Why is the time from diagnosis to death so short? Is colon cancer so dangerous? The reporter interviewed Chen Min, attending physician of the Department of Anorectal Surgery at Run Run Shaw Hospital affiliated to Zhejiang University School of Medicine.

Patients with advanced colorectal cancer

Try to get the chance of surgery to achieve tumor-free survival

Chen Min introduced that among all gastrointestinal malignant tumors, colorectal cancer is relatively one of the malignant tumors with positive treatment and better prognosis in terms of incidence rate and treatment effect.

The reason why the blogger's condition progresses quickly is related to the type of cancer pathology he diagnosed and the stage of the disease at the time of diagnosis. If the cancer is advanced at the time of discovery, the treatment effect is relatively poor. Chen Min said that about 20%-25% of all patients with newly diagnosed colorectal cancer will have distant metastases, but this does not mean that there is no hope for treatment.

For advanced colorectal cancer, Chen Min said that the clinic is mainly divided into two categories: resectable and unresectable, and targeted treatment is carried out.

"According to international authoritative data, the 5-year survival rate of advanced colorectal cancer patients without surgery is only 10%; If patients with advanced colorectal cancer that can be surgically removed, the 5-year survival rate can reach more than 40%. Chen Min introduced that at present, through the multidisciplinary collaborative diagnosis and treatment model, more patients with advanced colorectal cancer can get surgical opportunities, achieve tumor-free survival, and then prolong survival time.

Song Zhangfa, chief physician of the Department of Anorectal Surgery, Run Run Shaw Hospital affiliated to Zhejiang University School of Medicine, has received many patients with advanced colorectal cancer at first diagnosis.

One of the female patients, 40 years old, had a medical examination seven years ago for blood in the stool. CT reports suggest that the female patient has multiple suspected tumor metastases on the liver. After further examination, he was diagnosed with rectal cancer with liver metastases.

In terms of the patient's condition at that time, direct surgical treatment was not possible, so the anorectal surgery team perfected genetic testing and formulated a treatment plan for the female patient: through chemotherapy combined with targeted drug conversion therapy, hoping to transform the unresectable tumor into a resectable state.

After eight treatments, the anorectal surgery team won the opportunity for the female patient to operate, and successfully operated on bowel cancer combined with liver metastases. After surgery, the female patient underwent relevant adjuvant chemotherapy and had a good prognosis.

During the follow-up process, the female patient had two single metastases, lung tumors and liver tumors, and through local treatment methods such as surgery and radiofrequency ablation, she spent 5 years of survival, and is now in a tumor-free state of survival, and the quality of life is no different from ordinary people.

How can young people prevent and treat colorectal cancer early?

Chen Min said that for young patients, colorectal cancer is mainly divided into two types: "genetic" and "sporadic".

Patients with hereditary colorectal cancer have a clear family history. If there are 2-3 or more bowel cancer patients in the family such as parents or grandparents, it is recommended that colonoscopy screening can be done after the age of 18 to diagnose hereditary related diseases early.

Sporadic young colorectal cancer patients are mainly related to bad living habits, such as staying up late, working pressure, smoking and drinking, high-calorie, high-fat eating habits, and insufficient dietary fiber intake, all of which induce the younger incidence of colorectal cancer. In addition, young people are less vigilant about the symptoms of physical discomfort and often come to the doctor only when severe symptoms appear, which is also the reason for the high proportion of patients with intermediate and advanced bowel cancer found in young patients.

"It is recommended that young people quit smoking and alcohol, exercise more, and control their weight. Develop the habit of regular physical examination, colorectal cancer is gradually evolved from benign polyposadenoma, active physical examination can be detected early, early treatment, so as to prevent the occurrence of colorectal cancer. Chen Min reminded.
