A 50-year-old man swallowed jujube kernels by mistake, ate ginger and drank vinegar on his own, and almost caused perforation of the esophagus

A team of experts endoscopically removes the jujube kernels for the patient.

Red Net Moment, February 9 (Correspondent Li Juan) During the Spring Festival, there are all kinds of hard dishes on the New Year's table, and when everyone enjoys the warmth of reunion and the pleasure of eating, do not forget the risk of foreign body incarceration.

During the Spring Festival holiday, the Gastroenterology Department of Hunan Zhizhong Hospital received several cases of foreign bodies from the digestive tract. Among them, 50-year-old Uncle Wang, who swallowed the jujube kernel by mistake, actually endured for three days, and tried various local methods such as drinking vinegar and eating ginger until he could not drink water and his chest pain was unbearable.

Emergency CT examination found that Wang Dabo's esophagus had a long fusiform foreign body in the middle of his esophagus, both ends of the foreign body had penetrated the esophageal wall, the upper esophagus wall was obviously hyperemic and edematous, and a large amount of food could be seen in the lumen of the tube. Liu Xinwen, director of the Department of Gastroenterology, believes that Wang Dabo's esophageal foreign body has penetrated the wall of the esophagus, close to the common carotid artery, and if it is not treated in time, it may lead to esophageal perforation, mediastinal abscess, and even cause the risk of rupture of the common carotid artery and bleeding.

After Liu Xinwen urgently contacted the joint consultation of cardiothoracic surgery, radiology, entolaryngology and anesthesiology experts, he agreed that transgasoscopic esophageal foreign body removal under painless anesthesia was the most appropriate treatment at present.

Wang Dabo was sent to the digestive endoscopy center through the green channel, and the expert team worked together to successfully remove the jujube kernel under endoscopy.

"Once in the esophagus, a large number of foreign bodies blocked the upper part of the esophagus, the field of vision can not see the foreign body, all are food residues, we use foreign body forceps to carefully exhaust the food more than a dozen times, and finally saw the jujube kernel of Kunton, during which the anesthesiologist was also fully engrossed, afraid that the depth of anesthesia was not enough, and the patient coughed and caused the food to fall out of the trachea."

Liu Xinwen said that the jujube nucleus is the most dangerous type of foreign body in the digestive tract, with sharp ends and easy to pierce the esophagus. After taking the food, it was found that the jujube nucleus was stuck 18 cm from the front teeth of the esophagus, and the tips at both ends were pierced into the wall of the esophagus, forming a deep ulcer locally.

"Both ends of the jujube nucleus are embedded in the esophageal wall, consistent with the preoperative CT performance, we used foreign body forceps to try repeatedly, and finally loosened one side of the tip, and then clamped this side tip with foreign body forceps, carefully lifted upwards, so that the other side tip was smoothly detached from the esophageal wall, the whole process was completed in one go, the date nucleus was smoothly clipped out and dragged into the gastroscope transparent cap, and brought out of the mouth together." Wu Wanfeng, deputy director of the Department of Gastroenterology, as an operating physician, said with great satisfaction: "The operation process was very smooth, and two obvious longitudinal ulcers at the jujube nucleus could be seen after the operation, although the possibility of penetration was not ruled out, but most of this penetration can be completely cured after internal medicine treatment." The success of the operation avoids the risk of further surgical procedures, reduces the suffering of the patient, and also reduces the cost of medical treatment."

"The patient took the wrong method after the occurrence of the esophageal foreign body, and did not see the doctor at the first time, resulting in jujube nuclei into the wall of the esophagus, and too much food in the esophagus, all of which brought different degrees of difficulty and risk to endoscopic surgery." In fact, trying to swallow a foreign body by eating is not advisable, and even very dangerous! Liu Xinwen pointed out.

Gastroenterologists remind that esophageal foreign bodies can occur at any age, mostly in the elderly and young children. Most patients have a esophageal foreign body, that is, there is pharyngeal pain, exacerbation during swallowing, foreign body sensation, dysphagia, increased saliva, choking sensation in swallowing saliva, and larger esophageal foreign bodies can compress the trachea and cause dyspnea.

What should I do if i have a foreign body of the esophagus? Stuffing steamed buns, swallowing bread, choking rice, drinking vinegar and other folk remedies are all not allowed, and all swallowing, eating, and even water should not be drunk. Please do not delay, the sooner you go to the hospital, the safer it is, because the longer it is, the more likely it is to progress to serious complications. A professional doctor will develop an appropriate treatment plan through examination.
