At the beginning of June 2020, many hospitals in Wuhan have closed the cabin, and the patients have been transferred to our hospital for treatment of complications, and our hospital is the last to close the cabin. In July 2020, the crew of "Chinese Doctors" came to our hospital to recruit medical consultants and put forward three requirements: first, the anti-epidemic time should be long enough; second, they are very proficient in various severe rescue techniques and the treatment of new crown pneumonia; third, the communication and coordination ability should be strong, because there are hundreds of actors and stars who need medical training and let them enter the role of medical staff as soon as possible.

Finally, our dean thought of me. From the beginning of February to the beginning of June 2020, I have been working in the ICU ward of the new crown pneumonia, staying in it for the longest time; I am also a dry and severe patient, with 13 years of intensive medical experience, very proficient in ecmo (extracorporeal membrane oxygenation) various rescue techniques, colleagues call me "ECMO little prince"; in terms of communication, I have no problem, usually more enthusiastic.

In the middle of July 2020, the crew held a symposium in our hospital, and director Liu Weiqiang and I chatted for a while, and finally said that it was you.

Before entering the group, I had a mental struggle, because I had not been home for 4 months before, and I did not want to make another long errand. The crew said that from the start of the film to the end of the game, the medical consultant should follow the whole group, can not let the film appear any shots that violate medical principles, once the shooting starts, it can not go away, originally scheduled for more than 3 months, the actual final work for 5 months.

The three people have different divisions of labor, I am the intensive care medical guidance, the head nurse of our department, Liu Xueqing, is the medical guidance of nursing, and Xiong Wei, the chief technician of the infection management department of our hospital, is the hospital sense guidance, responsible for how to wear and take off protective clothing, how to deal with various medical waste, and the scene construction symbol does not meet the layout of the infectious disease hospital.

On July 31, 2020, three of our medical consultants came to Wuxi Film and Television Base and officially started work on August 1. The film started in October 2020, and in the first two months of the film, we mainly did two jobs, one was to continuously improve the script with the screenwriter, and the other was to train the actors.

When we entered the group, the crew already had a version of the script, the screenwriting teacher told us that this script was the first version, she interviewed a lot of anti-epidemic medical workers to write, but the character image is not three-dimensional enough, the story is not strong enough, so waiting for us, talk to us, like the film's Dean Zhang Jingyu, Director Wenting, and the intern resident, etc., how they speak clinically, how to perform, we also provide a lot of materials and stories to her, together with this script slowly polished, In fact, before the final release, the script is still constantly updated, and the final version of the script is very different from the original first version.

While perfecting the script, we also have to train the actors. The main actors in the training film have to face a problem, their schedule is not the same, not to say that several actors come to train together at the same time, most of the time is that today the actor has a schedule, only three days, we hurry to make a plan, what to learn in these three days, drive out the three days of classes, give him this course alone, so that he can learn everything as soon as possible for three days. A few days later another actor will come again, and we will quickly make a plan for him, what medical knowledge he has learned and what he has not learned, and then give him an explanation training. We usually have students together in class, and we can talk about it once, but the plan for each actor in the crew is different.

Like Yi Qianxi, he is the youngest in our crew, but he is a very modest and studious boy. In the first version of the script, he had very few scenes and his roles were not very three-dimensional. But we discussed with the screenwriters that in the process of fighting the epidemic, young people like the little sheep played by Yi Qianxi accounted for an absolute proportion and should have a process of growth. After that, Yi Qianxi's role slowly became more, and there were many things that needed to be learned, including ECMO, intubation and other theoretical knowledge and operations.

At the shooting scene, Yi Qianxi told us whether we could learn to intubate in between shooting, I said yes, we moved the mold and equipment of the intubation to the room next to the shooting scene, he kept practicing there, practicing at least hundreds of times, like Yuan Quan, Zhu Yawen, Li Chen, etc. Who had learned very skillfully before helped him correct the techniques.

In fact, for young residents who have just been admitted to the hospital for one or two years, intubation is still very difficult. In a large hospital like ours, there are only two kinds of people who can be intubated, one is an anesthesiologist, which is the role of Xin Wei played by Feng Wenjuan in the film, because the anesthesiologist does surgery every day and must be intubated, and the number of intubations is the largest. The second is a critical care doctor like me, who works in the ICU every day. Doctors in general wards have no access to intubation.

At the beginning of August 2020, when we entered the group, the construction of the scene had not yet begun, the venue was basically a flat ground, and the materials were still piled up in that place. The teachers in charge of the scene communicated with us, took the drawings to show us, we found that there was a problem with the drawings, because the layout of the three districts and two passages of the infectious disease hospital was not very reasonable, after the medical workers came out of the ICU, they should take off the protective clothing layer by layer, there were four take-offs, not to say that they would take off at once, after taking off the first layer, they entered the second take-off, and then took off the isolation gown, and then took off the mask and shoe cover of the third layer, and after the 4th floor, they all took off, and only the clothes we wore were left. At that time, the partitions on the drawings seemed to be only one off and two off, which was definitely not OK, to re-plan, to set up doors in each district, how much each sheet was, how many washbasins should be equipped, how to place the curtains, these are very detailed things, we all have to give advice to them in the early stage.

