On November 1, 76-year-old Mrs. Xing was transported by ambulance to the intensive care unit of the emergency department of the Kaiyuan Branch of the First Affiliated Hospital of Heke University due to vomiting and impaired consciousness. At that time, the patient was in a moderate coma, weak breathing, cyanosis of the lips, blood oxygen saturation was only 79%, and the pulse was fine. According to Mr. Guo (pseudonym), the child of Mrs. Xing, Mrs. Xing is usually in good physical condition and does not have any underlying diseases. Three days ago, Mrs. Xing had no obvious cause for vomiting, confusion and other symptoms, and after treatment in the local hospital, she did not see obvious relief, but the symptoms became more and more serious, during which she had two breathing stops, her condition was critical, and her family was anxious.

After receiving the treatment, Lu Zhaofeng, director of emergency surgery, immediately carried out a series of emergency examinations such as arterial blood gas, CT, and blood coagulation for the patient, gave non-invasive ventilation and other measures, and activated the stroke green channel and multidisciplinary consultation mechanism at the first time. The stroke team and the multidisciplinary team combined with relevant information such as physical examination and imaging to rule out the possibility of related diseases and poisoning, and asked in detail about the medical history and the existence of poisoning and other factors.

"Diseases without obvious triggers are the most frustrating because they don't know where to start." Lu Zhaofeng said, "Combined with Mrs. Xing's medical history and clinical manifestations, we judged that she may be a metabolic disease caused by internal environmental disorders. The results of arterial blood gases quickly confirmed our initial diagnosis: the results showed a blood sodium content of only 100 mmol/L, and metabolic alkalosis and hypokalemia. Bedside ultrasound showed that Mrs. Xing was also accompanied by low ejection fraction and thrombosis of the lower extremities. ”

After a multidisciplinary consultation to determine the cause, Lu Zhaofeng led the team to formulate a corresponding and accurate treatment plan on the basis of effectively stabilizing the patient's vital signs: by timely reviewing the changes in the biochemical environment, adjusting the parameters of the ventilator to improve oxygenation; understanding the brain condition through magnetic resonance examination, and applying air pressure pumps in anticoagulation and intermittently to prevent the occurrence of thrombotic events.

After treatment, Mrs. Xing's consciousness gradually recovered, her breathing also stabilized, her oxygen saturation returned to normal, and her blood pressure and heart rate gradually stabilized, which made Mr. Guo, Mrs. Xing's child, very moved: "I am also a doctor, and I know very well how serious my mother's condition is, so I know better how difficult it is to diagnose and treat in the middle." ”

Subsequently, Mrs. Xing received further standardized treatment and follow-up multidisciplinary consultation in the intensive care unit of the emergency department of Kaiyuan Hospital. In the subsequent diagnosis and treatment process, Lu Zhaofeng, Zhu Gangyi, Yang Dapeng and others paid great attention to the condition of the elderly, and assisted her family to help her turn over, remove phlegm and move her lower limbs every day. Under the meticulous treatment and care of the medical staff of the emergency department of Kaiyuan Hospital, Mrs. Xing's condition quickly improved and she was successfully discharged from the hospital on November 8.
