Health monitoring, intelligent alarm, and responsiveness, the elderly enjoy a full range of care similar to that of real nursing homes at home

Home "virtual nursing home" pilot, are you willing to pay the bill?

Reporter Huang Yongdi

Early spring is the season when basic diseases such as asthma occur frequently in the elderly. A few days ago, Zhang Lianfa, an 81-year-old man who lives in AiboSi Village in Minhang District, felt breathless and quickly pressed the help phone on the bracelet. A few minutes later, the paired community housekeeper Xiao Gu and the nurse of the Xinhong Street Comprehensive Service Center came to the door together to assist in oxygen inhalation operations, medication, etc., so that the old man quickly eased up.

This is not an isolated case. In the Xinhong Street Comprehensive Service Center for the Elderly, on a large information screen, real-time "links" to the safety and health status of hundreds of elderly people in the neighborhood committees distributed in the streets. Whether the elderly are bedridden, how well they sleep, how long they have been away from the bed or away from home, whether the heart rate and blood pressure are within the normal range, are all monitored for 24 hours; the elderly will respond immediately if there is an emergency at home or there is a need for help, and if necessary, they will quickly come to the door.

"Install sensing equipment and smart products at home, realize the linkage with the old-age service station at the doorstep, so that the elderly can enjoy all-round care similar to the real nursing home at home" - this is the home "virtual nursing home" that Minhang District is piloting in Xinhong Street, which currently covers more than 200 empty nesters, elderly living alone, disabled and dementia and other elderly people in the street, and it is expected that the pilot scope will be expanded in Xinzhuang Town, Qibao Town, Hongqiao Town and other streets and towns this year.

In the city's "9073" pension service pattern, the vast majority of the elderly choose to stay at home for the elderly. Such a home -- "virtual nursing home" can not only meet the wish of "not leaving home for the elderly", but also improve the quality of life of the elderly and reduce the pressure of institutional pension. However, the reporter found that although the people have applauded this new model, it is still difficult to further promote the application and make people willing to "pay the bill".

Children are not around, or live alone, getting older, more and more physically inadequate... Even if the elderly are usually relatively healthy, there are certain safety risks in home care.

"Once, when I was taking medicine, the capsule stuck in the mouth of my throat, fell on the floor and fainted, and when I woke up, I found that I had bleeding on my head." The 89-year-old Grandpa Su talked about the plight of living alone at home, and bluntly said that he was really a little afraid.

Therefore, soon after the launch of the home "virtual nursing home" in Xinhong Street, Grandpa Su took the initiative to equip digital devices such as smart watches and smart monitor mattresses. Now, after more than a year of use, Grandpa Su has a sense of security: "In the past, when encountering urgent things, there was nothing he could do alone; now when he encounters an urgent matter, he can directly call the community housekeeper as soon as he presses the watch, and they will come to the door immediately, which is very reliable." ”

What are the functions of the smart monitoring mattress? In the home of Zhang Lianfa's old uncle, his wife Aunt Chen and the community housekeeper Xiao Gu lifted the sheets together and showed the reporter a long strip of smart device, mainly placed in the heart position of the elderly after lying down, automatically monitoring the heart rate and leaving the bed when the elderly slept. Uncle Zhang's heart is not good, this device is very necessary, once the monitoring data appears abnormal, such as getting up at night after the delay in returning, there is a risk of falling, fainting and other risks, the monitoring platform will issue an alarm, the background housekeeper will contact the elderly or the elderly family in time.

According to reports, Uncle Zhang's mattress is a basic product, and the market price is two or three hundred yuan. There are also more functional smart mattresses, such as electronic control three-stage lifting, as well as sleep monitoring, one-key call and other intelligent bed control, the price ranges from hundreds, thousands, tens of thousands of yuan.

"All the smart devices we have chosen are our thousands of choices, and they must be more suitable for the elderly and cost-effective while having the required functions." Yang Hua, manager of the IT department of Shanghai Hengyan for the old service agency, the third-party operating unit of the Xinhong Street Smart Pension Service Project, told reporters that taking the intelligent monitoring mattress as an example, the mattress that was finally presented in the model room was screened out from more than 300 kinds of monitoring mattresses on the market, with the functions of automatically monitoring the heart rate of the elderly, leaving the bed, and generating sleep reports every day.

The smart device installed by Uncle Zhang's family is the "three-piece set" that most elderly people currently choose, including door magnets, smart bracelets, and smart mattresses. Door magnetic installation at the upper end of the entrance door, usually will monitor the elderly's entry and exit habits, if the elderly all day or more without going out, it will attract background attention.

