【Definition】Urinary incontinence is a disease that cannot be controlled by urine in the awake state and flows out on its own, which belongs to the category of "urine incontinence" in Chinese medicine.
【Etiology and pathogenesis】This disease is mostly caused by strain, worry, fatigue, post-illness qi deficiency, elderly kidney deficiency, etc., so that the lower yuan is not solid, and the bladder is out of contract. Others such as humid heat or bruising in the bladder can also cause urinary incontinence.
【Clinical Classification】
- Kidney qi is not solid type urine incontinence, urine is long, fatigue and cold, waist and knees are sore, and both feet are weak. The tongue is light, the moss is thin, and the pulse is weak and weak.
- Spleen and lung qi deficiency type frequent urgency, sometimes urine self-loss, even in coughing, talking and laughing can also appear urinary incontinence, lower abdomen sometimes swelling, white face shortness of breath. The tongue is pale and the pulse is weak and weak.
- Moist heat betting type frequent urination, burning urination, sometimes urine self-loss, drowning and smelly. The tongue is reddish, moss yellow and greasy, and the veins are fine and smooth.
- Lower scorched stasis type urine is not inhibited, the lower abdomen is full of dull pain, or a palpable lump. The tongue is dark or purple spots, thin moss, and astringent pulses.
【Key points of diagnosis】
1. When urine cannot be controlled and flows out on its own in the awake state, or when it is coughing, sneezing, walking, standing upright, exerting force, irritability, excitement, laughing, shouting, suddenly frightened or hearing a dripping sound, urine flows out on its own.
2. Routine urine examination is generally normal. Cystourethrography can determine the presence and location and extent of obstruction.
【Treatment】
1. Jin three-needle therapy
Main acupoint: three needles in urine.
Matching points: kidney qi is not solid, add a life gate, to supplement the kidney solidity; Spleen and lung qi deficiency, add lung Yu, spleen Yu, and Zusanli to replenish the lungs and spleen; Wet bet, add Yin Ling Spring, between the rows, to clear the humidity and heat; Lower scorching stasis, add secondary hip, tai chong, with live blood stagnation.
2. Other therapies
(1) Ear acupuncture: take the bladder, urethra, kidney. Milli needle prick, or press with Wang Bu Liu line.
(2) Electroacupuncture: take Qi Hai, Guan Yuan, Zhongji, Zusanli, and Sanyin intercourse, and when the three acupunctures of the abdomen are pricked, the needle sensation is required to radiate to the anterior genitals, and the electroacupuncture is stimulated with dense or intermittent waves for 30 minutes. 1~2 times a day.
