Public place sanitation permits apply to:
The eight categories of venues are as follows:
(1) Accommodation places: pointing to places that provide consumers with accommodation and related comprehensive services. Including beauty shops, beauty centers, beauty salons, beauty clubs, beauty salons, beauty spas. It does not include nail salons, medical beauty, massage slimming, and health care massage places. Application materials
The following materials shall be submitted when applying for a public place health permit:
1. Health permit application form;
2. Proof of identity of the legal representative or person in charge;
3. Schematic diagram of address orientation of public places, floor plan and layout plan of sanitary facilities;
4. Public place health testing or evaluation report;
5. Sanitation management system in public places;
6. Other materials required by the health administrative department, such as a letter of attorney or unit certificate of a legal person; 2 recent color photos of the legal representative or person in charge; pre-approval materials for the name of the enterprise of the industry and commerce department; a schematic diagram of the provision of water supply for secondary water supply facilities; a lease agreement for business premises; a roster of employees; preventive health audit materials for new construction, reconstruction and expansion projects.
