On April 20th, the first academic event of the year after the launch of the Controlling the Future project was successfully held in Beijing. More than 10 guests from many hospitals in Beijing attended the meeting, focusing on the key issues of perioperative management of major spine surgery, through the detailed interpretation of the "Expert Consensus on Perioperative Blood Management of Major Spine Surgery" and in-depth exchanges of relevant cases, to jointly promote the standardization and scientificization of perioperative management of major spine surgery.

In his speech, Mr. Jianguo, Chairman of the Conference and Director of the Department of Orthopedics of Peking Union Medical College Hospital, welcomed and thanked the guests. He reviewed the original intention of the establishment of the Future of Control project, and explained the profound meaning of "control", the first is to standardize academic activities under the current new medical situation, and the second is to rely on this platform to deepen the intersection of medicine and engineering, promote innovation and transformation, and enhance the professional value of spine surgeons.

After the opening ceremony, academic exchange activities were closely launched. Zhuang Qianyu took the lead in interpreting the "Expert Consensus on Perioperative Blood Management in Major Spine Surgery", and he introduced the background of the establishment of the project and the process of consensus formulation. The critical draft of the expert consensus is based on the principle of "maximum and complete" evidence-based, through the design of the questionnaire survey that makes the experts "the most time-saving and labor-saving", on the basis of giving the greatest respect for each word of the consensus formulation of expert opinions, and after expert discussion to overcome several major academic controversies that are difficult to make decisions, it has finally formed a crystallization from content to grammar that is fully close to the consensus publication requirements and clinical practice, and condenses the efforts of dozens of top orthopedic experts. This consensus formulation has formed a relatively standardized process, which provides synergy experience for the formulation of other guidelines and consensus in the future.

This consensus provides norms and recommendations for various common problems in the perioperative management of major spine surgery, which are intended to guide doctors to improve the safety of surgery and reduce the risk of related complications when performing major spine surgery, which is helpful to improve the quality of medical services and meet the current medical service requirements.

Cai Siyi from Peking Union Medical College Hospital shared "Blood Management Issues Related to Lumbar Degenerative Scoliosis Surgery". Through case analysis, he illustrated the advantages and potential risks of posterior and anterior surgery when choosing a surgical method, and also focused on the management of patients with occult bleeding after surgery. Panelists emphasized the importance of focusing on the hidden cause of blood loss by comparing blood loss between different surgical approaches.

Cheng Junyao made a report

Cheng Junyao, Department of Orthopedics, PLA General Hospital, shared the relevant experience of our hospital on "Perioperative Management of Kyphopedic Orthopedics for Ankylosing Spondylitis", including preoperative anesthesia and general condition assessment, selection of surgical timing, use of preoperative immune preparations, intraoperative anesthesia, use of antimicrobial drugs, blood transfusion as appropriate, spinal cord function monitoring, postoperative incision assessment, postoperative medication, abdominal skin care and prevention and treatment of complications. The panelists discussed his experience in improving postoperative digestive problems, intraoperative anesthesia, and reducing the risk of cerebrospinal fluid leakage in patients with ankylosing spondylitis.

Sun Zhuoran made a report

Sun Zhuoran from Peking University Third Hospital illustrated the experience of "Scoliosis Combined with Lumbar Spondylolisthesis Dysplasia" through two cases. He pointed out that when severe dysplastic spondylolisthesis and severe scoliosis coexist, a staged surgical approach is advocated, with the spondylolisthesis treated first to correct the base, and then the deformity corrected according to the situation. During the treatment process, it is necessary to find the main cause and formulate a treatment strategy to avoid misdiagnosis of compensatory scoliosis as structural scoliosis, so as to avoid unnecessary damage to the patient. For the selection of staged surgery for such patients, the guests shared their own experiences.

Wu Bingxuan, from Beijing Tiantan Hospital, Capital Medical University, introduced in detail the process of multidisciplinary consultation, preoperative preparation, intraoperative operation and postoperative follow-up for patients with spinal metastatic pheochromocytoma in the report "Perioperative Management of a Special Case of Spinal Metastases". In view of the occurrence of malignant hypertension in such patients, the participants discussed the risks and precautions for surgical operations.
