The party flag is flying high at the grassroots level| pipe to the east: the "patron saint" of seriously ill patients

author:Gaoqing County Procuratorate

Wearing a white coat, wearing a doctor's hat embroidered with the words "Party member pipe to the east", stepping on a pair of old cloth shoes, walking with the wind. The reporter's first impression of Guan Xiangdong was that he was an approachable and capable person, which was very compatible with his nickname "Guan Ye".

For more than 30 years of medical practice, Guan Xiangdong has adhered to the frontline of critical care medicine, leading the critical care medical profession of Zhongshan First Hospital from scratch and from weak to strong; In the 25 years since he joined the party, he has always been brave to be the person who carries the battle flag in the team, charging forward and not backing down.

"Critical care medicine is not only a cure, but also a lifesaver"

In 1991, after graduating with a master's degree, Guan Xiangdong entered the Department of Critical Care Medicine of Zhongshan First Hospital and became one of the first ICU (Intensive Care Ward) doctors in China. At that time, critical care medicine in China was just getting started. Guan Xiangdong laughed and said: "At that time, the doctors in the society, and even some general wards, did not know what the ICU was. ”

The newly established Intensive Care Department of Zhongshan First Hospital has only 8 ICU beds, limited equipment conditions, weak foundation, broken ventilator, and the pipe will be dismantled and repaired by itself to the east. "Since the 1990s, the country has gradually attached importance to the development of critical care medicine, and I have had the privilege of witnessing the development of critical care medicine." Pipe said to the east.

Explore the establishment of a comprehensive management of critical care medical centers, incorporate ICU wards of different clinical sub-specialties into overall planning, standardize clinical management, and take the lead in compiling the country's first guidelines for the construction and management of critical care medicine departments... Guan Xiangdong led the team to build the Department of Critical Care Medicine of Zhongshan First Hospital into one of the largest and most influential critical care disciplines in the country. In recent years, as the chairman of the Critical Care Medicine Branch of the Chinese Medical Association, he has led the national critical care professional peers, effectively promoting the development of the critical care profession in China.

What makes him persistently deepen his cultivation of critical care medicine? Guan Xiangdong shared one of his statistics with reporters: before 1991, there were only 4 cardiac surgery ICU beds in Zhongshan First Hospital, and the mortality rate of severe patients with organ failure in general surgery was more than 90%; By 1994, with the advent of the Intensive Care ICU, the success rate of severely ill patients in general surgery had reached more than 90%. "This is the value of intensive care medicine, which is not only a cure, but also a lifesaver."

As a clinical specialist, Guan Xiangdong insists on rounds 2 to 3 times a week, "ICU is I see you (I look at you), you must always pay attention to the changes in the patient." He proposed to create a "temperature ICU" and required tube bed doctors and nurses to take the initiative to contact the patient's family once a day and communicate with the family in an all-round way. "The patients in the ICU come in at a critical moment, they give their lives to us, we want to treat them as family, and everything is patient-centered."

During rounds and diagnosis and treatment, in order to make patients feel more at ease, Guan Xiangdong always wears the doctor's hat embroidered with the words "Party member Guan Xiangdong", "Many patients know that you are a party member, they will trust you more, and I believe that you will think about them wholeheartedly." ”

"At this moment of crisis, I have to take the lead and rush in"

In an interview, Guan Xiangdong had just returned to Guangdong from the frontline of the epidemic in Shanghai for more than a month. Since the outbreak of the new crown pneumonia epidemic in 2020, he has rushed to the rescue of the line of fire 13 times, fought in the epidemic area for a total of 333 days, and together with 7 other national experts, he was affectionately called "eight immortals of severe illness" by the media and netizens.

On the eve of the Spring Festival in 2020, Guan Xiangdong received a call from the National Health Commission, asking him to be on standby at any time and go to Wuhan as a national expert of the medical treatment team to support. On the fifth day of the first month of the first month, he was anxious about the epidemic and finally received an official notice, "I can't wait, I want to go immediately!" Early the next morning, he boarded the high-speed train to Wuhan and went backwards alone.

