On the evening of the lunar Chinese New Year's Eve on January 24, 2020, two messages of renewed requests for battle appeared on the mobile phone of Zhang Zongjiu, head of the medical treatment team of the Central Guidance Group and director of the Medical Administration of the National Health Commission, and it was Ma Xudong, deputy director of the Medical Quality and Evaluation Department of the Medical Administration bureau, who had just returned to Beijing from the frontline of the Wuhan anti-epidemic campaign a day ago.

Go straight to the front line to upgrade "prevention" and "governance"

On January 12, 2020, Ma Xudong, as a staff member appointed by the National Health Department, went to Wuhan to carry out epidemic prevention and control and medical treatment. At that time, in the early stage of the development of the epidemic, the setting and management of fever clinics in medical institutions in Wuhan were not standardized, the number of fever patients increased rapidly, and the characteristics of the disease and effective medical means were not clear... A series of issues have put enormous pressure on the working group ahead. Therefore, although it has not yet been determined whether the virus has been transmitted from person to person, Ma Xudong does not dare to slack off at all, he knows that in the prevention and control of infectious diseases and the treatment of patients, time is life, and every minute of delay increases the risk of patients' life safety.

To find and solve problems, you must go to the front line. As soon as he arrived in Wuhan in the early morning of the 13th, Ma Xudong decided to shift his center of gravity downward and go deep into the front line to carry out down-to-earth work. On the same day, he accompanied the experts to Tongji Hospital, Union Hospital, Hubei Provincial People's Hospital, Wuhan Fifth Hospital, Sixth Hospital, Puren Hospital and other hospitals with more fever patients to conduct research. "How many people are currently in daily fever clinics?" What is the admission process for patients with fever? How many patients are in the hospital? Does the hospital have an infection ward? How to set up three zones and two channels? "When did you come to the hospital and have you been to the Huanan Seafood Market?" Does your family have a fever and cough? ”...... At each hospital, he would go to the fever clinic and isolation ward to communicate with medical staff and patients.

Through several consecutive days of research and under the unified deployment of the leaders of the front group, Ma Xudong established a daily reporting mechanism for fever patients in medical institutions in Wuhan, and closely monitored the changes in the number of fever patients in different regions every day. In the monitoring, a situation attracted his attention: in less than a week, the daily fever volume of more than 60 medical institutions in Wuhan rose from more than 1,600 to nearly 5,000. According to the development of this trend, the next fever clinic may have a sharp increase in patients, and the upgrade of prevention and control cannot be delayed! Ma Xudong immediately made a decision to timely guide provinces, cities and relevant medical institutions to strengthen the transformation of fever diagnosis, reserve resources and reserve space, and cope with possible situations.

With the development of the epidemic, the capacity of medical institutions to receive and treat is gradually difficult to meet the needs of patient admission, Ma Xudong actively coordinated and participated in the "7 7" designated hospital setup and transformation work, providing a guarantee for the timely admission and treatment of patients. At the same time, in view of the large number of patients with pneumonia of unknown cause in the early stage, Ma Xudong actively coordinated the nucleic acid detection and diagnosis of fever patients in Hubei Province and Wuhan City, established a daily reporting and monitoring mechanism for patients with positive nucleic acid tests, paid close attention to and analyzed the contact history, population characteristics, regional distribution and onset time of infectious diseases of confirmed patients, and timely discovered the epidemic situation of diseases outside Wuhan such as Xiaogan, providing timely and reliable data support for the overall prevention and control policy formulation.

With the increasing number of fever patients, the detection capacity has also become the bottleneck of pneumonia diagnosis, he actively cooperated with the coordination of nucleic acid detection kits to decentralize medical institutions, greatly improving the testing capacity of Wuhan, and winning time for a large number of patients to be diagnosed and treated early.

Ordered to rest and sit still

After fighting for many days, Ma Xudong received telephone messages from colleagues and mothers, and at this time, he had just visited another hospital president in Wuhan who had been diagnosed with the investigation in the early stage. In the face of the three-day illness of colleagues around him who fell one after another and his son had a high fever for 3 days, and then looked at the difficulties faced by the newly opened designated hospital and nucleic acid testing work, he hung up the phone and continued to devote himself to the fight against the epidemic.

On January 22, the first day that Hankou Hospital, Red Cross Hospital, and Wuhan Seventh Hospital were identified as designated hospitals and began to treat patients with new coronary pneumonia, Ma Xudong accompanied the leaders to three hospitals to investigate, looking at the full outpatient hall, facing a family member who kept pouring out the illness of his octogenarian mother with a film, Ma Xudong's eyes filled his eyes with tears.

On the same day, Ma Xudong developed cough, diarrhea and other symptoms, checked his body temperature of 37.1 ° C, and tested negative for nucleic acid. After Director Zhang Zongjiu knew about it, he specially called to express his concern and condolences and arranged for him to go home to rest. On January 23, Ma Xudong returned to Beijing and rushed back to his unit to participate in the special work meeting before he could go home, providing forward information and suggestions for relevant experts to participate in the World Health Organization's study of PHIC (International Public Health Emergency of International Concern, referred to as PHIC). After returning home and isolating himself, looking at the news on the news, the eyes of Wuhan patients and their families appeared in front of him, he could not sit still, eat, sleep, and the next day, Chinese New Year's Eve night, he once again made a request to the leader to let him return to the front line to continue fighting.

Return to the battlefield to promote an autopsy

The leader asked him to rest for a few more days, but his desire to return to the battlefield many times a day made the leader see his determination to fight the epidemic. On the sixth day of the first lunar month, he finally stepped on an empty train and returned to Wuhan again.

Set up designated hospitals for the treatment of severe diseases, transport seriously ill patients overnight, actively collect information for analysis and judgment, establish a medical quality control mechanism, and promote autopsy and pathological research... Back in Wuhan, he was busier than before, sleeping an average of no more than 4 hours a day for weeks in a row, and staying up all night was a common occurrence. Recently, he has been responsible for coordinating the promotion of autopsy and pathology research on new crown pneumonia, and he has been busy.

Autopsy and pathological research are the key links to clarify the pathogenesis and death mechanism of the virus, and Wuhan, as the most serious epidemic area, is facing great difficulties in autopsy and pathological research.

In order to figure out the pathogenesis and death mechanism of the new crown pneumonia as soon as possible, and to do his best to save the lives of the people, he worked day and night: organizing special meetings, conveying the spirit of national documents, dispatching a team of pathology experts, determining the place of autopsy examination, allocating autopsy protective materials, and encouraging the mobilization of body donations... As of February 26, under his unremitting efforts, a total of 12 cases of general autopsy and 7 cases of minimally invasive autopsy were successfully organized by a team of experts, and the fourth expert exchange meeting on autopsy work was successfully held, and a preliminary opinion draft of "Expert Consensus on Pathology of New Crown Pneumonia" was formed.

At 1:30 a.m. on February 27, the lights in Ma Xudong's room were still on, and he was sorting out the preliminary opinion draft of the "Expert Consensus on the Pathology of New Coronavirus Pneumonia" submitted by the pathologist team overnight, and at the same time writing the "Report on the Autopsy and Pathological Research of the New Crown Pneumonia", which was sent to the desk of the relevant leading comrades of the central government after review at noon the next day. Its content on pathological research was eventually absorbed into the next edition of the diagnosis and treatment plan to guide the medical treatment of COVID-19 patients across the country.
