As a result of the accident, four relevant personnel, including the mine director, were recommended to be transferred to the judicial organs, and the executive director of Dafengshu Coal Mine was recommended to impose a fine of 19,000 yuan.
The direct cause of the "8•8" flood inverted well accident was that the exceptionally heavy rainstorm flash flood that exceeded the historical extremes since the local meteorological records were recorded poured into the Dafeng Coal Mine area, and the flood poured into the underground, blocking the escape channel for operators. At the same time, the mine did not strictly implement the instructions of the regulatory authorities to evacuate people, during the rainstorm warning period, 2 pump drivers were arranged to pump water into the well in violation of the law, and the underground personnel were not evacuated in time when the torrential rain and flash flood came, resulting in 2 people being trapped and lost.
The indirect causes of the accident include insufficient pre-judgment of mine risks; illegal command by management personnel; inadequate safety management; illegal operation of operators; inadequate implementation of the post responsibility system; and insufficient supervision of coal mine safety.
It is understood that the accident occurred at about 20:30 on August 7, the early shift (0 to 8 o'clock the next day) pump driver He Tingfang arrived at the coal mine early to prepare for early drainage in order to leave work early. Chen Shicai, a wellhead inspector, said that the dispatch room notified the early shift tile inspectors and the leaders with the shift not to go to work, but it is not clear whether the pump driver is working. He Tingfang called the mine director Luo Shiping and got that "if there is no rain, the pumping workers will have to go to work, but without a positioning card." In reply, He Tingfang then called to inform the same class of pump driver Liang Zhengquan to work in the mine. At 21:28 and 29, He Tingfang and Liang Zhengquan carried the positioning cards into the well one after another. He Tingfang went to the +200m pump room, liang Zhengquan to the +100m pumping room for pumping operations. At 22:02, Liang Zhengquan left the well to put his and He Tingfang's positioning cards in the charging room and immediately returned to the well.
Dafengshu Coal Mine in Miaoba Town, August 8, 3 to 4 o'clock began to rain, 4 o'clock began to increase the rain, at 5:7, the wellhead inspector Chen Shicai reported the news that "the coal mine dam caused a lot of water due to heavy rain and the water quickly entered the house" to the mine manager who was resting at home. At about 5:17, Chen Shicai arrived at the dispatch room (monitoring room) on the second floor of the office building, told the duty dispatcher Xu Jiaxiang about the rising water, Xu Jiaxiang immediately called He Mingyang, the ground duty leader on the day who had left his post and returned home, to report that the rain had entered the wellhead, during Xu Jiaxiang's call, the underground pump driver He Tingfang called the dispatch room to report the power outage, and Chen Shicai, who answered the phone, told He Tingfang that the ground flood was pouring into the well, so that he could escape quickly. According to the dispatch recording, at 5:19, He Tingfang called to inform Liang Zhengquan in the +100m pump room to evacuate immediately, and then the communication was interrupted. A large amount of flood water poured into the well, and He Tingfang and Liang Zhengquan were trapped and lost.
After the accident, Huang Qiang, deputy secretary of the provincial party committee and governor, Luo Wen, member of the standing committee of the provincial party committee and executive vice governor, and Luo Qiang, vice governor of the province, respectively made important instructions; the main leaders of the Emergency Management Department, the Sichuan Coal Regulatory Bureau, and the Dazhou Municipal Party Committee and Municipal Government went to the scene many times to guide the accident rescue work, and the responsible persons in charge of the Emergency Management Department and the Sichuan Coal Regulatory Bureau led the team to the scene to guide the rescue and rescue work.
After investigation by an investigation team composed of multiple departments, the accident was a flood damage accident that flooded the well and belonged to the production safety accident.
Regarding the responsibility determination and handling suggestions for the accident, the investigation team believes that the implementation of the safety production responsibility system in the Dafengshu Coal Mine is not in place, the implementation of accident prevention measures and emergency measures is not in place, and the main responsibility for the occurrence of the accident is mainly responsible. In violation of the provisions of Articles 19 and 41 of the Work Safety Law of the People's Republic of China, in view of the occurrence of the accident and certain natural factors, a fine of 290,000 yuan is recommended in accordance with the provisions of Article 109 (1) of the Work Safety Law of the People's Republic of China.
Luo Shiping, director of Dafengshu Coal Mine, is the first person responsible for the safety production of the mine and is mainly responsible for the accident. It has insufficient understanding of the seriousness and harm of torrential rain flash floods, and relies on experience to prevent floods and fight disasters; the responsibility system for safe production posts is not in place, and the mine leaders on duty during the suspension of production are absent from work at night; illegal command, evasion of supervision, in the case of the relevant departments issuing rainstorm warnings and suspension and evacuation instructions, illegal arrangements for He Tingfang and Liang Zhengquan to enter the well to engage in pumping operations, and instructed the personnel entering the well not to carry personnel positioning cards, resulting in 2 people being trapped and lost. On August 17, 2021, due to the crime of suspected major liability accident, the public security organs took criminal compulsory measures of release on guarantee pending further investigation, and it was recommended that the judicial organs investigate and deal with it; and in accordance with relevant regulations, it was recommended that his safety production knowledge and management ability assessment certificate be revoked.
He Mingyang, deputy chief engineer of Dafengshu Coal Mine, served as the mine leader on the ground duty on the day of the accident. The implementation of the "rainy season three defenses" duty duty system is not in place, the provisions of 24-hour duty in the mine during the flood season are not strictly enforced, the mine is left without authorization, the duty of the leader on duty is not performed, the 2 pump drivers are not found and prevented from entering the well in time, and the corresponding measures are not organized in time to take corresponding measures and withdraw the underground personnel when the rainstorm comes. Violating the provisions of Article 22 (5) of the Work Safety Law of the People's Republic of China shall bear important responsibility for the occurrence of the accident. In accordance with relevant provisions, it is recommended to transfer it to the judicial organs for investigation and handling in accordance with law, and it is recommended that their safety production knowledge and management ability assessment certificate be revoked.
Xu Jiaxiang, a duty officer in the dispatching room of the Dafengshu Coal Mine, was on duty in the dispatching room on the day of the accident. In the case of knowing that someone may enter the well for pumping operations, when the regulatory authorities inquire by telephone, only the personnel positioning system displays information to reply to the unmanned operation, and there is no further verification of the underground personnel; when the rainstorm comes, no measures are taken to evacuate the underground personnel in time. Huang Sen, executive director of Dafengshu Mining Co., Ltd., did not conscientiously perform his duties, safety management was not in place, the supervision and implementation of the safety production responsibility system was not effective, after receiving the instructions of the regulatory authorities to evacuate people from the rainstorm, he only arranged for the mine director to evacuate people by telephone, and did not further track the implementation, violating the provisions of Article 19 of the "People's Republic of China Work Safety Law" and the first paragraph of Article 4 of the "Sichuan Provincial Work Safety Production Regulations", and had leadership responsibility for the accident. According to the relevant regulations, it is recommended to impose a fine of 19,000 yuan.
According to the data, the Dafengshu Coal Mine is located in Dazhu County, with 107 registered employees. In January 2020, the Sichuan Provincial People's Government approved the joint upgrading of the main mine with the Shimamen Coal Plant in Paifang Township, Dazhu County to 300,000 tons / year, and the construction procedures for the upgrading and transformation project were being handled before the accident.
