In the early morning of May 5, 26-year-old patient Wen Moumou was admitted to Zunyi Hospital of Traditional Chinese Medicine due to severe multiple injuries. At the time of admission, the patient's condition was critical, and his life was in danger due to multiple injuries such as traumatic wet lung, bilateral fluid pneumothorax, bilateral rib fractures, renal contusion with perirenal hematoma, posterior peritoneal hematoma, and head injury. After emergency treatment, the patient was transferred to the ICU, but his condition was still difficult to control, the oxygenation index continued to be below 80mmHg, the hypoxia was severe, and the heart rate was increased.

Liu Xiaofei, director of the ICU, considered that the patient had severe traumatic wet lung and acute respiratory distress syndrome that oxygenation could not be maintained, and there was an indication for the support of artificial extracorporeal membrane lung (ECMO), and the abdominal cavity injury was severe, so emergency abdominal surgery was urgently needed. Kong Weijun, vice president in charge, immediately organized multidisciplinary MDT joint diagnosis and treatment, and the consultation made it clear that V-V EMO supportive treatment would be performed by the ICU to improve the patient's hypoxia, and the anesthesiology department would provide intraoperative protection and surgical emergency treatment. In the operating room, the patient's oxygen saturation improved immediately after the ECMO was successfully applied. After multidisciplinary collaborative surgery, the patient's multiple intra-abdominal mesenteric injuries and liver rupture were properly managed. From initiation to weaning, ECMO was guided and helped by Tian Renbin, the head of the national ECMO training base, a member of the expert group of cardiopulmonary bypass and ECMO of the National Center for Quality Control of Cardiovascular Diseases, and the deputy chief physician of the ECMO technical director of the Affiliated Hospital of Zunyi Medical University. After the operation, the patient was transferred back to the ICU to continue life support monitoring treatment.

With the support of ECMO for up to 96 hours, the ICU combined with traditional Chinese medicine characteristic treatments, such as naso-intestinal tube administration of "Taohe Chengqi Tang and Ginseng Gui Drink Plus and Minus", and rectal infusion of "Wenyang Turbidity Reducing Liquid", etc., the patient's pulmonary lesions and oxygenation were significantly improved, intra-abdominal pressure was reduced, and gastrointestinal function was restored. After a comprehensive evaluation, the patient gradually reduced the intensity of ECMO support, and finally was successfully weaned on May 10. At present, the patient is conscious, his vital signs are stable, and he is recovering.

Kong Weijun, vice president of Zunyi Hospital of Traditional Chinese Medicine: "This is a successful case of multidisciplinary collaborative diagnosis and treatment, application in ECMO, and integration of traditional Chinese and Western medicine. This successful treatment demonstrates the ability of Zunyi Hospital of Traditional Chinese Medicine in the treatment of trauma and critical care. In the future, the hospital will further deepen the medical services of traditional Chinese and Western medicine and provide better medical services for the people of Zunyi. ”
