Continue to improve the conditions for running compulsory education, maintain the basic stability of the existing health support policies during the transition period, and continue to strengthen the construction of village-level comprehensive service facilities in poverty alleviation areas... The Opinions of the CPC Central Committee and the State Council on Consolidating and Expanding the Achievements in Poverty Alleviation and Effectively Linking rural Revitalization make it clear that it is necessary to further improve the level of public services in poverty alleviation areas.

What is the implementation of this key work, what are the expectations of the masses in various localities, and what do the departments concerned intend to do? This reporter recently visited Yun County, Lincang City, Yunnan Province, and Shilou County, Lüliang City, Shanxi Province.

"The previous support policy is still there"

"Fortunately, the good policy is still there, otherwise it will really be dragged down!" Yun County Dazhai Town Tangliba Village Zhentou Group villager Zhang Wenhua this is not an exaggeration, 3 years ago, the mother treated glaucoma surgery, in February this year the father found the tumor, if it is not medical insurance reimbursement, first diagnosis and treatment after payment, one-stop settlement policy, he really does not know how to spend so much money at once.

In 2018, Zhang Wenhua's mother, Li Zhengcai, suffered from glaucoma and needed to go to YunXian People's Hospital for surgery. "At that time, the poor households who had set up a file could see a doctor first and pay later, and my mother was hospitalized quickly, so I basically did not worry about the problem of medical expenses, and I could concentrate on taking care of my mother."

After getting rid of poverty and removing the hat, the disease once again quietly targeted the family. One night in February this year, his father, Zhang Zhengwang, suddenly had a severe stomach pain, and after checking at the Yun County People's Hospital, he found that there was a tumor in the intestine. The condition is urgent, and surgery must be done to confirm whether it is a malignant tumor, which makes Zhang Wenhua difficult.

"In the past, the card household could be diagnosed and treated first and then paid, but now that it is out of poverty, I don't know if the good policy is still there?" Zhang Wenhua said that although the family conditions have improved a lot after poverty alleviation, after all, the old man is old, and the family's income mainly depends on his own four or five acres of land, if you want to raise tens of thousands of yuan at once, it is still not easy. "And my father is hospitalized, and I have to take care of it." The family's land can not be planted, the cattle are not raised, in case the source of income is cut off, how can the money be repaid next? ”

Attending doctor Lin Yingfang saw Zhang Wenhua's concerns: "The policy of poverty alleviation is not free, the previous support policies are still in place, or you can enjoy the first diagnosis and treatment and then pay, and then one-stop settlement when discharged." There are basic medical insurance, medical insurance for serious diseases, medical assistance, and the government's four-fold guarantee, don't worry about money, first accompany the elderly to see a good doctor!"

These words made the stone in Zhang Wenhua's heart fall to the ground. "During the hospitalization, the hospital never urged money, and the doctors and nurses were very good, especially concerned about my father's postoperative recovery, and our family felt very warm." Zhang Wenhua said.

On the day of discharge, the hospital settled the costs, surgery fees, medicine fees, and examination fees, adding up to more than 23,000 yuan. After enjoying the basic medical insurance, medical insurance for serious illness, and the protection of urban and rural residents, Zhang Wenhua only paid more than 2300 yuan, and the reimbursement ratio reached more than 90%.

Today, Zhang Wenhua also takes his father to the county hospital every 20 days for follow-up treatment, but most of the costs can still be reimbursed. Zhang Wenhua said: "The reimbursement policy is still there, saving a lot of medical expenses, and the pressure brought by the treatment of diseases to the family is not so great." ”

"Let the old people in the village eat a good meal"

When he met Chen Jiyuan in Mazhuang Village, Longjiao Township, Shilou County, he was having lunch at the village's day care center. The noodle toppings for lunch are tomato eggs, just a bite of onions, suck a bite of noodles, Chen Jiyuan ate with relish. After eating, he wiped his mouth: "Good grin!" Holding the corner of the table with one hand, grabbing the crutches in the other hand, slowly walking out.

Head-on encounter with Feng Siying, the village party secretary who came out of the village convenience service center. Chen Jiyuan shouted from afar: "Has the money in my 'kaka' arrived?" "Do you mean money for pension insurance?" When it's time, just go home and wait. Feng Siying said, "After a while, the agent of our service station will go to the ATM to help you withdraw the money and send it to your home." ”

Chen Jiyuan, 67 years old, has become increasingly inconvenient in moving in recent years due to his right leg disability, and "even daily eating has become a problem." One winter in 2018, Feng Siying went to the house to look for him, but found him nibbling on dried steamed bread. Feng Siying hurriedly dragged him to his home to eat, but Chen Jiyuan kept waving his hands: "After eating, I am not hungry!"

