Zhongwei People's Hospital gave preliminary treatment such as gastric lavage and catharsis at the first time, but further treatment also needs to be referred to Yinchuan.

The ambulance sped all the way and escorted the 2 patients to the General Hospital of Ningxia Medical University.

At this time, the two were unconscious, unable to speak, the questions and answers were not relevant, and they fell into a shallow coma, and one of them also had convulsions of limbs and urinary incontinence.

Combined with a history of drinking mandala medicinal liquor, the doctor diagnosed acute oral drug poisoning (mandala, severe), very critical illness, and was urgently admitted to the emergency intensive care unit.

Ma Xiao, deputy chief physician of the emergency department of Ningyi University General Hospital, said that the 2 patients are brothers, the older brother is 38 years old, the younger brother is 34 years old, because he listens to the folk remedies of others, he thinks that the medicinal liquor brewed with mandala can strengthen the body and treat many diseases, so he brewed this medicinal liquor by himself. Blood perfusion removes toxins and turns the crisis around

Ma Xiao said that the main toxic components of mandala are alkaloids such as scopolamine, atropine and scopolamine (mandala extract), and the current treatment is mainly based on symptomatic supportive treatment, with no specific antidote.

However, for poisoned patients, blood perfusion is currently an advanced and effective treatment method in China, which can quickly remove poisons from the patient's body.

"Our emergency department treats more than 1,000 cases of various poisoning patients every year, of which nearly half of the patients are admitted to the emergency ICU or the emergency general ward to strengthen treatment, while the patients in the emergency ICU are critically ill patients, and according to the type of poison, the amount of poison taken, the damage to the function of the organs and other factors, one or more times of blood purification or blood perfusion treatment will be performed." Ma Xiao said.

The emergency department team arranged hemoperfusion therapy for the 2 patients, and the toxins in the blood were filtered out through the blood perfusion machine. At the same time, with conventional intestinal decontamination and drug treatment methods, good results have been achieved.

After 1 day, the two patients recovered their consciousness, their physical indicators were relatively stable, they passed the most dangerous stage, and they were transferred to the general ward.

A week later, the brothers recovered well, were discharged one after another, and followed up well.

Ma Xiao said that the two brothers are relatively young, and the amount of taking is not much, each person drank 1-2 two medicinal wines, timely medical treatment, can turn the crisis so quickly.

Professor Yang Lishan, director of the emergency department, said: Mandala is a very common wild herb, often growing next to houses, roadsides or grasslands. It is poisonous in whole grass, with fruits, especially seeds, being the most toxic, followed by young leaves. Intoxication manifests as dry skin and mucous membranes, mydriasis, blurred vision and photophobia, altered mental status, hyperthermia, sinus tachycardia, decreased bowel sounds, urinary retention, tremors, and myoclononic convulsions. Other symptoms may include ataxia, short-term memory impairment, hallucinations (visual and auditory), restless delirium, seizures, coma, respiratory failure, and cardiovascular failure, with severe life-threatening effects leading to death.

Because the symptoms are not typical, the diagnosis of mandala poisoning is more difficult, and some patients behave more similar to LSD (diacetamide lysergic acid) or ecstasy, etc., and conventional drug screening cannot detect it, and there is currently no such poison concentration detection in Ningyi University.
