Hongyun Street Community Health Service Center is located in Building 1-A, Jinshangyuan Community, Hongjin Road, Hongyun Street, Wuhua District, which is a comprehensive community health service center integrating basic medical care, family doctor contract services, national basic public health services, health education and other service functions.

Primary care
Hongyun Sub-district Community Health Service Center has 6 clinical departments (Preventive Health Department, General Medicine Department, Medical Laboratory Department, Medical Imaging Department, Traditional Chinese Medicine Department and Pharmacy Department). It mainly provides vaccination, general outpatient clinics, traditional Chinese medicine diagnosis and treatment, as well as the diagnosis and treatment of common and frequent diseases in the community.

National Basic Public Health Service
The Hongyun Sub-district Community Health Service Center will work with 15 community health service organizations in the district to carry out 14 free national basic public health service projects. These projects target the main health problems of both urban and rural residents, with a focus on children, pregnant women, the elderly, and people with chronic diseases, and provide free basic public health services to all residents. The project includes resident health record management, health education, vaccination, health management of children aged 0-6, maternal health management, health management of the elderly, health management of chronic diseases (hypertension, type 2 diabetes), management of patients with severe mental disorders, health management of tuberculosis, health management of traditional Chinese medicine, reporting and handling of infectious diseases and public health emergencies, health supervision and co-management, etc.

Family Doctor Contracted Service
Hongyun Street Community Health Service Center will establish a solid contractual service relationship with residents in the jurisdiction through door-to-door contracting, centralized signing and outpatient contracting, etc., and become a doctor friend of residents in the jurisdiction. After signing a family doctor's contract, in addition to basic services (establishment of residents' health records, basic medical services, and basic public health services), residents can also enjoy personalized service packages, which only need to pay 24 yuan per person per year (12 yuan for individuals and 12 yuan for medical insurance funds), and they can enjoy the value-for-money personalized service packages set up by Hongyun Center for key groups and the general population. Residents of the jurisdiction can also bring their ID cards and medical insurance cards to the center for consultation and signing.

Health education services
In the next step, the Hongyun Street Community Health Service Center will carry out health education to residents in the jurisdiction by holding health education lectures and carrying out health consultation activities. Improve the health literacy level of residents, improve the health status of residents in the jurisdiction, and enhance self-care ability, and provide high-quality medical services for residents in the jurisdiction.
