Experience project 1: non-invasive ventilator

Experiencer: Zhang Mingming

Experience and feeling: I have brought countless non-invasive ventilators for patients, but today I realized that it is not so simple. The huge airflow impact of the ventilator, as well as stomach discomfort, chest fullness, dry mouth, etc., are difficult to adapt. Because I was worried about air leakage, the mask was fixed too tightly, and there was a feeling of suffocation, which made me inexplicably irritable, and I always wanted to take off the ventilator. After only twenty minutes, I couldn't hold on, leaving visible marks on my face and a lot of pain above my ears.

Improvements: Teach patients to cooperate with precautions, choose the right mask, do a good job of protecting the patient's skin, and be more patient when nursing.

Experience project 2: endotracheal intubation

Experiencer: Wang Jingbo

Experience feelings: Endotracheal intubation is really uncomfortable, and this time it is already very uncomfortable just by inserting it into the mouth. Saliva swirls in my mouth, I can't swallow it even if I want to, my mouth is firmly fixed by the gas plug retainer, I feel nauseous, and my face feels numb when my mouth opens. There was only one thought, and that was to extubate. But his hands were still restrained, he couldn't move, he couldn't speak, he could only passively wait and endure...... I can't shout if I want to, I can't struggle if I want to, and I feel helpless......

Improvement: From the psychological point of view, we can better understand the feelings of tracheal intubation patients such as "irritability" and "non-cooperation with treatment", and improve the fixation method in the work, release the restraint as soon as possible, operate gently, pay attention to the patient's feelings, and try to make the patient comfortable and reduce the pain.

Experience item 3: Gastric tube placement

Experience: When the gastric tube is placed in the nasal cavity, it feels that it is not easy to breathe, and when it reaches the throat, it is the most uncomfortable, involuntary nausea and tears, and involuntary desire to resist, if the resistance is fierce, there will be a risk of injury. The nose strip is fixed on the wing of the nose and feels that the nose is being dragged all the time, which is very uncomfortable.

Improvement: Before intubation of awake patients, explain to them, operate gently during intubation, ask patients to take deep breaths, talk to them, divert attention as much as possible, and reduce discomfort. Awake patients should drink water and swallow and enter the gastric tube at the same time, and the fixation patch should also be fixed in a suitable position.

When carrying out the necessary restraints, attention should be paid to the tightness, and the restraints should be reduced or lifted in a timely manner without bringing danger to the patient.

Improvement: Inform patients about their treatment experience before they receive treatment to dispel their fears.
