How the accident happened

◆ At around 15:40 on the 28th, Lin's friend Zhuo Moudeng and his family came to the Astro Boy convenience store.
◆ At about 19:53, Lin and Zhuo started a dinner in a convenience store and drank alcohol.
◆ At about 20:07, Chen Feng (Lin's wife) left the store. After that, a total of 7 people, including Lin, Lin Moubin (Lin's son), Lin Mouying (Lin's daughter), Chen Moujie (Chen Feng's nephew), Zhuo Moudeng, Chen Yan (Zhuo's wife), and Zhuo Mouling (Zhuo's daughter), went to rest in 2 compartments on the east side of the mezzanine of the convenience store.
◆ At about 5:50 on the 29th, Lin Moubin heard the sound and found thick smoke, and then woke Lin up.
◆ At 5:57, the outdoor billboard on the east side of the store was demolished by Lin, and four people, including Lin, Lin Moubin, Lin Mouying, and Zhuo Mouling, escaped through the window, and Lin and Zhuo Mouling were sent to the hospital for treatment after escaping. Three people, including Zhuo Moudeng , Chen Yan and Chen Moujie, were harmed by toxic smoke and were trapped in the mezzanine. At 6:18, rescuers successively searched and rescued 3 trapped people from the mezzanine and sent them to the hospital for treatment, but they died after rescue.
The direct cause of the accident
Based on comprehensive investigation and inquiry, on-site inspection, video analysis, inspection and identification, etc., the Fuzhou Fire and Rescue Detachment issued the "Fire Accident Identification", which determined that the fire started at about 05:50 on October 29, 2023, and the fire point was located on the ground on the north side of the mahjong table in the middle area of the mezzanine of the convenience store.
Indirect causes of the accident
First, the safety awareness of store operators and overnight personnel is weak. Store operators and overnight personnel lack basic safety awareness, take temporary rest on the mezzanine floor of the store in violation of regulations, and fail to use mosquito coils safely and prudently, laying hidden dangers for subsequent fires and casualties. Prior to the incident, the local community and relevant departments had gone to the store to carry out safety inspections and publicity tips, but the store operator had resisted this and failed to cooperate with and accept the safety inspection and safety publicity tips. On October 16, 2023, the staff of Qingxin Community went to the store to carry out safety publicity tips, and issued a safety publicity reminder leaflet of "Strictly prohibiting 'three-in-one' places" to Chen Feng, the operator of the store, reminding that no one should live in violation of regulations in the store, but it did not attract the attention of the operator.
Second, there is a lack of territorial supervision responsibility, and the investigation and rectification of hidden dangers and publicity and education are not effective. Qingkou Town and Qingxin Community did not conscientiously implement the fire safety grid management system, and the grid personnel did not accurately grasp the key points of the safety inspection of the stores along the street, and failed to accurately identify "whether there is a 'three-in-one' problem"; In the safety inspection, the willingness to find hidden problems is not strong, and the lack of awareness of being more serious, in the case that the store operators do not cooperate with the inspection, the inspectors of Qingkou Town and Qingxin Community do not conduct in-depth inspections of the store, and do not carry out safety inspections on the mezzanine. The Qingkou Town Government and Qingxin Community have not been targeted and strengthened enough to investigate and rectify the problem of illegal residents in the stores along the street in their jurisdiction. The Qingkou Town Government has failed to supervise and guide the fire protection work in the Qingkou community, and has neglected inspection, management, guidance and assistance for the investigation and rectification of fire hazards in the Qingxin community, fire protection publicity and education, etc.
Third, the linkage of departmental supervision is insufficient, and the implementation of territorial work has not been effectively guided. The public security police station did not conscientiously carry out daily fire supervision and inspection and fire publicity and education, and failed to effectively enhance the public's awareness of safety precautions. The fire and rescue department did not seriously guide Qingkou Town and Qingxin Community to carry out safety inspections, safety publicity, education and training, etc., and supervised and guided Qingkou Town and Qingxin Community to carry out the rectification of illegal residents in stores along the street, and the "five plus three" mandatory investigation of grassroots fire protection. The Qingkou Squadron of the Minhou County Urban and Rural Construction Management and Supervision Brigade did not investigate and deal with the illegal acts of outdoor advertising, and failed to discover the problem of outdoor billboards blocking the windows of the Astro Boy convenience store in a timely manner, affecting the escape of personnel.
Fourth, the implementation of property management responsibilities is not in place. Fuzhou Fuwanjia Property Management Co., Ltd., the property management unit of the community where the accident occurred, did not formulate a fire safety management system based on the actual situation of the community; Failure to take effective measures to dissuade and stop the relevant owners and operators of the long-standing illegal residents of the mezzanine storefronts along the street in the community; Failure to regularly maintain and manage the common fire protection facilities in the property service area.
The accident investigation report puts forward suggestions for dealing with the units and personnel responsible for the accident. Among them, one person responsible for the accident has died in the accident, and his criminal responsibility is no longer pursued; 1 operator of the fire site was transferred to the public security organ for investigation and handling in accordance with the law; In the course of the investigation of the accident, the seven public officials of the local government and relevant departments were found to have clues and relevant materials on the performance of their duties, and handed them over to the discipline inspection and supervision organs.
