85-year-old Xie Damiang of Xishan Village, Shadi Town, has been bedridden for nearly a year due to hypertension and sequelae of cerebral infarction. The "knock on the door" carried out the "four ones" free health services for the disabled elderly in the jurisdiction, namely: carrying out a door-to-door health management, providing a set of door-to-door health services, opening a health consultation hotline, and helping to solve a facility for the elderly.

In order to further accurately connect and meet the health service needs of the disabled elderly in the jurisdiction, and focus on the "urgent, difficult, and longing" problems of the elderly in the field of health and health, since the beginning of this year, Ganxian District has formulated the "Implementation Plan for Carrying out the "Health Knocking and Warm Heart Service Action" for the Disabled Elderly" and a list of service items. The "Knock on the Door" door-to-door health management includes health status assessment, physical examination, TCM constitution identification and health care guidance, etc., and the establishment of a file for the disabled elderly who are moderately dependent on their self-care ability and unable to take care of themselves. Door-to-door health services include nutrition and rehabilitation nursing guidance, integration of medical care and elderly care, psychological comfort, oral health, medical referral suggestions, emergency assistance and other health services. The health consultation hotline is a family doctor who regularly takes the initiative to contact the disabled elderly or their guardians through telephone, WeChat groups, and the issuance of warm-hearted service contact cards, etc., to understand the health status and health service needs of the disabled elderly, push targeted health information free of charge, and answer the health consultation raised by the disabled elderly or their guardians in a timely manner. So far, the inspection team of the District Health Commission has carried out patrol supervision and guidance to 24 township health centers, held a total of 25 training courses for health service teams, carried out door-knocking and warm-hearted services for 1,653 people, and issued 1,210 warm-hearted service contact cards and 5,620 sets of health education leaflets.
