I once thought about changing careers, why did I re-strengthen my determination to practice medicine later?

Guan Zhonghai was born in 1989 and is a native of Wenzhou, Zhejiang. The character traits of Wenzhou people who are bold and resolute and dare to try new things can be found in Guan Zhonghai. But he also has a special sense of mission as a healer. During the dialogue, Guan Zhonghai mentioned more than once the social responsibility of being a doctor. Whether it is the initial study of medicine and practice, or the transformation of pediatrics, and now joining the doctor to run medicine, it is to better practice this sense of mission.

When he was in middle school, his grandmother died of bile duct cancer, and Guan Zhonghai came up with the idea of studying medicine, so he volunteered to fill in medicine for the college entrance examination. In 2006, Guan Zhonghai was admitted to the School of Medicine of Zhejiang University to study clinical medicine, and spent his entire medical career here.

Firm belief in medicine, but also feel confused at this stage. Guan Zhonghai saw the helplessness of front-line clinicians in the public system, whether it was the doctor-patient relationship or the practice status, it was difficult to achieve his ideal state, what is the way out in the future?

Later, he claimed to have been fortunate enough to witness the state allowing doctors to practice in multiple locations and freely, and to see the rise of doctors' groups. Dr. Zhang Qiang, the founder of China's first doctor group, said that he would "explore a new path for Chinese doctors", which encouraged him.

Guan Zhonghai, who was studying for a doctorate at that time, realized that medical reform requires the active participation of doctors, and if doctors themselves do not have the will to seek a way out, it is in vain, and doctors running medical services may be the most direct and feasible way for doctors to respond to medical reform and seek a way out.

Since then, the ideal of a doctor to start a business and run a medical school has planted seeds in Guan Zhonghai's heart.

From adult surgery to pediatric surgery, and finally putting down the scalpel, why?

In the middle of his Ph.D., the gears of fate began to rotate, and Guan Zhonghai, who originally majored in adult oncology surgery, plunged headlong into the ocean of pediatrics with a stubbornness and without hesitation.

If he adheres to the original direction, it is foreseeable that Guan Zhonghai's future prospects are limitless in terms of work, treatment and future development. Therefore, this decision was not understood and discouraged by almost everyone, including the mentor, the senior brother and even the family members, who found it incredible.

But in Guan Zhonghai's view, the more painful the place, the more there is a need for change. At that time, it was the general consensus in the industry that pediatrics was busy, pediatrics was tired, and there was a shortage of doctors in pediatrics, but he felt that it was a kind of calling - to "do what is right but others are not willing to do", because in Guan Zhonghai's eyes, doctors are a sacred and glorious profession.

He talked about Dr. Schweitzer, who studied at Zhejiang University School of Medicine: "Schweitzer, who received three doctorates in theology, philosophy and music, started studying medicine at the age of 30 and joined the jungle medicine after obtaining a medical doctorate at the age of 38. ”

In this way, after graduating from the doctorate, Guan Zhonghai went directly to the Children's Hospital of Zhejiang University to engage in pediatric surgery and pediatric oncology surgery for six years.

In the Children's Hospital of Zhejiang University, Guan Zhonghai thinks while working, looking for opportunities for innovation and entrepreneurship in this subdivided specialty field. What makes him confused and anxious is that clinical work has slowly turned into repetitive work day after day, scientific research work has slowly become a response to the index requirements of publishing articles and applying for topics, and the relationship between clinical and scientific research seems to be fragmented and conflicting.

Just when he was struggling internally, a friend told him that a urologist in Shanghai put down his scalpel and founded the Ding Niaoji Doctor Group, focusing on the non-surgical treatment of pediatric phimosis and occult penis. So, after the epidemic, Guan Zhonghai was introduced by a friend, Guan Zhonghai went to Shanghai to visit the clinic of Ding Niaoji and met the founder Dr. Zhu Hua. Most of the children happily climbed into the consultation bed and left the consultation room happily, and the doctor-patient relationship was very harmonious.

"Over" medical treatment and "too little" medical care (too conservative) can be two sides of the same coin, such as the problem of pediatric foreskin, and it is too much to recommend surgery without first trying low-risk, low-harm non-surgical treatment, and conversely, too little medical treatment or too conservative treatment at all may lead to having to face surgery in the future. Ding Niaoji will formulate a personalized non-surgical intervention or treatment plan according to the disease and psychological characteristics of different children, and combine non-surgical treatment with the management of home care habits, collectively referred to as "non-surgical management". The concrete implementation of these concepts of "moderate medical care" also refreshed him.

In his spare time, Dr. Guan will also actively participate in the public welfare of pediatric oncology, and based on the same concept of "moderate medical treatment", he provides long-term companionship personalized science popularization and consulting services for the families of some pediatric cancer patients, so as to help them avoid detours and avoid excessive or under-medical treatment as much as possible. "I believe that the concept of moderate medical care and the spirit of technological innovation will inspire more reforms and innovations in other medical specialties," said Guan Zhonghai.

Is it a pity to give up the opportunity to promote the deputy senior title of a public tertiary hospital?