Every day, the director will take us to go through the scene morning and evening to see what problems there are in the construction, if it does not meet the needs of the infectious disease room, it will be changed quickly, and it cannot be adjusted after the construction is completed.

The construction of the scene took us a lot of time, and we looked at the drawings every day, feeling that we had become engineers and could understand the drawings. The crew is also very professional, you just need to tell them what they need, they will quickly come out with drawings, and the revised version will come out immediately. If this is not enough, they will soon produce another version, very efficient, very professional team. As soon as you come up with it, they can give it to you immediately.

In addition to the scene construction, the drugs used during the epidemic must really meet our requirements at that time. Of course, these drugs are not real medicines, real drugs are too wasteful, and very expensive, they are all made of props, but they are exactly the same as the real ones, including the size, shape, color, and packaging box of each bottle. If you think about it, there are hundreds of drugs during the epidemic, each of which is different, and we need to confirm one by one what the packaging of this drug looks like, whether it is powdery or liquid, whether it is colored or not, the props team is very strict with us to confirm, after confirmation, they have spent a lot of effort to completely restore.

In addition to the drugs, the director also asked us to confirm the medical equipment, there were many manufacturers sponsored medical equipment at that time, slowly entered the scene, from the end of September to the beginning of October 2020 will start shooting with these medical equipment, we must be fully prepared, once the shooting and then adjust there is no time. After we read it, we found that some ventilators, dialysis machines, ECMO machines, some outdated, not what we usually use in hospitals now, some ventilators are decades ago, certainly not, if shown on the screen is not very real. We quickly made a request to the director team and quickly changed the machine.

In fact, I am very familiar with those manufacturers, so I matched them, these manufacturers heard that we are going to make such a movie, but also very supportive, quickly transfer the instruments from all over the country, (both sides) still need to have a certain courage to make this decision.

After all the instruments and equipment are debugged, it is also necessary for the actors to learn to use them, understand what the various numbers on the screen of the ventilator and dialysis machine represent, and then they must be able to operate, how to choose, and how to operate is the most in line with the doctor's behavior habits. It's so difficult that even when shooting, the actors are constantly learning. Some are really too difficult, for example, we doctors have been learning this thing for more than ten years, and it is still difficult for zero-based actors to quickly master in one or two months. When the difficulty was too great, the director thought of a way to shoot an action to stop for a while, then shoot another action, and finally edit it.

There is disagreement about the "ICU blackout"

During the filming process, there were also disagreements. Director Liu Weiqiang sometimes wants something that may touch some medical principles, and we must adhere to medical principles at this time. There is a scene in the film, Tao Jun and Wu Chenguang and other experts who have just arrived at Jinyintan Hospital and put into rescue work, the director wants to make that scene completely blackout, all the equipment is also stopped. We told the director that this is impossible, because the ICU is a dual power supply system in the whole hospital, one more power supply system than other wards, and the two power supply systems are completely destroyed, not impossible, but very rare.

What the director asked was that after the power outage, all the instruments and equipment were also stopped. I said, that's even more unlikely, because there are now batteries behind the instruments and equipment, which can last at least half an hour. But the director still insists on his own ideas, trying to create a very hectic, very extreme situation. Then, I told the people on the Bona producer about the quarrel between the two sides, and finally the producer took our advice, after the power outage, the various instruments and equipment continued to operate, but operating in a very inefficient state may cause the patient's condition to worsen.

In fact, at the beginning, we didn't want the ICU blackout scene, and finally everyone compromised. Jinyintan Hospital usually does not have so many serious illness numbers, there are not so many places that need electricity, now all are patients, which place needs electricity, overload will be cut off, there is no problem.

On the day of the premiere of the movie "Chinese Doctor" on July 4, I did not catch up, I watched it on the day of the release of the movie. To say that there are too many places that touch me in the movie, such as Zhang Zifeng (receiving the death notice of his parents), the sentence that turned his head to the past, the tears really can't be contained. But as a medical worker, what impressed me most was the scene where experts discussed how to treat patients scientifically, arguing about whether to intubate or not. This scene was the last thing we added, and it touched me a lot.

The scene involved a lot of medical terms, and the screenwriter said that the professional vocabulary was too awkward, and I said, it should be like this, so that others feel that we are very professional. In fact, in the end, the screenwriter processed it artistically and expressed it in a more smooth word. At first, I was worried that they would be unprofessional, but after reading it, I thought it was quite real, and we usually discuss difficult cases like this. It says oxygen and index, carbon dioxide, oxygen how to get in, many medical terminology is very accurate. This scene seems to be very calm, but in fact, we are thinking about what is going on with this disease, how to treat it, and provide the world with China's anti-epidemic experience.
