined with pine bone tension screw internal fixation mentioned in this study is a relatively novel minimally invasive internal fixation method for the treatment of tibia plateau collapse fractures.

Background: A metaphyseal void will appear after the reduction of the tibia plateau fracture collapse fracture, and effectively filling the gap and maintaining good support performance is an urgent problem to be solved.

ObjectiveTo compare the stability of different minimally invasive treatments for Schatzker type III tibial plateau fractures.

Methods: A total of 32 frozen upper tibia specimens were collected and randomly divided into 4 groups, the control group remained intact, and the remaining 3 groups uniformly prepared a cadaver model of fresh tibia platform Schatzker type III fracture. The standard group adopted the traditional window pry to reduce the internal fixation of the autologous bone grafted loose bone tension screw, the simple bone cement group adopted balloon expansion reduction calcium sulfate bone cement filling treatment, and the bone cement screw group used balloon expansion reduction calcium sulfate bone cement filling combined with cancellous bone tension screw internal fixation. The displacement and stiffness of the fixed fracture block under 350 N load, the load and stiffness at 3 mm displacement, and the displacement under the maximum load were compared; the position of fractures, cement and screws was observed.

Results and conclusions: (1) Under 350 N load, the inter-group displacement comparison was &lt; the control group &lt; bone cement screw group &lt; the simple bone cement group &lt; standard group, and the difference was significant (P&lt; 0.05); the comparison of stiffness between the groups: the control group &gt; bone cement screw group &gt; the simple bone cement group &gt; standard group, and the difference was significant (P &lt; 0.05) ;(2) When the displacement between groups was 3 mm, the load comparison between the groups was &gt;: bone cement screw group &gt; control group &gt; The difference was significant in the standard group &gt; the simple bone cement group (P &lt; 0.05); the stiffness between the groups was compared: the bone cement screw group &gt; control group &gt; the simple bone cement group &gt; standard group, and the difference was significant (P &lt; 0.05) ;(3) suggesting that calcium sulfate bone cement injection in the treatment of tibia plateau collapse fractures had good filling ability, and combined screw fixation could provide better support and higher stability than traditional reduction internal fixation. This shows that balloon dilation reduction can be injected with calcium sulfate bone cement filling for the treatment of tibia plateau collapse fractures with good clinical application value.