The reason why he chose to join the social medical service before being promoted to deputy high school was not because he did not have the conditions for promotion, but on the contrary. He is not only a clinical postdoctoral fellow in pediatric surgery at the Children's Hospital of Zhejiang University, but also has presided over projects such as the China Postdoctoral Science Foundation and the Natural Science Foundation of Zhejiang Province, and has published more than 20 SCI papers. "There are all the topics in the article, and it is a doctoral degree, as long as you pass the exam, it should be logical later. ”

On the eve of applying for the deputy high school examination, many people persuaded Guan Zhonghai to complete the promotion of the deputy high school first and then resign. "With my understanding of myself, I chose to be promoted, but it may lead to my reluctance to leave, so I must leave the system before I am promoted. So, after two months of careful consideration, although many people didn't understand, Guan Zhonghai resolutely changed the track.

Guan Zhonghai believes that for the treatment of diseases and saving people, the title is not important, the key is the doctor's medical service itself. In Western countries, attending is already the highest professional title. "At present, most of the doctor groups in China, including the Ding Niaoji Doctor Group, also adopt the attending doctor title system." Doctors should do scientific research to improve the clinical level, not for promotion, and the evaluation of professional titles should not be based on scientific research as heroes, which may create some useless scientific research for promotion, which may have little value for the improvement of clinical technology, and may even lead to academic fraud.

"Probably because I don't approve of this kind of professional title evaluation system, so I chose to vote with my feet," Guan Zhonghai said, "This is also a way and attitude to respond to medical reform." In addition, I also want to set an example for young doctors to show up with their own examples when forming a team of doctors in the future, and I want to tell young doctors that they don't have to be promoted to senior titles."

Looking at the medical community: Doctor Group has been developing in China for nearly 10 years, what kind of changes do you think Doctor Group will bring to Chinese doctors?

Guan Zhonghai: I think the doctor group has really created a new path for doctors in China and given doctors more opportunities to choose. There will be two sides to everything, depending on the individual's choices and ideals. If your dream is to grow up in the public sector, that's fine, but if you feel like you have a better chance of doing your best if you leave the system, joining a group of doctors might be a better option.

Looking at the medical community: Do you have any specific plans for your future career development?

Guan Zhonghai: I hope to continue to improve in the field of pediatric foreskin in the future, including but not limited to theoretical and technical aspects, and I also hope that there will be opportunities to further improve or innovate. Because I am an academic doctorate, I also hope to apply some scientific research methods to the diseases of pediatric phimosis and occult penis, make more summaries and report to my peers, and make progress together. Ding Niaoji already has a good reputation among patients, but if we want more peers to know that there are non-surgical methods, so that more patients can avoid over-surgery, then we need to summarize our experience more and tell our peers through academic means.

Looking at the medical community: What do you think is the biggest difference between your current practice status and public hospitals?

Guan Zhonghai: I am more free to work, and my heart is much freer. Whether it is the daily consultation volume or the time of the visit, it can be arranged independently, and it is a form of appointment system, which is not only more calm, but also more valuable is to have more sufficient time for doctor-patient communication. In addition, there are no night shifts now, and there is little overtime, so it is much easier and I can balance life and work.

Seeing the medical profession: Is it difficult for you to put down the scalpel and start learning a new technique for non-surgical treatment?

Guan Zhonghai: As a surgeon, I have also done a lot of surgeries, and I am still relatively confident in terms of hands-on ability, and the surgical techniques, anatomical knowledge, and sterility concepts I have learned before will still be of some help to learn new technologies.

The main reason is that the non-surgical treatment of pediatric foreskin has strong individual differences, and different children should combine many factors to develop personalized treatment strategies, once the strategy is not appropriate, the treatment effect and experience will be greatly reduced, and there will be differences in the details of the technique in different situations, not to mention that many strategies and techniques are in the details, which are beyond imagination and fine details, so it is not simple.

I am Dr. Zhu Hua personally taught, Dr. Zhu Hua is very strict with the training of new doctors, must follow the observation of the treatment process after thousands of people before you can start from the simplest operation, practice accumulated 300 to 500 people and passed the theoretical assessment before obtaining the brand doctor authorization can be independent consultation, so from the observation and learning to the practice to finally obtain the brand authorization to open the clinic independently, I probably spent nearly half a year before and after, which is much more difficult than I imagined.

Looking at the medical community: As the medical director of Ding Niaoji Doctor Group and the head of Hangzhou, can you introduce the planning of Ding Niaoji in terms of medical services and academics, as well as the development and future plans of Ding Niaoji in Hangzhou?

Guan Zhonghai: In terms of medical services, in the future, we will continue to optimize details, improve processes, do a good job in doctor training and quality control, and further improve the treatment experience. At present, Ding Niaoji has accumulated thousands of successful cases of non-surgical treatment of pediatric phimosis and occult penis, including many cases of very severe cases of treatment of occult penis and pathological phimosis, which were once recognized by the medical community as necessary for surgery, and we will continue to accumulate more, and hope to report this major breakthrough to the medical community at home and abroad in an academic way in the future.
