The matters related to the declaration of outpatient special chronic diseases (employee medical insurance and urban and rural residents' medical insurance) in Yunyang District in the fourth quarter of 2021 are hereby notified as follows:

1. Declare diseases

(1) There are 17 diseases in the medical insurance of employees, namely: 1. malignant tumors (including leukemia), 2. outpatient dialysis of chronic renal failure (uremia), 3. outpatient anti-rejection therapy after organ transplantation, 4. systemic lupus erythematosus, 5. diabetes mellitus, 6. aplastic anemia, 7. hypertension (extremely high risk), 8. severe psychosis, 9. chronic severe hepatitis, 10. cirrhosis, 11. hemophilia, 12. Parkinson's disease, 13. Parkinson's syndrome, 14. rheumatoid arthritis, 15. Tuberculosis, 16. cerebrovascular accident sequelae, 17. coronary heart disease.

(2) There are 25 diseases in the resident medical insurance, namely: 1. malignant tumors (including leukemia), 2. systemic lupus erythematosus, 3. aplastic anemia, 4. systemic sclerosis, 5. diabetes, 6. hypertension (extremely high risk), 7. severe psychosis, 8. chronic severe hepatitis, 9. cirrhosis, 10. Parkinson's disease, 11. Parkinson's syndrome, 12. rheumatoid arthritis, 13. tuberculosis, 14. sequelae of cerebrovascular accidents, 15. coronary heart disease, 16. Myasthenia gravis, 17. Thalassemia, 18. Ankylosing spondylitis, 19. Chronic osteomyelitis, 20. Rheumatic heart disease, 21. Chronic pulmonary heart disease, 22. Dialysis of chronic renal failure, 23. Outpatient anti-rejection therapy after organ transplantation, 24. Hemophilia, 25. Phenylketonuria.

2. Declaration requirements

1. The applicant must have been insured in Yunyang District for one year or more;

2. The declared disease is limited to one and the declaration materials cannot be submitted repeatedly;

3. Those who have already enjoyed the treatment of chronic diseases cannot make duplicate declarations.

3. Declaration time

From November 1, 2021 to December 20, 2021 (holiday extension), for a period of 50 days, after which it will not be accepted.

1. A copy of the ID card and social security card of the applicant for declaration (if you entrust someone else to do it, you must provide a copy of the ID card of the agent at the same time).

3. The declaration materials required for employee medical insurance and urban and rural residents' medical insurance are the same, as follows: from January 1, 2021 to the present, the comprehensive medical institutions with the second level or above agreement in Yunyang District related to the declared outpatient chronic diseases (tuberculosis and psychiatric-related specialized medical institutions, the level of medical institutions hospitalized in other places should not be lower than the level of the highest-level agreement medical institutions in Yunyang District), the tertiary agreement medical institutions with national key specialties related to the outpatient chronic diseases, and the diagnostic basis of the relevant specialty agreement medical institutions , including relevant inpatient medical records, various examination reports, discharge summaries, etc. If the declaration materials have been included in the hospital's case management, a copy can be provided, but the case number must be marked and stamped with the medical certificate of the treated hospital.