When the reporter interviewed the xinhong street comprehensive service center, he suddenly saw a red "fall alarm reminder" automatically jumping out of the big screen, which showed the name of the old man, the alarm time, the alarm equipment, etc. The backstage service staff immediately called the old man, and after understanding the situation, they found that it was a false alarm. It turned out that the old man was cooking in the kitchen, because the wrist movement was too fast, it was judged by the smart bracelet to be "acceleration abnormal" through attitude monitoring, so it issued an alarm reminder for falling.

There are also elderly people who have installed "four-piece sets" and added gas alarm equipment. Some elderly people often forget to turn off the fire when cooking, and when they reach a critical point, the smart device will issue an alarm, which not only reminds the elderly, but also notifies the background service staff. If there is a gas leak, it will be detected in time.

"With the aging of the population, especially after the aging of the parents of the first generation of only children, the demand for multi-level and diversified old-age services is more urgent. Digital transformation has brought new opportunities to improve the energy level of home care services. Wu Xuenian, head of the pension service section of the Minhang District Civil Affairs Bureau, told reporters that the home "virtual nursing home" piloted in Minhang mainly establishes a smart old-age service platform by using government guidance and third-party organization operation, so that the family pension bed has the function of "quasi-institution".

According to statistics, there are 12,158 elderly people over the age of 60 in Xinhong Street, and 208 smart monitoring mattresses and 323 smart watches have been installed on a pilot basis in empty nests, elderly living alone, incapacitated and dementia, etc., providing 24-hour services such as remote medication reminders, blood pressure heart rate measurement, positioning, monitoring of the elderly's sleep quality and health data, and early warning of not returning from bed. So far, Xinhong Street has achieved 106 "virtual pension beds" at home and more than 14,000 door-to-door service work orders.

The reporter saw that in more than 14,000 service work orders, the alarm that did not return from the bed accounted for the first place; followed by the blood pressure alarm, accounting for more than 40%; the next one was the bracelet SOS alarm, the door magnetic detection alarm, the fence alarm, the fall alarm, etc., all accounting for single-digit percentage points.

"Home smart old-age care is the future social development trend. In the future, whether it is the elderly living alone at an advanced age, the elderly with disability and dementia, or a large number of ordinary healthy elderly people, they actually hope to have higher quality and more diversified intelligent old-age services. Therefore, we hope to show some good intelligent application scenarios so that more and more community residents can understand and accept these service projects. Gu Binhuan, director of the Xinhong Street Community Service Office, said.

Nowadays, in the xinhong street comprehensive service center for the elderly, to create a "model room" that can allow the elderly to visit and experience, the scene restored a complete set of elderly home smart environment, including living room, bedroom, kitchen, entrance, bathroom and other smart scenes, the elderly through the intelligent voice control system, as long as they master the basic password, you can easily control the equipment.

"Open the curtains", "Turn off the TV", "Turn off the lights"... Each time, the elderly who came to the "model room" to experience the visit were very excited, and with a password, the wisdom application of "named" in the room began to work.

It is understood that due to the dual control of price and quality, the price of the home "virtual nursing home" basic package piloted in Xinhong Street in Minhang District is not expensive, and the elderly can enjoy projects including wisdom protection, housekeeping services, health care, and spiritual comfort when they pay 598 yuan per month. Of course, if the elderly need other personalized services, such as cleaning services, bathing services, meal services, agency services, escort services, etc., they can also be freely combined after drawing out on the list, and will enjoy a more preferential service price than the market price.

The home "virtual nursing home" piloted by Xinhong Street combines the three ways of home care, community pension and institutional pension, and strives to explore the "six-up integration" of care, medical care, health care, cultural care, physical care and wisdom pension, hoping to realize the original residence of the elderly and the elderly.

For example, Zhang Lianfa Laobo, in addition to enjoying smart old-age care services at home, also goes to the day care center next to the community every day to chat and rest, and often follows the nursing staff to do handicrafts, and eats lunch there at noon. "As long as 8 yuan, today's dishes include tsitstrike roast meat, chicken feet, yam fungus, a big meat, a small meat, a vegetarian soup and a meal, I and my wife eat together to eat it all; at 3 o'clock in the afternoon, I also have to eat a snack, sometimes it is rotten pasta, sometimes it is vegetable porridge or eight treasure porridge." Zhang Laobo said that such a daycare service only needs to pay 300 yuan per month.

