Pathogenesis of "pimples"

"Pimples", also known as acne, is a chronic inflammatory skin disease that occurs in the hair follicle sebaceous glands, mainly in adolescents, but is not completely limited by age. The occurrence and development of acne is closely related to the large secretion of sebum under the action of androgens, changes in the composition of sebum, abnormal keratosis of hair follicle sebaceous glands, colonization of Propionibacterium acnes, inflammatory response and immunity. Blockage of the hair follicle sebaceous glands is the initial factor that leads to acne. Abnormal desquamation mixes with filaments and lipid droplets to build up, forming micro-comedones. Hair follicles are filled with lipids, bacteria, and keratinous detritus, showing whiteheads or/and blackheads that are visible to the naked eye. If Propionibacterium acnes proliferates and produces inflammatory mediators, inflammatory papules, pustules, nodules, and granulomatous lesions develop.

Typical manifestations of "pimples"

The first step to defeating "pimples" is to understand their own acne situation, so as to prescribe the right medicine.

We can classify acne as a grade IV depending on the severity of the lesions:

Whitehead acne: the hair follicle mouth is accumulated and blocked by keratinocytes, and the sebum is filled in it, forming a white keratinized papule, and the surface is covered with epidermis and is not communicated with the outside world. This is mild acne, which is grade I acne.

Blackhead acne: Pore-clogging sebum is directly exposed to the air and oxidized, forming blackheads. This is mild acne, grade I acne.

Papules: small, reddish bumps, which are moderate acne, or grade II acne.

Pustules: Pustules appear at the apex of the papule, which are moderate acne, or grade III acne.

Nodules: hard and painful lumps on the surface of the skin, which are severe acne, that is, grade IV acne.

Cyst: A lump with pus that painfully touches under the surface of the skin, which is severe acne, or grade IV acne.

"Acne" topical drug treatment

Acne is the result of a combination of pathogenic factors, and clinically it is mainly for the treatment of excessive sebum secretion, abnormal hair follicle epithelial desquamation, proliferation of Propionibacterium acnes and inflammatory reactions. This paper selects several common and clinically used topical drugs, introduces their indications and specific use methods, and helps patients use drugs correctly to achieve better treatment results.

01 Tretinoic acid class "player" one adapalene gel

★ Drug effect

(1) Improve hair follicle sebaceous ductal keratosis and reduce the formation of acne

(2) Dissolve micro-acne and acne

(3) Anti-inflammatory

(4) Prevent and improve the effects of pigmentation and acne scarring after acne inflammation

★ Indications

Closed mouth acne, blackhead acne, red and swollen acne, acne marks, acne pits

★ How to use and precautions

(1) Tretinoin drugs are easy to decompose when seen in light, and it is best to use them at night and wash them off in the morning.

(2) Adapalene has skin irritation, at the beginning of use, some patients may have symptoms of erythema, scaling, itching and discomfort, and the symptoms of skin lesions may be more serious, which is not aggravation of the disease, but the drug is working. The drug should be tried for a small area for 3-5 days, which can be applied on a small area behind the ear or on the cheek, and after making sure that it is not irritating to the skin, then start applying it in a large area. If the irritation reaction is severe, stop taking the drug and do not apply it to the mucous membranes such as the mouth and eyes.

(3) Establish tolerance: you can take a short-term contact method to establish tolerance, such as the first time to start a large area of application, you can wait for 5-10 minutes after the immediate washing of water, continuous use for 2-3 days, and then try to wash off with water after 10-20 minutes, after 2-3 days can try to wash off after 20-30 minutes, and so on, until you can apply overnight, generally continuous medication for about 4-6 weeks can establish tolerance.

(4) During the use of Adapalene gel, a thin layer can be applied, and too thick application will not enhance the efficacy of the drug, but also increase the drug stimulation.

(5) Do not use it for women who are trying to conceive, pregnant or breastfeeding.

02 Antibacterial "player" peroxide benzoyl gel

(1) Inhibit excessive keratosis of the hair follicle mouth and reduce acne formation

(2) Kill propionibacterium acnes

(1) Newly formed acne

(2) Acne that occurs immediately

(1) The oxygen free radicals released by benzoyl peroxide can cause the inactivation of vitamin A drugs, and should not be used together. Benzoyl peroxide gel can be applied in the morning and adaparene gel in the evening.

(2) When benzoyl peroxide gel is used, it can only be used in the form of spot coating, and only a small amount can be taken on the fingertips, kneaded into a thin layer, and pointed at the acne site. When using it for the first time, it is best to wash it off after half an hour, and after a few uses, gradually increase the residence time of the drug, so that the benzoyl peroxide gel can stay on the acne for at least 2 hours (the method of establishing tolerance is similar to Adapalene), and never apply to normal skin and mucous membranes.

(3) After use, the skin may dry out and peel off, and a thin layer of moisturizing repair lotion can be applied to the affected area to repair the skin barrier damaged by the benzoyl peroxide gel.

03 Antibiotic category "player" fusidic acid ointment gel

Anti-acne propionibacterium, anti-inflammatory

Suitable for severe acne, superficial acne lesions such as papules and pustules

(1) Cotton swab dipping, local dotting, gently kneading, morning and evening, it is recommended not to use continuously for more than 14 days.

(2) Fusidic acid is an antibiotic drug, which is prone to drug resistance, and it is not recommended to use it alone or for a long time. It can often be used in combination with adapalene gel and benzoyl peroxide gel. General evening adapalene gel, morning fusidic acid cream. If you add benzoyl peroxide gel, it is adapalene gel at night, a single-day benzoyl peroxide gel in the morning, and a double-day fusidic acid cream.

(3) If used in combination with adapalene gel and benzoyl peroxide gel, it should be noted that both ointments have irritation, of which benzoyl peroxide gel stimulates greater. Therefore, it is necessary to establish good tolerance and moisturizing when using.

(4) Do not use it for women who are trying to conceive, pregnant, or breastfeeding.

04 Antibiotic category "player" monomethanidazole gel / clindamycin metronidazole liniment

Red, swollen acne, large outbreaks of acne that may be combined with rosacea

(1) Clindamycin metronidazole is irritating and is not recommended for people with sensitive skin.

(2) Metronidazole is an antibiotic drug, which is easy to produce drug resistance, and it is not recommended to use it alone or for a long time.

(3) Simultaneous use of metronidazole gel with other drugs may produce drug interactions, so when you need to use other drugs when you are using this drug, you should inform your doctor.

(4) Women who are trying to conceive, pregnant, and breastfeeding should ask a doctor for guidance.

P.S. For ointments that are irritating or allergic, they need to be applied on a small area behind the ear or on the cheek, and after making sure that it is not irritating to the skin, start applying a large area. In the early stage of use, you can also apply a thin layer of lotion to the affected area for half an hour before using the ointment, which can reduce the irritation of the ointment to the skin.
