What is the Immigration Medical Surcharge?

The Immigration Health Surcharge (IHS) is a medical service fee levied by the National Health Service (NHS) on non-British citizens who have lived in the UK for more than 6 months, so that anyone who legally enters the UK and holds a long-term visa can enjoy the NHS of the UK like a local resident, and promotes a fair and sustainable healthcare system.

IHS fees are payable at the time of submission of a UK visa application and vary depending on the length of stay and the applicant's status. After paying the IHS fee, visa applicants can enjoy free medical services such as general practitioner consultation, referral to a hospital for treatment by a general practitioner, hospitalization, emergency services at the hospital, and ambulance services.

However, it is important to note that the IHS does not cover the cost of prescription drugs (prescription drugs are only paid in England, and prescription drugs are free in the other three regions). In addition, emergency prescription drugs are free of charge throughout the UK), dental clinic services and opticians, which are available at an additional cost.

When the Immigration Medical Surcharge was first introduced in March 2015, immigration applicants were only required to pay £200 per year. In December 2018, the fee doubled to £400 and in December 2020 it rose again to £624 per adult applicant per year and £470 for children and students. On 6 February 2024, the fee rises to £1,035 per person per year for adults and £776 per person per year for international students and children.
