Introduction

The doctor admits frankly: there is a mismatch between salary and pay!

The anti-corruption of medicine and the issue of doctors' remuneration have once again become the focus of attention in the industry. The most discussed question is whether the income of the doctor group is high or low.

The income of doctors should appropriately reflect the value of labor services and the amount of work they have done. Last year, a report on the current state of human resources in Chinese hospitals in 2022 showed that the average clinical income of doctors was 94,000 yuan, and 70% of doctors said that their pre-tax clinical income was less than 100,000 yuan.

This time, the doctors interviewed by the reporter all said that their income is mainly composed of clinical income and reasonable non-clinical income, which is related to the regional economic level, hospital level and department. In their eyes, those who are "on the news" are all big names, and some people even ridicule their departments for being "used to being poor", which has nothing to do with the rumored "sky-high corruption".

Coastal grassroots doctors work for 3 years

It is comparable to the deputy chief physician of the western region for more than 30 years

The reporter compares the three roles of grassroots doctors, doctors in tertiary hospitals in the central and western regions, and chief physicians in economically developed coastal areas, and the salary of doctors has a lot to do with the regional economic level.

Dong Ning (pseudonym) has been practicing since 1988 and is currently the deputy chief physician of internal medicine in a tertiary hospital in the central and western regions. After 35 years of medical practice, Dong Ning's income is mainly composed of basic salary and performance bonuses. Now, Dong Ning's monthly basic salary is about 4,000 yuan, and his performance fluctuates between 7,000 and 8,000 yuan, with a total monthly income of about 12,000 yuan.

Due to its location in the central and western regions, Dong Ning's clinical income is basically the only source of his income, and the extraclinical income such as topics, trials, manuscript fees, and lectures is basically negligible. However, due to his long experience and high professional title, Dong Ning's income has been higher than the average level of the department.

But if we compare this standard with economically developed places, Dong Ning's income is not even as good as that of young doctors in some grassroots medical institutions.

Xiaoyu (pseudonym), born in the 90s, is in a primary medical institution in a county on the eastern coast. Xiaoyu's unit is a fully funded institution, so its performance is less affected by business volume. To increase income, it mainly depends on the promotion of professional titles and positions, but it is difficult to be promoted.

According to him, the current annual income is about 130,000 yuan. In addition to clinical income, Xiaoyu also lectures at part-time training institutions and health managers, which brings him some non-clinical income. However, because of the primary medical institutions, Xiaoyu can receive fewer exchange meetings and industry conferences to participate. Some of the academic conferences were attended by Xiaoyu out of his own pocket.

Wang Ze (pseudonym) is already the chief physician of a tertiary hospital in a first-tier city, which has reached the pinnacle of the doctor's title. Although he did not disclose his specific income level, Wang Ze said that the level of a doctor's professional title has an impact on the volume of business and patients, which may cause performance differences between doctors with different professional titles. However, as an internist, there is no other income such as "flying knives", and the gap between Wang Ze's clinical income from this hospital and other doctors in the same department is not too big.

Interviewee: Corruption is far away from you

The cost of upfront training for doctors is significant

There are great differences in the regional economic level, and the income of doctors at different levels of hospitals, departments and ranks. However, the doctors interviewed all felt that the current salary system did not match the actual contribution of doctors, and the training cost of clinicians in the early stage was very large, and there was a huge workload after becoming doctors.

From the perspective of education cost, in order to enter a more ideal medical institution, clinicians need to go through five years of undergraduate training, three years of master's training, if you want to enter a region or hospital with better conditions, a doctoral degree is essential, and the study is often more than 10 years. Different from other professions, medical students who want to become full-time physicians after graduation also need to undergo standardized residency training, commonly known as "regular training". According to the regulations, the "5+3" model is the main model, that is, graduates who have completed 5 years of undergraduate education in medical majors will receive 3 years of standardized residency training in the training base.

Dong Ning said, for example, that the regular income of students in his hospital is 1,000 yuan per month, and if the night shift is 100 yuan a day, it is equivalent to "subsidy" training. However, Xiaoyu recalled that he still had an income of several thousand yuan a month during the training, which was enough to maintain living expenses.

Even if you survive the learning and training stage, it does not mean that the doctor can lie flat. With more than 30 years of practice, Dong Ning's home has never been more than 1 km away from the hospital. Rounds at 7 a.m. every day, being on call at duty, and phone calls from patients at any time have been throughout every day since he started his practice.

There are opinions on whether doctors' incomes are high or low, but it is certain that most ordinary and front-line doctors are very far away from the rumored corruption of tens of millions or even hundreds of millions.

2023vs2022年,

More than 37% of health workers have seen their pay fall

The issue of doctors' income has always been a topic of concern to all sectors of society, and the "2023 Medical Talent Employment Development Report" recently released by Huayi.com has further understood the salary status of medical workers in 2023 through an online survey of more than 20,000 medical practitioners. Q: Please rate your current job satisfaction - salary and benefits?

At present, mainland hospitals implement a basic salary + performance + allowance system, which is mainly linked to the doctor's business, including performing surgeries, applying for projects, publishing papers, etc., and the allowance is related to the doctor's academic title. The income levels of doctors in different regions, levels of hospitals, departments and seniority vary greatly.

So, what is the salary situation of doctors, what is the salary between different occupations/titles/educational groups, and what is the impact of salary changes on the career development of medical staff?
