Xiaohan was rushed to Xinhua Hospital due to severe anemia and pancreatic mass. After comprehensive examination, Xiaohan had a total pancreatic mass, abnormally rich blood supply to the tumor, splenic vein compression, increased regional portal pressure, common bile duct compression, intrahepatic bile duct dilation, abnormal liver function, and tumor indicators showed that the child had a significant increase in gastrin. The medical history found that Xiaohan had no obvious history of hematemesis or blood in the stool.
Why is there such severe anemia? What kind of tumor is a pancreatic mass? What is the relationship between anemia and tumors? The results of genetic testing showed that all the VHL alleles of Xiaohan were mutated, and the No. 3 exon derived from the mother's VHL gene had a termination mutation, which was the main pathogenic mutation point and belonged to the high-risk group. Mistranslation mutations in exon 1 derived from the paternal VHL gene have unknown significance.
According to experts, the VHL gene, also known as the Von Hippel-Lindau gene, is an important gene located on chromosome 3. When this gene is mutated, it causes abnormal blood vessel growth, which can lead to tumors throughout the body. The disease is inherited dominantly, and the patient's first or second degree relatives are in a high-risk group and require close lifelong follow-up.
So far, the truth of Xiaohan's etiology has been revealed: it is a pancreatic endocrine function neuroendocrine tumor (gastrin type) caused by VHL gene mutation, high-risk group.
According to medical statistics, the incidence of pancreatic endocrine tumors is about (1-4)/100,000, accounting for only 1%-2% of pancreatic tumors, and patients with familial background like Xiaohan are even rarer, of which VHL disease accounts for only 10%-17% of such patients.
After completing a cycle of neoadjuvant chemotherapy for Xiaohan, the experts found that the tumor size remained largely unchanged, and total pancreatectomy became the only option. Because of the difficulty of the operation and the complexity of the child's condition, the hospital organized several multidisciplinary consultations and formulated a detailed surgical plan.
Before the operation, the Department of Pediatric General Surgery used 3D reconstruction technology to accurately measure and plan the key parts of the child, such as the pancreas, splenic vein, and superior mesenteric vein. The operation took 12 hours, and the total pancreatectomy was successfully performed, and Xiaohan turned from critical condition to safety, and his recovery was good. In the future, Xiaohan needs to regulate blood sugar independently through micropump insulin replacement, and carry out follow-up treatment and rehabilitation.
According to experts from the pediatric solid tumor team of Xinhua Hospital, the treatment of pediatric solid tumors is very difficult and requires close cooperation of multiple disciplines.
