Status asthmaticus (stubborn phlegm, airway reluctance, asthma, heart and yang deficiency)

Huang, female, 2 years and 2 months, was admitted to the hospital on March 12, 1992 mainly due to "recurrent chest asthma for 1 year and recurrence for 1 month". The child has been suffering from asthma for more than 1 year, and has been transferred to many hospitals, treated with Western medicine to fight infection, intravenous injection of hydrocortisone anti-inflammatory, combined with α-chymotrypsin, bamboo drain ultrasonic nebulization inhalation to reduce phlegm, etc., and traditional Chinese medicine syndrome differentiation treatment, the condition is mild and sometimes severe, and asthma attacks are recurring.

Due to continuous hospitalization, intravenous fluid administration, blood vessel damage to the scalp, hands and feet, and difficulty in infusion, conventional oral treatment of Chinese and Western medicine was temporarily given after admission.

On March 14, the child's chest asthma intensified, phlegm in the throat, sound like a saw, mouth and shoulders raised, cough against the upper breath, atrophy and irritability, pale complexion, blue around the lips, pale purple tongue, thick and greasy moss, purple fingerprints, and qi stagnation.

Physical examination: pulse 150 beats/min, respiration 41 breaths/min, both lungs are covered with coarse and medium wetness, wheezing, low heart sounds, 2.5 cm below the sword of the liver, 5 cm below the ribs, and medium.

Routine blood test: white blood cells 27.1×109/L, neutrophils 43%, lymphocytes 57%;

Chest x-ray: increased light transmission in both lungs, widening and flattening of the intercostal space, and blurred lung markings.

Western medicine diagnosis: bronchial status asthmaticus.

TCM diagnosis: asthma attack (stubborn phlegm, airway reluctance, asthma, heart and yang deficiency).

治法:涌吐豁痰,开闭平喘。

Prescription: Rescue and salivation.

15g of pig tooth soap, 6g of alum. 1 dose of rapid decoction. Soak the pig tooth soaphorn in cold water for half an hour, fry it over a fire, then change to a simmer and cook for 15 minutes, take 100mL of juice, melt the alum into it, and take 30mL warm. 45 minutes after administration, the child vomited sputum and salivated with milk food twice, about 300mL, the phlegm in the throat decreased suddenly, and the child fell asleep quieter at night than before. The next morning, another 30mL was taken, and about 200mL of spitting saliva was poured again.

Follow-up on March 15: the phlegm in the throat disappeared, the breathing tended to be stable, the bruising around the lips was significantly relieved, the spirit improved, the hunger and food, the tongue was light, the moss was white and yellow, the fingerprints were light purple, and the wind was off.

Physical examination: 26 breaths/min, wheezing disappeared in both lungs, only dry and a little wet sound were heard, pulse was 110 beats/min, heart sounds became clear and powerful, liver retracted, 1 cm below the xiphoid process, 2 cm below the ribs, soft;

Follow-up blood test: white blood cells 6.5×109/L, neutrophils 55%, lymphocytes 40%. Then stop taking the saliva and disperse, give 6g of red ginseng decoction, a small amount of frequent consumption, to strengthen the righteous qi, and add flavor to the Liujunzi soup to nourish qi and strengthen the spleen, warm phlegm drink.

Prescription: Nansha Ginseng 15g, Roasted Atractylodes Macrocephalus 6g, Poria Cocos 6g, Citrus Citrus Peel 6g, Banxia 6g, Cinnamon Branch 5g, Shenqu 5g, Glycyrrhiza Glabra 3g. After taking 6 consecutive doses, he did not breathe, his breathing and pulse were normal, he was hospitalized for 21 days, and was clinically cured and discharged.

Note: In this case, the child's asthmatic status is not only the cause of the closure of the airway and the cause of asthma, but also the source of qi stagnation and blood stasis, and the decay of heart and yang deficiency. In the application of the vomiting method, attention should be paid to grasping the urgency of the specimen, and the symptoms should be treated if it is urgent, and the root cause should be treated if it is slow.

This method is applicable to children who are prosperous and evil, or who are evil but not yet too weak. Note that the strong agent can only be used temporarily, not for a long time, and the illness will be stopped, so as not to hurt the body. After vomiting, the spleen and stomach should be regulated, the spleen and dampness should be transported, and the source of phlegm should be eliminated.

Sweating, spitting, lowering, warming, clearing, dissipating, and replenishing are the eight traditional Chinese medicine treatment methods.
