Recently, under the guidance of Li Wei, chief physician of the Digestive Center of Beijing Friendship Hospital, a "group-style" medical aid expert in Beijing, Lhasa People's Hospital completed the first case of endoscopic colon polyp endoscopic submucosal dissection in children in the Tibet Autonomous Region. At the same time, the Beijing Li Huanying Medical Foundation intervened in a timely manner, which greatly solved the problem of patients' economic difficulties.

Heartwarming! Beijing and Tibet worked together to complete the operation of tibet's first child with black spot polyposis syndrome

A 29-year-old Tibetan youth lives in Dingqing County, Changdu City, Tibet, more than 800 kilometers from Lhasa City. 6 years ago, The second son, Ta Mou, found that there was a prolapse in the anus during defecation, and he could return to it on his own after defecation. However, due to the lack of medical and health knowledge, the child's symptoms have not been taken seriously, and he has not been treated in medical institutions. In July this year, Ta's symptoms worsened, accompanied by a large amount of blood in the stool, and Ci felt that he could not delay any longer, so he took him to lhasa People's Hospital for treatment. After examination, it was found that Ta's intestines were scattered with multiple polyps, and they were lateral developmental polyps, and the pathology suggested that it was a villous tubular adenoma.

The case attracted the attention of Li Wei, chief physician of the Digestive Center of Beijing Friendship Hospital. On July 25, in accordance with the unified deployment of the Beijing Municipal Health Commission and the Beijing Municipal Medical Management Center, Li Wei, as an expert of the ninth batch of the third phase of the Beijing Municipal Medical Aid Team, set off for the Lhasa People's Hospital in Tibet to carry out a one-year assistance mission. Li Wei is the vice chairman of the Endoscopy Committee of the Endoscopy Physician Branch of the Chinese Medical Doctor Association, and has more than 20 years of clinical work experience, especially in the field of digestive endoscopic diagnosis and treatment technology.

Li Wei keenly found that most of The family members of The Second Mou were scattered with small spots of skin pigmentation on the lips and hands, and according to the child's lips, skin pigmentation and colonoscopic performance, the child was eventually diagnosed with dark spot polyposis syndrome (PJS).

Melanoma polyposis syndrome is a rare tumor-susceptible autosomal dominant genetic disorder characterized by skin/mucosal dark spots and multiple gastrointestinal polyps, with a family history in about 50% of patients with PJS and an incidence of about 1 in 200,000. The disease can cause complications such as intestinal obstruction, intestinal bleeding, anemia, and polyp cancer. Due to the high risk of cancer, Li Wei recommends hospitalization and endoscopic submucosal dissection (ESD) as soon as possible.

In September this year, Ta was admitted to the pediatric department of Lhasa People's Hospital. Under the guidance of Li Wei on the spot, the local doctors of Lhasa People's Hospital independently completed the first case of endoscopic colon polyps ESD surgery in children in the Tibet Autonomous Region, and the operation was smooth, with no complications during and after the operation, and the recovery was good.

Subsequently, Li Wei learned from Ci that in the past few years, Ci's mother and three brothers and sisters had died of bowel cancer. Considering that a person may also have melatoplakia polyposis syndrome, it is recommended that he or she have a complete gastrointestinal endoscopy. Unsurprisingly, under gastrointestinal endoscopy, doctors detected that the patient's stomach and colon had multiple P-J polyps, confirming Li Wei's judgment.

The diagnosis of the condition was clear, but due to the patient's family difficulties and financial constraints, the second was shaken, and once considered giving up treatment.

The intervention of the Beijing Li Huanying Medical Foundation brought new hope to a family. After learning about the patient's family situation, Li Wei actively contacted and won financial assistance from the foundation for the second time, and assisted the patient to complete the relevant procedures. Recently, the second successful admission, the local doctor performed endoscopic gastric and colon multiple polyps surgery, the current operation is smooth, there are no relevant complications during and after the operation, the patient's condition is recovering well. Li Wei said that the patient with this difficult and complex disease can be diagnosed and treated in a timely manner on the snowy plateau, which represents the further improvement of the diagnosis and treatment level of digestive system diseases in the Lhasa People's Hospital in Tibet, and provides more accurate diagnosis and treatment services for patients with digestive system diseases in Tibetan areas.
