Q:
What is Omentum Cake?
A:
The large omentum thickens under the pathogenic factors, increases the density, is uneven, and resembles a "cake" located between the anterior abdominal wall and the intestinal canal, called "omentum cake".
Omental dissection
Ovarian cancer is generally considered to be the most common malignancy causing omental cake, as in the above cases, in fact, peritoneal metastasis of other abdominal malignancies can also cause omental cake. Statistics: 35% of metastatic tumors are ovarian cancer, 30% are colon cancer, 10% are gastric cancer, and other malignant tumors account for about 5%.
What does the image show?
Omental paieties are mainly seen in the thickening of the large omentum, which is presented on CT as peritoneal localized or diffuse thickening, irregular or round, can be adhered to into clumps, and its internal density may be uniform or uneven. At the same time, the adjacent mesenteric membrane can be thickened.
Diagnostic considerations
The focus is to raise awareness, and when reading films, you can start from the following aspects:
(1) Because the omentum and peritoneum have a large range, the degree of lesions may be mild, so the observation should be comprehensive and careful;
(2) It is necessary to pay attention to the adjustment of the window width, and at the same time, the key cases should be multi-faceted, multi-sectional, multi-site scanning observation to reduce omission;
(3) In the diagnosis, it is necessary to pay attention to the omentum, visceral, wall peritoneum and its formations (as well as mesentery, ligaments, etc.) have their specific site characteristics and membranous structure lesion characteristics.
