Recently, under the leadership of Deng Youwen, the director of the spine surgery department of the Third Xiangya Hospital of Central South University, successfully operated on a patient with type A skull base depression combined with syringomyelia. The patient's postoperative symptoms were significantly improved, and good clinical results were obtained.
Mr. Song (pseudonym) is a 67-year-old from Lipu, Hunan Province, who has suffered from strange diseases for many years, with atrophy of the muscles in his right hand, unsteady holding of chopsticks, headache, dizziness, unsteady walking, and serious difficulties in daily life. The cranial spine surgery team led by team leader Li Jinsong performed a complex operation for Mr. Song: "transoral atlantoaxial lateral block space release + anterior atlas arch resection + posterior atlantoaxial arch resection + occipital axial fixation and fusion + iliac bone grafting + cranial traction".
In particular, since the patient's atlantoaxial joint cannot be directly reduced by heavy skull traction, it needs to be released through the oral, which greatly increases the difficulty and complexity of the operation.
The anterior transoral surgery completely loosens the atlantoaxial joint and resects the anterior atlantoaxial arch, while the posterior surgery uses the powerful reduction function of the nail plate system to reduce the dentate process to the level of the atlas and embed the gap in the anterior arch of the atlas, thereby maximizing the slope pivotal angle and relieving the compression of the extended spinal cord.
The procedure is extremely complex and risky, as the surgical area is dominated by the spinal cord on the side and the vertebral artery on the lateral, with minimal safety margins. In addition, the surface of the atlantoaxial joint is rich in venous plexuses, which can easily cause uncontrollable hemorrhage. The cranial spine surgery team completed the combined anterior and posterior surgery with skillful technology, without the need for blood transfusion and intensive care unit.
After the operation, Mr. Song's symptoms such as headache, dizziness and unsteady walking were significantly relieved, and he was able to turn around freely.