This situation is quite common in the village. Feng Siying said: "The permanent population of our village is 570 people, 278 people over 60 years old, and 73 people over 80 years old. To this end, in 2019, the village raised funds from many sources, picked up several old houses of the village collective, and built a day care center for the elderly, aiming at a goal: "Let the elderly in the village eat well." ”

The care center began operation in July of that year, and the daily expenses were borne by the village collective. Chen Jiyuan came to eat a few meals and was very satisfied: "Every day I 'change tricks' to eat, and I don't want money!"

With the operation of the day care center, Mazhuang Village was officially lifted out of poverty that year. But after a while, Feng Siying found a new situation: "The number of elderly people who come to eat is slowly decreasing, especially in winter." The reason was quickly found: "Although the village is not large, but in the cold winter and waxing moon, the old people's legs and feet are not good, it is still inconvenient." ”

Last year, Wang Ruiping, director of the Shilou County Civil Affairs Bureau, came to Mazhuang Village to investigate and also found this situation. "After poverty alleviation, comprehensive service facilities should continue to keep up." The county has selected several pilots to help establish homes for the elderly in villages with more elderly people, and Mazhuang Village is one of the pilots.

At present, on the basis of the original day care center, Mazhuang Village has expanded 5 rooms on the left and right, with water, electricity and heating. "In the future, this will be a big platform, and the convenience service center will also move here, sending and receiving express delivery, unified handling of certificates and receiving subsidies, and other matters, one-stop service." Not long ago, with the financial support of the county, Feng Siying installed a television for the center.

"Enhance the ability and level of service to the people"

During the transition period, the existing health support policy will be basically stable, so that families like Zhang Wenhua have hope and confidence in the face of difficulties, and after poverty alleviation, the local government is also actively exploring ways to continuously improve the diagnosis and treatment capacity of county-level hospitals.

"The county can cure the disease, who wants to run to the province?" Shi Ling, vice president of Yunxian People's Hospital, believes that in addition to continuing the previous health assistance policies such as first diagnosis and treatment and post-payment, one-stop settlement, etc., to reduce the pressure and cost burden of patients' hospitalization advances, it is also necessary to improve the level of grass-roots medical care and solve the problem of rural people seeking medical treatment nearby. "Through a variety of linkage methods such as specialized alliances between large hospitals and grass-roots hospitals, telemedicine, etc., we will strengthen the construction of talent teams in grass-roots medical institutions, improve the diagnosis and treatment capabilities of common diseases and multiple diseases, and let the masses cure diseases at the doorstep of their homes." Shi Ling said.

In the long run, to improve the health level of the people in poverty-alleviated areas, we must also promote the prevention threshold to move forward. In Yun County, the construction of a compact medical community was launched in 2014, and since 2019, it has begun to explore the total amount of medical insurance funds packaged and paid, forcing hospitals to shift from "increasing income" to "reasonable cost control". "After the reform, the county and rural tertiary medical institutions have paid more attention to prevention and rehabilitation, not only by improving the family doctor contract signing system, publicizing healthy living habits to the masses, etc. to 'prevent pre-existing diseases', but also establishing a 'county-township-village' three-level rehabilitation and treatment management system, which not only improves the quality of life of the masses, but also reduces the burden on hospitals and society." Shi Ling said.

In Shilou County, more and more elderly people like Chen Jiyuan can enjoy high-quality public services without leaving the village. Wang Ruiping introduced that since 2018, the local government has invested 2.5 million yuan to transform idle places in rural areas, increased chess and card rooms, lounges, restaurants and other facilities, and built nearly 50 "semi-managed" day care centers, achieving full coverage of villages with more than 1,000 people in the county. "In the future, we will continue to improve the village-level public convenience service capabilities and create more complete comprehensive service facilities."

"Continue to strengthen the construction of village-level comprehensive service facilities in poverty alleviation areas, and improve the ability and level of service for the people." Liu Zhijie, director of the Department of Agriculture and Rural Affairs of Shanxi Province, believes that the foundations of different regions are different, and it is necessary to pay attention to the classification of policies and grasp the key links. "For example, it is necessary to gradually improve the long-term mechanism for the operation of village day care centers and clinics; we must build a number of rural infrastructure and public service projects, accelerate the promotion of the whole village, build the whole village, and support the whole village, and continuously improve the production and living conditions of the masses."
