Lancet video | Cao Bin: Always be a "resident doctor", constantly grow and work hard

author:柳叶刀TheLancet

"Learning ability" is something that runs through a doctor's career for a lifetime. A doctor may feel that the stage of his or her fastest growth, that is, the stage of life with the greatest growth slope, is the residency stage. So no matter how old you are — Xiao Cao, Lao Cao and Old Man Cao — treat yourself as a "resident doctor" and keep growing and working hard.

When talking about topics such as "doctor" and "teaching teacher", Mr. Cao's mood will become particularly good, which is his work experience in the past 23 years and a portrayal of his hard work day after day. In the conference room of the China-Japan Friendship Hospital, Teacher Cao and we had a dialogue. The following is the dictation of Teacher Cao Bin, which we have sorted out slightly.

Communication with patients: building a relationship of trust in a short period of time

The communication and communication between doctors and each patient is different, because each patient has his own needs, so it is impossible to program the rules of seeing a doctor. No matter how close the physical distance between people and people is, in fact, the distance between hearts is quite far, especially in a short period of time, it is more difficult to establish a relationship of trust. If it's a patient from out of town and I happen to be in that place again, I might talk about the scenery or food that is characteristic of the place, so that I can at least get closer to the patient at some point.

During the outpatient visit, the doctor should understand in a short period of time what problem the patient wants to solve. In addition to the patient's own description of his or her own pain, the doctor also needs to take the initiative to inquire and examine the body, see the medical information brought by the patient to make a judgment, and give the patient and his family a reasonable explanation and let them understand in a short period of time. People who do not study medicine may not understand this job very well, and think that this matter may be very simple, but in such a short period of time, you have to make analytical judgments from some very small clues, and give him reasonable suggestions, this process is a high degree of physical and mental work.

Teaching and inheritance: Benefit from teachers and be more willing to share

For patient care, doctors can do it alone, but often require a team, including young doctors and older doctors. The older doctor has the responsibility to help and guide the young doctor, so that he can adapt to the team as soon as possible, be more capable, so that the whole work becomes more efficient, and this process is the process of teaching.

Because a doctor needs to master a lot of knowledge, the cycle of education is also particularly long, so this is more reflective of the importance of experienced superior doctors to teach. We have been learning since we were medical students, and we are learning from teachers. The more I benefit from the teachings of my superiors, the more willing I am to share my experience, a process called inheritance.

What does learning come from? The source is unknown. Sometimes, the doctor may have limited ability to help patients, and what I learn may not be able to meet the requirements of the patient I am facing, so what should I do? Most doctors will go to the literature to see if their peers have similar experience, or introduce them to experienced doctors, and then learn, and then give patients advice when they encounter similar situations. The process by which we try to solve those unknown problems is the so-called research process. As long as the doctor has enough enthusiasm for the profession and has enough reverence for life, he will surely find some problems.

Professional feedback: maintain curiosity, cure the patient at the same time as the patient is cured

My teachers, both in their 80s, still retain curiosity. If you find that "this case is very interesting" during the discussion, you will write down what you don't know in the notebook first, and then ask the young doctor repeatedly. She may feel that young doctors are more quick to accept new knowledge and have a broader mind. Her idea was: Although I am 80 years old and the young doctor opposite me is 30 years old, I should humbly ask for advice if I don't understand. To be able to make such a move must be a good doctor. Fortunately, the profession of doctor keeps me curious.

Due to the characteristics of the profession, doctors have a long training cycle, the intensity of work is not small, and they must also take care of their families. But when you go all out to help the patient, give very helpful advice, and the patient also gives positive feedback, you are happy in your heart, and you have a sense of accomplishment, which is the healing effect of this profession.

Professional life: Xiao Cao, Lao Cao and Old Man Cao

I told my graduate students that a person's life is long and short, and a doctor's career is up to 40 years. I often think that a student who has spent 3 to 5 years studying with me, in addition to professional knowledge, I also want to teach them a way of looking at problems, discovering their own strengths, and having the opportunity to release their abilities, such a learning and teaching process I think is the best.

For me, the professional life of a doctor can be divided into three stages, that is, Xiao Cao, Lao Cao and Cao LaoTou. Xiao Cao refers to the stage of learning and education in medicine and later becoming a young doctor, this stage continues to accumulate experience, more of a learning role, is an object of education. I think that although this stage is busy, it is actually quite happy, because anyway, you are Xiao Cao, and there is nothing wrong with one thing, because you are still learning and growing.

When you become Lao Cao, you have a responsibility. Of course, what you see outside is that you may have a certain qualification or popularity, but this time it is more of a mentor role, guiding young doctors to do their work. At this stage, the pressure is actually quite large, and it is necessary to be independent in medical treatment, and even to play the role of the final decision. At the same time, there are more social positions, including departments, hospitals, disciplines or all levels of social development to consider a lot of issues, to take everyone forward together. But in fact, to the stage of Lao Cao also need to learn, this time should be more aware of their own shortcomings. At this time, there is no innate advantage, because when Xiao Cao is young, others love to teach you, and when you are old Cao, others do not love to teach you, so at this time you are actually a weak person in learning, which means that you have to take the initiative to find opportunities, including reading books and reading literature, and finding opportunities to learn from young people. It's not that these young doctors should be your students, but there are many situations from whom you want to learn. At this time, Lao Cao had to firmly tell Xiao Cao, "In fact, I am not so strong, you don't have to lift me up so high, as if I can't understand anything."

By the time of Old Man Cao, colleagues have basically grown up, and you or not have little impact on the department, in fact, at this time you are more standing outside the circle, and then looking at your colleagues with admiring eyes. Old Man Cao is still slightly cute, and occasionally gets together with colleagues. At this stage, although he often reads books and keeps up with the times, but more as a person who has come over, Old Man Cao will give Xiao Cao and Lao Cao more encouragement, share more of his own lessons, and let everyone go as smoothly as possible. But for him personally, Old Man Cao is actually a bit lost, because he will feel that his colleagues don't need you anymore. So at this time, you may need to find some places that you feel are still useful.

An understanding of life

Doctors work in an environment that gives us more understanding of life than other professions. When entering the clinic, we inevitably consider what is life and what is death. Life is a process, death is a point, death and life are the same, it is all part of nature. Man is nothing more than a combination of several elements, the combination of several elements is life, and falling apart and returning to nature is death.

When we understand this, the unhappiness we see, hear, or feel every day doesn't seem so important. The past has already happened, it is useless to think about it, tomorrow's things do not know what, just do today's things first. A simple truth: When you make any choice, you give up other choices — when you put on this white coat, you can no longer be a movie star. Therefore, life is a defect, a defect is life, a defect is beauty. So we don't want to be perfect for ourselves, you have to know where you are strong and where you are weak, you have to know your strengths and weaknesses.

I hope that when I leave the world, I can still have one or two people closest to me who can hold my hand, I can still read a book or two that I like to read, and I can say a word or two to them, and then I can go with confidence and enter the stage of elemental decomposition.

Finally, one sentence: always be a "resident doctor". Learning ability, this is something that runs through your career for a lifetime. A doctor may feel that the stage of his or her growth is the fastest, that is, the stage of life where the growth slope is the largest, that is, the residency stage. So no matter how old you are, Xiao Cao, Lao Cao and Old Man Cao all regard themselves as a "resident doctor", constantly growing and working hard, then you as a doctor will not be in vain.
