On the third day of the 2024 AATS Conference, the Voice of China shared wonderfully.

introduction

The 104th Annual Meeting of the American Academy of Thoracic Surgery (AATS) kicked off on April 27, 2024 local time at the Metropolitan Convention Center in Toronto, Ontario, Canada. This year, Chinese thoracic surgeons have contributed more than 30 high-quality clinical studies related to the diagnosis and treatment of lung cancer. The results of the study will be presented in both an "oral presentation" and a "poster presentation". On the third day of the conference, the "medical community" will organize and present the clinical results presented by Chinese scholars on April 29, 2024 for the benefit of readers.

【Oral Presentation】

1. Prof. Yi-Long Wu: Disease characteristics and treatment outcomes of patients with surgically resected early-stage ALK-positive non-small cell lung cancer (NSCLC) (Guangdong Provincial People's Hospital)

Surgical resection is the mainstay of treatment for stage I-III non-small cell lung cancer (NSCLC). In patients with ALK-positive NSCLC with stage IB-IIIA, although adjuvant platinum-based chemotherapy is recommended, the efficacy is not significant. Alectinib is widely recognized as the preferred first-line treatment for advanced ALK+ NSCLC. ALINA (NCT03456076) is a Phase III, randomized, open-label trial comparing the efficacy and safety of alectinib versus adjuvant chemotherapy in patients with ALK+ NSCLC after radical resection IB (≥4 cm) - stage IIIA (seventh edition UICC/AJCC). Alectinib was found to show consistent DFS (disease-free survival) benefits across different lymph node stages, with a high safety profile. Therefore, alectinib provides an effective new treatment strategy for patients with ALK+ NSCLC who have undergone surgical resection.

1. Dr. Ran Zhong: Detection of Exhaled Volatile Organic Compounds by High-Voltage Photoionization Time-of-Flight Mass Spectrometry for Diagnosis of Lung Cancer (The First Affiliated Hospital of Guangzhou Medical University/National Center for Respiratory Medicine)

This study aims to evaluate the potential of high-pressure photoionization time-of-flight mass spectrometry in the detection of exhaled volatile organic compounds (VOCs) in the diagnosis of lung cancer. Traditional diagnostic techniques have limitations such as invasiveness and high cost. Exhaled organic compound analysis has the advantages of rapid, non-invasive, and convenient sampling. The study found that there were differences in VOC changes between healthy individuals and patients with lung cancer or nodules, and there were significant differences in the results of some VOCs in lung cancer patients with different TNM stages and different lung adenocarcinoma types. This study highlights the potential of exhaled metabolites in the diagnosis and treatment of lung cancer.

2. Dr. Hengrui Liang: LungPath: Improving the Diagnosis of Early Invasive Lung Adenocarcinoma Based on AI-Driven Histology Pattern Recognition (The First Affiliated Hospital of Guangzhou Medical University/National Center for Respiratory Medicine)

This study aims to develop a deep learning algorithm and evaluate its clinical ability to distinguish histological patterns in preoperative computed tomography (CT)-based early invasive lung adenocarcinoma (ADC). We developed a deep learning algorithm called LungPath for automatically segmenting lung nodules and predicting high-risk histological patterns in early lung ADCs based on CT scans.

3. Dr. Shuliang Zhang: Preliminary clinical experience of a novel robot-assisted navigation system for CT-guided percutaneous lung microwave ablation (Union Hospital Affiliated to Fujian Medical University)

Several studies have shown that local thermal ablation can be an alternative treatment option for patients with stage I non-small cell lung cancer who cannot undergo lobectomy. However, traditional ablation techniques rely heavily on the experience of the operator. Therefore, a robot-assisted navigation system was developed specifically for interventional operations. This study aims to explore the feasibility of using robotic-assisted percutaneous microwave ablation of the lungs. The study found that the robot-assisted navigation system used in this paper showed satisfactory effectiveness and is expected to standardize percutaneous ablation procedures. This study can serve as a reference for thoracic surgeons when applying ablation techniques in clinical practice.

4. Dr. Zhen-Bin Qiu: Quantifying the progression of early-stage lung adenocarcinoma using radiomics trajectories (Guangdong Provincial People's Hospital)

Capture the progression of computed tomography (CT) radiomics features of early lung adenocarcinoma (es-LUAD) under contrastive learning framework. The study found that radiomics trajectories were able to quantify the progression of esLUAD and reveal a more detailed progression process to assist in clinical diagnosis.

5. Dr. Yunyi Bian: ALDH1A1 Inhibition of ferroptosis leads to resistance to KRAS targeted drugs (Zhongshan Hospital Affiliated to Fudan University)

KRAS is one of the most important genes in the RAS family, and its mutations are one of the most common oncogenic factors in a variety of malignancies. In recent years, the emergence of small molecule inhibitors against KRAS G12C, Sotorasib and Adagrasib, has broken the curse of "undruggable" KRAS and brought hope to patients with KRAS mutant tumors. However, there is a significant problem of resistance to these inhibitors. Considering the difficulty of drug initiation of KRAS inhibitors, in-depth exploration of the resistance mechanisms of KRAS G12C inhibitors is essential to maximize their therapeutic potential. Our study reveals the relationship between the resistance mechanism of KRAS G12C-targeted drugs and ferroptosis, providing ALDH1A1 as a target for adjuvant drugs, which can enhance the therapeutic potential of KRAS G12C-targeted drugs.

On the third day of the conference, experts in the field of thoracic surgery in mainland China brought six major research results to display. Experts from the First Affiliated Hospital of Guangzhou Medical University/National Center for Respiratory Medicine contributed 2 of them. The content involves the use of exhaled volatile organic compounds and artificial intelligence technology for lung cancer diagnosis. At the same time, the research content on adjuvant targeted therapy, targeted therapy resistance mechanism and treatment and other fields is also presented, which can be described as a brilliant show! After three days of compact and rich conference agenda, the 2024 AATS Annual Meeting is about to come to a wonderful conclusion.

At the 2024 AATS Annual Meeting, the team of the First Affiliated Hospital of Guangzhou Medical University and the National Center for Respiratory Medicine made a grand appearance with a total of 5 "oral presentations" and 6 "poster presentations". It fully demonstrates the demeanor of Chinese scholars, and indicates that the voice of mainland researchers in the field of thoracic surgery is getting louder and louder. It is worth noting that Professor Li Shuben of the First Affiliated Hospital of Guangzhou Medical University, with his outstanding achievements in minimally invasive tracheobronchial reconstruction and autologous lung lobe transplantation after neo-adjuvant immunotherapy, won the honor of new member of AATS at this conference, and became the "only" elected expert in general thoracic surgery in China this year!
