Androgenetic alopecia

In recent years, the incidence of hair diseases, especially various types of hair loss, has been increasing year by year. There are about 250 million people with hair diseases in mainland China, of which androgenetic alopecia and alopecia areata account for more than 95%.

Androgenetic alopecia, also known as seborrheic alopecia (AGA), is the most common type of hair loss and is a hair loss condition that begins during puberty or late adolescence with the miniaturization of hair follicles. It can affect both men and women, with a prevalence of about 21.3% in men and 6% in women.

The disease is a polygenic recessive genetic disease with a genetic predisposition, and the cause is not fully understood, and 53.3%-63.9% of patients have a family history of hereditary inheritance. Androgens play a decisive role in the pathogenesis of the disease. Inflammation of hair follicles, emotional problems (such as increased stress, tension and anxiety, poor lifestyle and eating habits, etc.) can aggravate the symptoms of AGA.

Manifestations of androgenetic alopecia

Androgenetic alopecia generally begins during adolescence and is characterized by progressive thinning of hair diameter, decrease in hair density, and hair loss, until varying degrees of baldness, often accompanied by increased scalp oil secretion.

In the early stage, the hairline of the forehead, bilateral frontal horns, and bilateral sideburns is receding, or progressive hair loss on the top of the head, with increased vellia hair at the hairline and an enlarged exposed area of the scalp, usually accompanied by symptoms of increased scalp oil secretion.

The main manifestations are diffuse thinning and fine hair between the top of the head and the hairline, the scalp is not fully exposed at the hair loss, and the position of the forehead hairline does not change, which is also accompanied by the symptoms of increased scalp oil secretion.

There is a detailed grading scale for androgenetic alopecia in women, which is divided into 5 grades by the width of the central part of the hair loss area:

Class I: Normal.

Level II: Widening of the median divide.

Grade III: Translucent bands between the widened median line and the hairs on both sides.

Grade IV: Enlargement of the bald area along the anterior side of the median divide.

Grade V: Late baldness.

Conventional treatment of androgenetic alopecia

(1) Topical treatment: topical use of 5% minoxidil.

(2) Systemic therapy: oral finasteride (male only), spironolactone, cyproterone, etc. are commonly used in female patients.
