Ji Xiaolong: If you don't do anything more, you'll be finished! Only 20% of county hospitals can see pathology!

"At present, the development of pathology in China has reached the brink of collapse!" When the reporter asked how to view the development of the primary pathology department, Ji Xiaolong, director of the pathology department of the Armed Police General Hospital, answered worriedly.

As a basic clinical department, the Department of Pathology undertakes the clinical pathological diagnosis, difficult pathological diagnosis, difficult pathological consultation, health examination and other work of the whole hospital, providing clear pathological diagnosis for the clinic, determining the nature of the disease, and identifying the cause of death. Compared with other diagnostic methods such as B ultrasound, conventional radiation, CT, etc., the pathological diagnosis is still recognized by the medical community in various countries in the world as the most reliable, the most reproducible and the most accurate is still the pathological diagnosis, because the pathological diagnosis report is not an imaging description, but a clear disease name, and the clinician mainly decides the treatment principle, estimates the prognosis and explains the clinical symptoms according to the pathological report. This authority of pathological diagnosis determines its central role in all diagnostic means, so the quality of pathological diagnosis not only has a great impact on the relevant departments but also on the overall medical quality of the hospital.

Pathology development is facing collapse

Ji Xiaolong explained that in more than 2,000 counties in China, only one-third of the county-level hospitals have pathology departments, and the pathology departments of county-level hospitals are basically living in mud. The data shows that only one-third of the pathology departments of county-level hospitals still have 30% of the diagnostic results that are wrong, and the development of pathology departments in county-level hospitals is quite severe.

The reason why the pathology department of the county-level hospital cannot develop, Ji Xiaolong believes, is that the first reason is that the managers of the county-level hospital do not pay enough attention to it, that the pathology department cannot bring benefits to the hospital, and to a certain extent, the setting of the pathology department may also invest a lot of money, and the gain is not worth the loss.

Secondly, it is because it is difficult to cultivate doctors in the department of pathology. "At present, there is still no standardized training for pathologists in China, and even if there are pathologists in county-level hospitals, they are not willing to stay in county-level hospitals due to income problems." Ji Xiaolong said worriedly, "Another reason is that the unchanged pricing of pathological examinations for decades has seriously restricted the development of pathology and pathology. ”

Of course, the pathology department of county-level hospitals also has a better development, such as the economically developed coastal cities, hospital managers are aware of the importance of pathology, and do not expect the pathology department to bring benefits to it, but will strengthen the investment in the pathology department.

The development of the hospital is not lacking in pathology

Ji Xiaolong believes that if a hospital wants to develop well, it must first have a pathology department. Whether it is a county-level hospital or other medical institutions, if there is no pathology department, doctors often dare not draw conclusions when diagnosing, for fear of misdiagnosis, etc., which is actually a reason why patients in county-level hospitals run to large hospitals for treatment.

"I want to tell those who come to study through this training course that without pathology, hospitals cannot develop. I want them to understand what pathology is for, what role it does, what problems pathology can help them solve in the diagnostic process, and as a doctor, it is very important to understand pathology. Ji Xiaolong said in a serious tone.

Digital pathology is the future direction

On the wall of the pathology department of the Armed Police General Hospital, the words "Digital Pathology Remote Consultation Center" came into view of the reporter. Ji Xiaolong told reporters that in the future development process, glass slices into digital slices are the inevitable development trend of pathology. Subsequently, Ji Xiaolong led the reporter to visit the room where he usually applied digital pathology remote consultation. "This is the digital pathology slice I look at every day now." Ji Xiaolong skillfully demonstrated to the reporter that his monthly workload was densely recorded in the huge computer, sometimes even up to hundreds of cases. Most of these cases come from county-level hospitals.

At present, the digital pathology remote consultation center led by Ji Xiaolong has been carried out for 3 years, although the coverage area is not very large, but this consultation center is mainly for the grass-roots level. "Grassroots hospitals require technical guidance on pathology far more than hospitals in big cities." Ji Xiaolong said.

However, Ji Xiaolong also stressed that even if the development of digital pathology remote consultation centers can help county-level hospitals provide professional technical guidance, it is still necessary for county-level hospital managers to change the original concept, invest funds to introduce the equipment and instruments required by pathology departments, and join the digital pathology remote consultation centers to improve the shortcomings of the development of pathology departments in county-level hospitals.

On December 16-18, 2016, the second round of training courses of the School of Liver Disease of China Brand Doctor College initiated and hosted by China County Health magazine was held at Beijing Business Hotel. At that time, Professor Ji Xiaolong of the Department of Pathology of the Armed Police General Hospital, Professor Meng Qinghua of the Department of Severe Hepatology of Beijing You'an Hospital Affiliated to Capital Medical University, Professor Liu Yulan of the Department of Gastroenterology of Peking University People's Hospital, and Professor Li Weiwei of the Cirrhosis Diagnosis and Treatment Center of the People's Liberation Army 302 Hospital will personally attend the scene, guiding the development of the Hepatology Department of the vast number of county-level hospitals in China!
