Anal fissure, unspeakable pain

Today, I would like to talk to you about a common anorectal disease – anal fissure. Anal fissure is a small ulcer formed after laceration of the skin layer of the anal canal, which is mainly manifested by anal pain, and severe anal pain can affect the quality of life of patients. It is more common in young and middle-aged people. Anal fissures tend to occur anteriorly and posteriorly because the anal muscles are relatively weak in the anterior and posterior areas, and it is relatively easy to tear when hard stools pass through.

The three typical symptoms of anal fissures are periodic pain, blood in the stool, and constipation.

1. Pain Pain is the main symptom of anal fissure, which is generally intermittent and cyclical, and is closely related to defecation. Typical anal fissure pain is discomfort or mild pain in the anus immediately after defecation, and when stool passes through, the pain suddenly worsens and becomes tearing or pinprick-like pain. The pain after a bowel movement is almost gone. After about 30 minutes, the pain reappears and gradually worsens, and it is crampy, severe and lasts for minutes, tens of minutes, or hours.

2. Blood in the stool Blood in the stool is also one of the most common symptoms of anal fissure, the bleeding of anal fissure is generally small in amount, bright red in color, or blood is adhered to the surface of the stool, or it can only wet the surface of the tissue. Blood and pain in the stool usually go hand in hand.

3. Constipation Constipation is both a common cause and a common symptom of anal fissures. Because patients with anal fissure are afraid of stool due to pain, they artificially reduce the frequency of bowel movements, making stool dry and difficult, and eventually causing phobic constipation, while on the other hand, constipation will increase the chance of developing anal fissure. There is often such a contradictory and mutually causal relationship between them.

In addition to these symptoms, anal fissures may also induce discomfort such as anal itching, dampness, and foreign body sensation due to the formation of ulcer surfaces. Therefore, we need to be aggressive in the prevention and treatment of anal fissures.

1. Keeping the stool soft and forming and being able to pass it smoothly is the main means of prevention and control, and the specific measures include: (1) Adjust the diet reasonably, eat more foods containing high fiber to increase the amount of stool, and eat less spicy, fried and other foods. (2) When constipation, you can take laxatives in moderation under the guidance of a doctor to keep the stool soft and avoid mechanical damage to the cleft. (3) Develop the habit of regular bowel movements. Waking up in the morning and half an hour after eating is the most active time for bowel movements, and it is also a good time to have a bowel movement. (4) Timely treatment of anal inflammatory diseases to prevent the formation of ulcers and subcutaneous fistulas after infection. Timely treatment of diseases such as ulcerative colitis to prevent concurrent anal fissures.

2. Local treatment: such as sitz bath with warm water or traditional Chinese medicine after defecation, to promote the circulation of local blood in the anus, improve sphincter spasm to relieve pain; can also be inserted with suppositories and external ointment to promote the healing of anal fissure wounds; in addition, anal dilatation treatment is also an important treatment method for anal fissure.

3. Surgical treatment: If the conservative treatment effect of old anal fissure is not good, surgical treatment can be selected, and the surgical methods include anal fissure resection and anal sphincterotomy.

As the saying goes, "nine hemorrhoids in ten people", hemorrhoids is a common anorectal disease with a high incidence, so there is this exaggeration. Modern medical research believes that hemorrhoids are soft veins formed by poor blood return in the anus and varicose veins.

Hemorrhoids are unique to humans because humans are upright walking animals, and there are many reasons why hemorrhoids can occur. For example, sitting and standing for a long time, squatting on the toilet for too long, anal infection, spicy and stimulating diet, genetic factors, etc., will induce and aggravate hemorrhoids.

According to the location of occurrence, it can be divided into internal hemorrhoids, external hemorrhoids, and mixed hemorrhoids.

(1) Internal hemorrhoids: according to their clinical manifestations and severity, they can be divided into 4 stages. Hemorrhoids in stage I: intermittent hematochezia, mainly hemorrhoids in stage II: intermittent hematochezia, easy to detach outside the anus during defecation, can be retracted by themselves; III. Hemorrhoids in the period: blood in the stool is reduced, and the hemorrhoids protrude out of the anus after defecation, and need to be retracted by hand; IV. Hemorrhoids in the period: annular prolapse cannot be reduced.

(2) External hemorrhoids: Specifically, they are divided into connective tissue external hemorrhoids, varicose vein external hemorrhoids, thrombotic external hemorrhoids, and inflammatory external hemorrhoids, and their manifestations are mainly anal discomfort and pain.

(3) Mixed hemorrhoids: There are symptoms and signs of both internal and external hemorrhoids, so that there is something in the anus that prolapses out during blood and stool, and pain, anal swelling and discomfort are the main manifestations.

The treatment of hemorrhoids is divided into conservative treatment and surgical treatment. Conservative treatment is mainly for patients with mild hemorrhoids, less blood in the stool, small hemorrhoids, and contraindications to surgery. The treatment is mainly based on traditional Chinese medicine fumigation sitz bath, hemorrhoids embolization and anal plugging.

Surgical treatment is recommended for patients with hemorrhoids with the following conditions: (1) frequent blood in the stool, large amounts, frequent attacks, and poor drug treatment effect. (2) Those who have large hemorrhoids, protrude out of the anus after defecation, and cannot be accepted by themselves, and need to be supported by hands and rested flat to return to their positions, or those who are embedded outside the anus and cannot be retracted. The occurrence and development of hemorrhoids are closely related to our lifestyle habits and bowel habits. Developing good lifestyle habits and bowel habits can delay the development of hemorrhoids.

(1) Eat more foods rich in fiber, such as various vegetables, fruits, etc., and need to consume enough water every day. Eat less spicy and irritating foods.

(2) Moderate exercise, do not sit for a long time, combine work and rest, develop good work and rest habits, do not be tired, stay up late, and have a regular life.

(3) Develop good bowel habits. Keep the stool smooth, prevent constipation or diarrhea, and do not take too long to defecate.

(4) Pay attention to anal hygiene. Wash the anus with warm water after using the toilet.

(5) Rehabilitation exercise guidance. For example, anal contraction exercise, also known as "levator ani exercise", that is, self-adjustment of the anal sphincter, contraction, relaxation of the anus, one retraction and one release, 15-20 times each time, lasting about 2-3 minutes, 1-2 times a day.
