"Double Bubble Sign", also known as the double bubble sign, is a relatively straightforward image sign, what you see is what you get, that is, the appearance of two bubble-like shapes on the upper abdomen in the abdominal flat film, and the double bubble sign is an imaging manifestation of duodenal obstruction in newborns or infants.

Image performance:

On plain X-ray of the abdomen, two balloon-containing cavity-like structures appear in the upper left and slightly lower right sides of the midline, with a vertical visible liquid gas plane and no or only a small amount of inflation at the distal end. This sign may also be seen on barium or ultrasound in the upper gastrointestinal tract. In duodenal obstruction, the duodenum and stomach proximal to the obstruction show progressive gas, fluid accumulation, and dilation, forming a so-called double bubble sign.

Bivestic signs are more common in duodenal atresia, but may also be seen in duodenal stenosis, annular pancreas, or poor intestinal rotation.

According to the bicuicular sign and its accompanying signs, the degree and cause of the obstruction can be roughly judged, if the double vesicle is large and distally inflated, it indicates complete duodenal obstruction (duodenal atresia); the biveal vesicle is small and has more or less inflated distal end, which is mostly incomplete obstruction (poor intestinal rotation, duodenal stenosis, circular pancreas, etc.).

In addition, when incomplete obstruction is absorbed by the intestinal wall because the gas has not entered the distal end of the obstruction or a small amount of inflation at the distal end of the obstruction is absorbed by the intestinal wall, it may present as a simple double bubble sign.

On the other hand, when the distal intestinal duct of the biveal sign is inflated, the possibility of complete obstruction cannot be ruled out, because duodenal atresia is combined with abnormal bile duct development, the proximal gas of the obstruction can enter the distal end of the obstruction through the abnormal bile duct.

Therefore, the degree of duodenal obstruction cannot be judged by the presence or absence of inflation at the distal end of the obstruction. Instead, it should be fully observed, comprehensively analyzed, and if necessary, barium or ultrasonography should be performed.
