In order to meet the needs of the people in handling affairs in different places, provide convenient and intimate services, and effectively serve the vast number of orphans and de facto unsupported children, according to the relevant documents, Guangdong Province will uniformly use the National Child Welfare Information System (hereinafter referred to as the information system) to accept applications for the identification of orphans and de facto unsupported children in accordance with the method of off-site collection and handling, and realize "inter-provincial communication".
1. Bidding method
The identification of orphans or de facto unsupported children shall be handled in the form of arbitrarily accepting the application and the place of household registration is responsible for reviewing, and the applicant's application for the identification of orphans or de facto unsupported children may be submitted to any township people's government (subdistrict office) nationwide, and is not subject to restrictions on the place of household registration.
2. Application conditions
Orphans: Minors under the age of 18 who have lost their parents or whose birth parents cannot be found.
De facto unsupported children: children whose parents are both seriously disabled, seriously ill, serving a sentence in custody, compulsory isolation for drug rehabilitation, who have been subject to other measures restricting personal freedom, who have lost contact, who have been disqualified from guardianship, who have been deported (deported);
3. Application process
1. Submit your application. The identification of orphans and de facto unsupported children shall be handled in the form of arbitrarily accepting applications, pushing them online, and reviewing them with the responsibility of the place of household registration. Guardians or orphans themselves (orphans who have reached the age of 18 and are still enrolled in full-time schools) may apply to any township people's government (subdistrict office) nationwide for the identification of orphans or de facto unsupported children.
2. Acceptance and inspection. The township people's government (subdistrict office) of the place of acceptance shall inform the applicant of the materials required at one time, and conduct a formal review and identity verification of the application materials.
3. Material flow. The township people's government (subdistrict office) of the place of acceptance shall enter the application materials into the information system within 3 working days from the date of receipt of the application materials and push them to the township people's government (subdistrict office) of the applicant's household registration.
4. Preliminary examination of qualifications. The township people's government (subdistrict office) of the place of household registration shall submit preliminary opinions within 15 working days from the date of receipt of the push materials, and report to the civil affairs department of the people's government at the county level.
5. Qualification. The civil affairs department of the people's government at the county level shall complete the review work within 15 working days from the date of receipt of the relevant materials of the township people's government (subdistrict office), and feedback the handling results to the township people's government (subdistrict office) of the place of acceptance through the national system.
6. Inform the results. The township people's government (subdistrict office) of the place of acceptance shall inform the applicant within 3 working days from the receipt of the handling result.
7. Implementation of benefits. Where children are identified as orphans or de facto unsupported children, the civil affairs department of the place of household registration shall issue the orphans' basic living expenses and the basic subsistence allowance for de facto unsupported children the following month.
4. Application materials
(1) Materials required for applying for recognition of orphans living in the social diaspora:
1. "Approval Form for the Declaration and Approval of the Basic Living Expenses of Orphans living in the Social Diaspora" (Annex 1);
2. Orphan Care Agreement (Annex 2);
3. The original and copy of the orphan and guardian's household registration booklet and ID card;
4. The original and copy of the relevant certificate of loss of parents, including:
Where a parent dies, submit the page or certificate issued by the public security organ for the orphan's parent's death account booklet, or the orphan's parent's death certificate issued by the medical and health unit, or the orphan's parent's death cremation certificate issued by the funeral home.
Where parents cannot be found or their parents are missing, submit a certificate of disappearance issued by the people's court or a certificate issued by the public security department that the parents cannot be found;
5. The original and copy of the guardian qualification certificate (such as relevant judgments, mediation documents, agreements, notarized documents, proof materials of the residential (village) committee, etc.).
6. Orphans recent 1 inch crown-free photos.
7. Orphans who have reached the age of 18 and are enrolled in full-time compulsory education stage schools, ordinary high schools, secondary vocational schools, higher vocational schools and ordinary undergraduate colleges and universities must submit the original and copy of the certificate issued by the school.
8. A copy of the orphan's or orphan's guardian's bank card.
(2) Materials required for the application to establish factual unsupported children
1. Application Form for Basic Subsistence Allowance for De facto unsupported Children (Annex 3)
2. Agreement on the Use of Basic Subsistence Security Funds for De facto unsupported children (Annex 4)
3. Identification materials of the factual unsupported child:
(1) The fact that the child who has no one to support him/her household registration book or birth certificate.
(2) Identity card of the de facto unsupported child.
(3) Factual 1-inch photo of an unsupported child without a crown recently.
(4) If the fact that the unsupported child has reached the age of 18 and is still enrolled in a full-time school, he or she must provide a student card or a certificate of enrollment issued by the school within the validity period.
4. Parents' identity card, household registration booklet, marriage certificate or divorce certificate, and the following supporting materials:
(1) Death: It is necessary to provide the "Medical Certificate (Inference) of The Death of a Resident" ("Death Certificate") issued by the medical institution or public security organ, or the cremation certificate issued by the funeral home, or the legal document issued by the people's court declaring death.
(2) Disappearance: It is necessary to provide a certificate of disappearance issued by the people's court or a certificate issued by the public security organ that the parents cannot be found;
(3) Serious illness: a diagnosis certificate issued by a second-level or above medical institution is required.
(4) Severe disability: It is necessary to provide a first- or second-level disabled person's certificate or a third- or fourth-level intellectual disability or mental disability certificate within the validity period.
(5) Serving a sentence in custody, compulsory isolation for drug rehabilitation, or other measures restricting personal freedom: a written judgment (decision) issued by the people's court, or a certificate of detention or detention issued by a public security organ, a "Decision on Compulsory Isolation for Drug Rehabilitation", etc. must be provided.
(6) Missing contact: It is necessary to provide the public security department to issue the "Receipt for the Search of The Missing Parents of Children" (Annex 5), or the "Form for the Identification of the Missing Parents of Children's Parents" (Annex 6), or adopt the method of "one matter and one discussion", the village (resident) committee will propose a plan, and after inspection by the township people's government (subdistrict office), report to the county-level child protection related coordination mechanism research confirmation results.
(7) Revoked guardianship qualifications: A judgment on the revocation of guardianship qualifications issued by the people's court is required.
(8) Deportation (deportation): Proof of deportation (deportation) in accordance with law is required.
5. Guardian certification materials:
(1) Identity card and household registration book of the legal guardian or the legal maintenance or maintenance obligor.
(2) Guardian qualification documents (such as relevant judgments, mediation documents, agreements, notarial certificates or village (resident) committee certification materials).
6. A copy of the guardian's or child's own bank card.