"At that time, the gold and silver pond could be described as the 'Dragon Pond Tiger's Den.'" As soon as he arrived in Wuhan, Guan Xiangdong immediately went to Jinyintan Hospital, which concentrated on the treatment of severe and critically ill patients, and entered the speed of life and death. Taking Jinyintan Hospital as the starting point, Guan Xiangdong runs to the ICU of intensive care treatment in various designated hospitals in Wuhan every day, guides ward isolation, patient management, researches treatment ideas, and works for more than ten hours a day.

At that time, the mortality rate of severe disease in Tianmen City and Xiantao City was high, and he was ordered to go to the local designated hospital, did not meet to listen to the report, directly put on the protective clothing into the ICU to see the facts, and instructed the medical staff to carry out intubation treatment and turn over for the patient. "At the moment of crisis, there is no room for hesitation, some people are afraid of difficulties, and I have to take the lead in rushing in." After his guidance, the mortality rate of Tianmen and Xiantao was greatly reduced.

In this battle without smoke, Guan Xiangdong was a person who carried the flag to charge. "One person's strength is limited, and if he can lead his peers to the front line, he can save more seriously ill patients." At the beginning of the epidemic, in the name of the chairman of the Critical Care Medicine Branch of the Chinese Medical Association, he carefully drafted a proposal, and joined hands with two other experts in charge of the critical care academic organization to jointly call on the national critical care medical professionals to go to Hubei to support. The proposal received a wide response, with 19,000 of the 42,600 healthcare workers in Hubei from critical care specialties.

After returning from Wuhan and resting for only 3 days, Guan Xiangdong received an urgent task and went to Suifenhe, Heilongjiang to fight the epidemic. Xinjiang, Yunnan, Liaoning, Henan... For more than two years, he has been running where the party and the people need him the most, and has always maintained the record of zero deaths of serious and critically ill patients under direct management, "I now have a carry-on bag and a suitcase, waiting to be called to leave at any time." ”

"The unchanging original intention is the supremacy of the people and the supremacy of life"

In Guan Xiangdong's view, cultivating more outstanding critical care medical talents is the key: "Each ICU is easy to build, allocate a floor, buy some ventilators, but it is not easy to build, and it is necessary to have a group of critical care professional medical staff to pounce on it in order to support it." ”

In 2003, Guan Xiangdong became the first doctoral supervisor of critical care medicine in China, and has trained hundreds of master's and doctoral students so far, the best of which have become the backbone of the department of critical care medicine. In order to stimulate more young medical students' interest in the critical care profession, Guan Xiangdong also opened the first "intensive care medicine" undergraduate self-selected course in China, and each course was full as soon as it was launched. "The level of critical care doctors is related to the lives of patients, and my requirements for students are that they must be able to save their lives when they go out of the division, and they must persevere to the end for each patient."

In addition to taking students, Guan Xiangdong is also committed to the training of leaders of the intensive care departments of county-level hospitals, and trains and transports a large number of critically ill backbone talents for each county-level hospital every year, so that more seriously ill patients have the opportunity to enjoy ICU treatment nearby and regain a new life.

Talking about the next development of critical care medicine, Guan Xiangdong was full of interest, "The informatization and big data of critical care medicine is the only way to go in the future. Although ICU has been introduced in many hospitals across the country, the sudden death rate in general wards is still not low. Zhongshan First Hospital is currently taking the lead in piloting the rapid response system for severely ill patients in the country, wearing special electronic watches for patients in general wards, recording patient signs in real time, emergency calls when abnormal, and a special team responding to treatment quickly, using big data to detect and reduce the sudden death rate. If the pilot matures, it will identify potential dangers for more patients and protect lives. ”

"I am a critical care doctor and a doctor party member, and my unchanged original intention is the supremacy of the people and the supremacy of life." After decades of practicing medicine, Guan Xiangdong has been "confessing" to the party with practical actions, and the patient's life and health and the shining party member badge on his chest are deeply placed in the most important position in his atrium.
