"I have lost almost 100 pounds in the past 7 months, from more than 260 pounds to 160 pounds now!" The patient, Mr. Gao, told reporters. In order to welcome the arrival of the "5.11" World Obesity Prevention and Control Day, popularize the correct knowledge of obesity, and provide appropriate and effective weight management programs, on May 10, the Third Affiliated Hospital of Sun Yat-sen University (hereinafter referred to as the "Third Affiliated Hospital of Sun Yat-sen") held a free weight loss clinic activity of "Weight Loss Surgery, Surgical Writing Health", and a number of senior professional doctors answered questions for residents on the spot.

For patients weighing more than 260 pounds, surgical weight loss and slightly obese people are not recommended for treatment

"Diabetes, hypertension, hyperlipidemia, fatty liver...... I have all of these obesity-related diseases. Affected by family genetic factors, Mr. Gao weighed more than 260 pounds, was in poor physical condition, had poor sleep quality, and suffered from obesity for six or seven years.

Mr. Gao's grandparents and grandchildren have been football players for three generations, and obesity has stopped him from exercising. Last year, Mr. Gao learned about the Bariatric Center of Zhongshan Group of Hospitals, and completed the bariatric surgery of laparoscopic sleeve gastrectomy under the guidance and recommendation of the expert team, and after the operation, he lost about 100 pounds in 7 months according to the weight management plan. In his opinion, his mental state and quality of life have improved significantly.

"In terms of the effect of weight loss, the effect of surgery for weight loss is the strongest." Professor Chen Yanming, vice president of the Third Hospital of Sun Yat-sen and director of the bariatric center, told reporters that according to China's guidelines for the surgical treatment of obesity and type 2 diabetes, body mass index (BMI (weight/height 2) is an important evaluation indicator. He reminded that in fact, not every patient is suitable for medical or surgical treatment, and scientific and standardized hierarchical management is more important.

Surgery is recommended for simply obese patients without diabetes mellitus, with a BMI of ≥ 32.5, and for obese patients with type 2 diabetes, with a BMI of ≥ 27.5, and for patients with a BMI < 25.0, surgery is not recommended.

"Now I see a kind of 'strange circle' in the clinic, people who are really fat do not necessarily come to the doctor, but many people who are slightly fat say that they want to lose weight as soon as they come." Chen Yanming said that don't have too strict requirements for your body, and learn to evaluate yourself. Judging from the current clinical and large-scale population data, some people who are mildly overweight and have a BMI between 25-26 live the longest.

Bariatric surgery ≠ scientific health management of liposuction is the key to weight loss

When it comes to losing weight through surgery, many people think that "bariatric surgery = liposuction". In fact, bariatric surgery is an active intervention for normal tissues and organs, by changing the structure of the gastrointestinal tract in the body, changing the human body from multiple aspects such as appetite, food intake, absorption, hormone regulation, etc., so as to achieve weight loss effect.

"A lot of people have misconceptions about bariatric surgery." Chief physician Chen Tufeng, deputy director of the bariatric center of the Third Hospital of Sun Yat-sen, said that everyone will worry that the operation will bring physical harm, but in fact, bariatric surgery has quite high requirements for safety and efficacy, and its surgical risk is far lower than that of some conventional surgeries.

Chen Tufeng introduced that the two main bariatric surgeries currently used in China are minimally invasive surgeries, which only use 3-4 small holes of 0.5-1.0cm in the abdominal wall and are completed under laparoscopy. One is laparoscopic sleeve gastrectomy and the other is laparoscopic gastric bypass, which exerts weight loss and metabolic improvement by reducing gastric volume, reducing the secretion of hormones that produce hunger, and reducing the volume of the stomach and the area of nutrient absorption in the small intestine, respectively. "After the surgery, the patient recovers relatively quickly, and is able to get out of bed and move and eat normally the next day."

"Weight management needs to be comprehensively managed, first of all, based on the premise of a healthy lifestyle, including diet, exercise, and behavioral interventions." Chen Yanming told reporters that weight loss is a multidisciplinary and comprehensive problem, covering endocrine, mental health and other aspects. Bariatric surgery is just the beginning, and controlling diet, exercising properly, and adjusting your schedule are all essential and important parts of the weight loss process.
