Red Network Moment January 14 news (correspondent Dong Yangzhou Huang Yeni He Rang) On January 13, the Third Department of Orthopedics and Injuries of Hunan Provincial Hospital of Traditional Chinese Medicine (the Second Affiliated Hospital of Hunan University of Traditional Chinese Medicine) learned from telephone follow-up that Grandma Zhang, who was discharged from the hospital nearly a month ago, showed that there was no new thrombosis, the coagulation function was basically normal, and the family reflected that she had been able to get out of bed to support single crutches and move freely, and her basic life could take care of herself and recover well. The news of Grandma Zhang's smooth recovery has made the medical staff very happy, and it also means that this critically ill and difficult case has been successfully treated under the "hospital-wide general consultation" organized by the medical department of the hospital and the coordinated escort of multiple departments.

On November 30, 2021, Grandma Zhang, who was transferred from Pingjiang County Hospital of Traditional Chinese Medicine to Hunan Provincial Hospital of Traditional Chinese Medicine for treatment, was preliminarily diagnosed as a fracture of the left femoral neck, deep vein thrombosis of both lower limbs, gastrointestinal bleeding, osteoporosis, hypertension, chronic bronchitis, and cerebral contusion. On the second day of admission, the doctor found that the thrombus in both of her lower extremities was progressing rapidly, and laboratory tests showed a significant decrease in hemoglobin, and her condition was critical, and the risk of pulmonary embolism was extremely high. The department immediately consulted the Intervention Center and performed "inferior vena cava contrast + filter implantation" at noon on the same day to prevent serious thromboembolic events. Under the great attention of the leaders at all levels of the hospital, the Medical Affairs Department launched an emergency hospital-wide general consultation with the participation of experts from multiple departments such as gastroenterology, endoscopy center, general surgery minimally invasive center, respiratory and critical care medicine, neurology, anesthesiology, and ICU.

The 65-year-old Grandma Zhang has many underlying diseases, acute and complex disease, deep vein thrombosis and gastrointestinal bleeding occur at the same time, and there is a contradiction between thrombolytic anticoagulation and hemostasis treatment. How to balance anticoagulant thrombolysis and hemostasis, how to create conditions for surgery as soon as possible, how to let patients smoothly pass the postoperative dangerous period, experts reached an agreement through serious discussion, formulated a detailed treatment plan, and fully communicated with patients and their families.

Pull out the dark clouds to see the sky, and keep the clouds open and the moon is bright. On December 7, the expert team successfully performed "left artificial total hip arthroplasty" for Grandma Zhang, and closely observed her condition after surgery. On December 13, he performed "inferior vena cava angiography and filter removal", and the inferior vena cava was unobstructed during the operation, and his condition improved, and he was successfully discharged from the hospital and returned to his home in Pingjiang on December 15. After two follow-up visits at the hospital, it was learned that Grandma Zhang was recovering well.

In 2021, under the organization and coordination of the Ministry of Medical Affairs, with the active participation of clinical, medical technology, clinical pharmacy and other departments, Hunan Provincial Hospital of Traditional Chinese Medicine completed nearly 70 cases of hospital-wide general consultations, a significant increase over 2020, including complex and difficult case consultations with the participation of professors from foreign hospitals, and many night emergency consultations organized for the rescue of critically ill patients.
