At about 14:10 on September 16, 2023, a high-altitude fall accident occurred in Building 1 of the Shuikou TCL project site in Huicheng District, resulting in 2 deaths. The direct economic loss is about 5 million yuan.
On April 10, 2024, the accident investigation team released the relevant accident investigation report: the accident investigation team determined that the general fall from height accident of Huicheng Shuikou TCL "9.16" was a production safety liability accident caused by the illegal construction of the enterprise.
What happened to the accident
At about 13:50 p.m. on September 16, 2023, painters Liu, Yan, Ou and Min went to the west side of Building 1 to start preparing for exterior wall paint repair work. Liu and Yan successively entered the hanging basket of Building 1, and Yan controlled the switch of the hanging basket hoist to rise, preparing to go to the top floor (about 55 meters on the 9th floor) for exterior wall painting repair operations.
At 14:10, the hanging basket rose to the 4th floor of Building 1 (about 18 meters high), and the right hanging rope suddenly broke, causing the hanging basket to tilt to the right (at the time of the accident, Liu and Yan were not wearing seat belts and wearing safety helmets), causing Liu and Yan to fall to the ground before and after, and died after 120 on-site rescue.
At the time of the incident, Liu and Yan operated the hanging basket by themselves and began to rise from the ground. Due to the rust of the hoist rope (main rope), it has been deformed by the upper and lower extrusion of the hoist ratchet for a long time, and the hoist rope is broken at the crushing site.
After on-site investigation, the lifting rope after the elevator rope exit is missing, and when the hanging basket rises, the elevator continues to peel off the broken strand of steel wire. Winding (Fig. 3), causing the main rope to be stuck at the guide pulley, the main rope between the guide pulley and the elevator rope inlet is pulled off under the action of the hoist mechanical force, the right side of the hanging basket falls, and then the safety lock locks the auxiliary rope, but under the dual action of the gravity of the hanging basket and the load and the inertia of sudden locking, the auxiliary rope breaks at the support pulley of the roof suspension platform, causes the right side of the hanging basket to completely sag, and finally leads to the occurrence of the person falling accident from height.
Analysis of the cause of the accident
1. There are major defects in the gondola equipment and facilities
The project department of a company did not use the gondola equipment as required, and dismantled, installed and displaced it many times during the construction period, and replaced and mixed the gondola equipment at will, which reduced the overall stability and safety of the accident gondola, resulting in the main rope on the right side of the gondola being broken during the lifting process, and the gondola fell and overturned instantly.
2. Workers work without a license or in violation of regulations
Painters Liu and Yan did not obtain the operation certificate for high-altitude operations, did not have the qualifications for high-altitude operations, and did not wear safety belts as required when using the gondola equipment to work at heights on the exterior wall, and the rope broke during the lifting of the gondola, causing the gondola to be unstable and unbalanced and fall on the right side, resulting in two people falling from the gondola to the ground and dying.
Suggestions for dealing with the persons responsible for the accident:
(1) Persons recommended that the public security organs conduct further investigation and handling (1 person)
Mr. Zhu, the deputy project manager of a silver company, is the person in charge of the TCL project site. Illegal commanders disassemble and assemble gondola equipment and replace gondola safety facilities (slings) without authorization; illegally organize workers who do not have the corresponding qualifications to use gondolas that do not have safe conditions to carry out high-altitude work, fail to investigate and eliminate the hidden dangers of major accidents at the construction site in time, and still take the risk to organize operations, and bear responsibility for the occurrence of accidents. It is recommended that the public security organs conduct further investigation and handling.
(2) Units and individuals recommending administrative punishments
1. Yin company, in violation of the provisions of Article 4 of the "Work Safety Law of the People's Republic of China", is responsible for the accident, and it is recommended that the Huicheng District Emergency Management Bureau impose an administrative penalty on it in accordance with the provisions of Article 114, Paragraph 1 (1) of the "Work Safety Law of the People's Republic of China".
2. Huizhou Branch of a supervision company violated the provisions of Article 41 of the Work Safety Law of the People's Republic of China, and it is recommended that the Huicheng District Emergency Management Bureau impose an administrative penalty on it in accordance with the provisions of Article 102 of the Work Safety Law of the People's Republic of China.
3. Tang, the project manager of Yinguangsha Company, is the main person in charge of the TCL project. Failure to perform the duties of the main person in charge of production safety in accordance with the law, the implementation of the responsibility system for production safety is not in place, the supervision and inspection of production safety management work is not in place, the problems of illegal command and risky operations on the construction site are not discovered in a timely manner, and the hidden dangers of production safety accidents are not eliminated in a timely manner, and the leadership responsibility for the occurrence of the accident is borne and administrative punishment is imposed.
4. Li, Huicheng Shuikou TCL Project (Phase I) Project Constructor. Failure to record the construction log as required, failure to timely report for review of the entry and moving hanging baskets, failure to effectively supervise the construction unit to carry out the investigation and management of potential accidents, eliminate potential accidents, organize operations without approval of high-altitude operations, and fail to perform statutory safety production management duties, it is recommended that the Huicheng District Emergency Management Bureau impose administrative penalties on it in accordance with Article 96 of the Work Safety Law of the People's Republic of China.
5. Li Moumin, project director of Huizhou branch of a supervision company. Failure to take effective measures to eliminate the hidden dangers of major accidents at the construction site, the negligence of the construction party's use of hanging baskets that have not been submitted for review and special operations personnel who have not been certified to work, and the failure to implement the inspection and side station work during the construction of the sub-project with greater risk, shall be subject to administrative punishment.

Construction and operation personnel must wear safety helmets, fasten safety ropes (belts), do not wear hard-soled shoes, plastic shoes or other slippery shoes, it is strictly forbidden to use ladders, stools, shelves and other climbing tools in the hanging basket, and it is strictly forbidden to run and jump in the hanging basket.