After Laobo Zhang's asthma attack a few days ago, the community housekeeper not only came to the door to do emergency treatment for Laobo Zhang, but also helped to make an appointment for the next day's outpatient clinic of the community health service center.

"It can be seen that home medical care, community embedded pension and institutional pension are not separated." Wu Xuenian introduced that in Xinhong Street, third-party institutions have also undertaken the operation tasks of home care, community pension and institutional pension, and can use the smart pension service platform to achieve integrated management of service resources, while the elderly can customize the pension model according to their own needs.

"In this way, we can send community housekeepers and nursing staff nearby, 5 minutes to 10 minutes to reach the elderly home, the elderly need cleaning, bathing, food delivery and other services, but also in time to integrate social resources." Yu Xinyong, deputy general manager of Shanghai Hengyan for the elderly service institutions, introduced that if the physical condition of the elderly allows them to stay at home, then they can choose home medical care; if the elderly need a rich cultural and recreational life, they can come to participate in the activities of the comprehensive service center for the elderly and the neighborhood center; when the elderly have physical problems, need postoperative rehabilitation, long-term care, or children need respite services, Hengyan can also help refer them to hospitals or pension institutions.

Grandma Du, 87, and her 89-year-old wife are at home together. In their lives, they perfectly combine the two modes of home care and community pension.

The home of the two elderly people is equipped with smart home care systems such as remote monitoring, smart mattress, emergency call, fall alarm, water, electricity and coal alarm. In their spare time, they will go to the street comprehensive service center for the elderly, in the multi-functional fitness place for sports and wellness; in the handicraft workshop, make scarves, backpacks, LEGO handicrafts; in the family doctor's pension consultant studio, consult on health issues; in the community "wisdom canteen", enjoy meals suitable for the tastes of the elderly...

Grandma Du said: "We are very happy now, my son and daughter often call to ask, I said that after installing smart devices at home, it is safe, and the staff with problems will come to the door in time, and also come to deliver fruits and clean up, you are busy with your own work, even if we are not around, we are very at ease." ”

Why is it difficult to promote?

■ Smart pension is not a problem in technology, it is difficult to integrate technology and background services

■ Many elderly people are more frugal, and their consumption concepts still need to be cultivated and guided

At present, Minhang District has promoted home "virtual nursing homes" in various streets and towns. When the staff distributed leaflets and conducted demand research in each neighborhood, the elderly in the community expressed "need" and showed great interest in various scenarios, but when asked whether they would buy services, everyone shook their heads.

The reporter learned that at present, Minhang District has equipped 300 families of the elderly who meet the conditions or have needs with smart devices, and equipped 3900 elderly people with smart watches. But most of them are paid or subsidized by the government, and only a very small number of families have actively procured services.

"The consumption concept of the elderly also needs to be cultivated and guided, and there is a process." Wu Xuenian believes that many elderly people are more frugal, will not consider too much when the physical condition is good, and they do not know too much about smart products, and even have fear, so they need to carry out pilots, demonstrations, do it for everyone to see, and then gradually push to marketization, "with the passage of time, the upgrading of consumption concepts and the cultivation of a new generation of elderly people, smart old-age care will be the trend of the times."

For industry insiders, smart pension is no longer a problem in technology, and it is difficult to integrate technology and back-end services.

Yu Xinyong frankly said that now, there are a variety of smart products on the market, however, there are not many cost-effective products, and the degree of suitable aging is not enough, "not smart is useless, too smart will not be used", which requires repeated comparison experiments in the application to find the most economical and most suitable integration solution for the elderly.

Back-office services are even more important. Otherwise, the elderly have installed smart devices in the light, but no one provides 24-hour response and door-to-door service, etc., and "virtual nursing home" has become an empty phrase. Therefore, in addition to building a big data platform, third-party companies must also build an emergency response circle of 5 minutes to 10 minutes in the region, and the construction of the service network and the layout of personnel is a problem. If the customer base is not large enough, operating costs will remain high.

In Zhang Lianfa's old uncle's home, the reporter saw that the community housekeeper Gu Haiyan has been chatting with the two old people non-stop, the whole process of laughter is continuous, the old man repeatedly thanked her for recommending a kind of rice noodles delicious, she told the old man to "go out must wear a mask, go home and remember to wash your hands." Such service "stickiness" is the core selling point of "virtual nursing home" and the most scarce.

In addition, because the current market space for smart pension is still very small, and the iteration of intelligent products is too fast, the enthusiasm of enterprise investment is not high, which leads to the product development and integration research of smart pension will be relatively lagging behind, which in turn will affect its promotion and application.
