Expert: 17-year-old Zhang Zhijie has a high probability of sudden cardiac death, and it is not suitable to exercise when the temperature is too high and the humidity is too high

On July 1, the Asian Badminton Federation and the Indonesian Badminton Association issued an announcement saying that in the mixed team group stage of the 2024 Asian Youth Badminton Championships held in Indonesia, Zhang Zhijie, a 17-year-old player of the Chinese badminton team, suddenly fainted and died after being sent to the hospital.

After the tragedy, the Asian Badminton Federation and the Indonesian Badminton Federation wrote in the announcement that the tournament doctor and medical team rescued Zhang Zhijie, who was taken to the hospital by ambulance in less than two minutes. However, there are many people who question the treatment process of the event organizers.

Liu Li, deputy director of the emergency department of Renji Hospital affiliated to Shanghai Jiao Tong University School of Medicine, said in an interview with Jiefang Daily and Shangguan News that the beginning and end of the unfortunate incident can only be traced back from a video of the competition. In the video, Zhang Zhijie had an accident during a men's singles match with a Japanese player, and the situation on the court was stalemate. At 0 minutes and 27 seconds in the video, Zhang Zhijie suddenly fell forward to the ground.

After being tied by his opponent, Zhang Zhijie walked around to wipe his sweat and didn't see anything unusual, and then passed the badminton ball to his opponent to prepare to receive the serve, but in the process (video time is 0 minutes and 27 seconds), he suddenly fell forward to the ground, and then his body began to contwitch.

At 0 minutes and 34 seconds, a man wearing a red T-shirt and suspected of being coach Zhang Zhijie walked into the room to observe. Zhang Zhijie was still conscious at the time, raised his head, and supported the ground with his hands, but he couldn't get up. The referee then motioned for the man to leave the field. At 1:07, medical personnel entered the venue. As soon as the man left the scene, he called the medical staff.

At 1 minute and 7 seconds, medical personnel entered the venue and conducted a simple examination of Zhang Zhijie; In 2 minutes and 06 seconds, the stretcher was sent into the venue, and in 2 minutes and 13 seconds, the medical staff signaled to help, and then 2 volunteers and 4 medical personnel carried Zhang Zhijie onto the stretcher; At 2 minutes and 26 seconds, Zhang Zhijie was carried off the field.

According to the analysis of this video, Liu Li said that Zhang Zhijie had tics and falls after falling, "My colleagues and I generally believe that the symptoms triggered by cardiac disease may be considered to be different from the convulsive state during a seizure." ”

Liu Li said that due to the patient's young age and no information on the history of the underlying disease, it may be caused by malignant arrhythmia Adams-Stokes syndrome (cardiogenic cerebral ischemic syndrome), which can lead to cardiac arrest.

"Generally speaking, sudden death is closely related to cardiovascular and may also be cerebrovascular accident. Although such cases are rare in young people, sudden intracerebral hemorrhage and ruptured aneurysm cannot be completely ruled out. Liu Li said, but in terms of probability, sudden cardiac death is still the most important consideration, "then, cardiac resuscitation is the most critical part of rescue." ”

In the rescue, there is a saying of "golden four minutes", the longer the incident drags on, the lower the probability of successful rescue. "In recent years, AED equipment should be equipped in public places, especially in such sports venues, and in the event of sudden cardiac death, cardiopulmonary resuscitation should be performed immediately on the spot, and AED first aid should be activated at the same time."

Liu Li said that there is no follow-up information in the current video, so it is impossible to judge whether the rescue measures have kept up, "If there is cardiopulmonary resuscitation and AED treatment within a few minutes, and there are emergency personnel to open the airway or endotracheal intubation, it is a rescue with a composite standard process, then if there is no rescue back, it may be a pity; If there is a lag in the on-site rescue time, then even if the standard process is followed, there may be shortcomings and delays. ”

Liu Li said that in recent years, more and more young and middle-aged people have devoted themselves to sports, but sudden deaths have occurred from time to time, and they have a lot to do with the amount of exercise. "Generally speaking, we can judge from heart rate, subtracting age from the maximum heart rate. If you are 20 years old, you can use 220-20, that is, the maximum heart rate is about 200 beats, then in exercise, the maximum heart rate is 40% to 55%, that is, low-intensity exercise, such as slow walking, etc., and you will not feel uncomfortable if you continue to exercise; The maximum heart rate is between 55% and 75%, that is, moderate-intensity exercise, such as jogging, moderate-speed running, etc., which may be difficult to adhere to for a long time; The maximum heart rate is above 75%, that is, high-intensity exercise, such as brisk running or other intense exercise, etc., it is necessary to make appropriate and reasonable adjustments according to the individual's physical condition, considering whether there is recent illness, fatigue, nutritional status, etc. She emphasised that, unlike athletes, 10 to 30 minutes of moderate-intensity exercise can also benefit the general public, and it does not have to be long and high-intensity.

"For some people with diabetes, about 150 minutes of moderate-intensity aerobic exercise a week can improve physical function, lose weight, and have cardiovascular benefits; However, if this burden is exceeded, if you have cardiovascular disease, it may not be suitable. Liu Li said that especially in the recent high temperature and Huangmei season, high temperature and humidity will have an impact on the cardiovascular system. In recent years, we have also treated some patients who suddenly fainted during exercise and were sent by passers-by, and advised the general public not to exercise during the period when the temperature is too high and the humidity is too high, and if the body has issued an 'alarm', do not be reckless, and resume exercise after a thorough recovery and sufficient rest. ”
