Working on the “battlefield” of treating critically ill patients infected with COVID-19, the Central Hospital of Wuhan has a team of 12 people each deserving the title “critical soldier” thanks to their special skills. They use high-flow, non-invasive or invasive ventilators as their “weapons”, and the ICU and wards as their “fronts”. “爷爷,我们等下给您拔气管插管,您放轻松,好好配合我们……”1月3日,在武汉市中心医院呼吸与危重症科监护室,治疗团队为88岁的陈爷爷成功拔除气管插管。

“Grandpa, we are going to disconnect the tracheal intubation. Try to stay calm and cooperate with us...” On January 3rd, the medical staff successfully removed the tracheal intubation of the 88 year-old grandpa Mr. Chen who was being treated in the RICU of the Central Hospital of Wuhan.

一周多前,陈爷爷因冠心病、心力衰竭合并新冠感染,被送进武汉市中心医院。当时,老人呼吸困难,血氧饱和度低至57%,已出现呼吸衰竭。医护紧急给予经鼻高流量吸氧、无创机械通气治疗后,他的血氧饱和度仍不能维持。情况危急,必须马上进行气管插管!呼吸与危重症医学科监护室(RCU)护士长、呼吸治疗师喻正浩以最快速度调试好呼吸机,配合科室医生和麻醉医生迅速完成气管插管,打通老人呼吸通道。

Grandpa Chen was admitted to the Wuhan Central Hospital more than a week ago due to his coronary disease, heart failure and COVID-19 infection. When he was taken to the hospital, he had difficulty breathing, and also had a blood oxygen saturation as low as 57% with signs of respiratory failure. Despite receiving emergency treatment with high-flow nasal cannula oxygen therapy and non-invasive mechanical ventilation, his oxyhemoglobin saturation had not returned to normal. As he was in critical condition, he needed to be intubated immediately. Yu Zhenghao, respiratory therapist and head nurse of the RICU, adjusted the respirator as quickly as possible and worked with doctors and anesthesiologists to complete tracheal intubation so as to assist the elderly man in breathing.

老人治疗期间,喻正浩和监护室的呼吸治疗师轮班,24小时守护,时时监测呼吸机的数值,并精准调整参数,同时帮助老人俯卧位通气、排痰。几天后,陈爷爷的病情逐步稳定。在上机后的第七天,老人顺利拔除气管插管,转危为安。

During the course of treatment, Yu Zhenggao and the other respiratory therapists in the intensive care unit worked in shifts throughout the day, monitoring the values on the ventilator and accurately adjusting parameters, while assisting the elderly in exhaling and expulsion of sputum while lying on his stomach. A few days later, Grandpa Chen's condition eventually stabilized. Seven days following the intubation, the machine was successfully removed and he was no longer in danger.

据了解,一些高龄患者本身有基础疾病,在感染新冠后呼吸困难、血氧饱和度低,往往需要呼吸支持治疗。在救治危重症患者的过程中,呼吸治疗师是救治团队中非常重要的一员。

According to information provided to Jimu News reporters, some elderly patients with underlying diseases often require respiratory supportive care following COVID infection due to breathing difficulties and low oxyhemoglobin saturation. During the treatment of critically ill patients, respiratory therapists are vital members of the medical team.

“来,跟我一起练习缩唇呼吸,用鼻子吸气,呼气时像吹口哨一样把嘴巴嘟起来……”1月5日上午,在呼吸与危重症医学科监护室,呼吸治疗师蒋玉洁指导患者锻炼呼吸功能。患者是一位80岁的老人,刚入院时血氧饱和度只有85%,当时医护给予了高流量吸氧,并建议进行俯卧位通气。但老人喘气不适,一开始对趴着的姿势十分抗拒,蒋玉洁耐心劝解,才让老人慢慢接受。现在,老人的病情好转,也更加积极配合治疗。

Inhale through your nose and exhale through your mouth as if you are about to whistle.” In the RICU, Jiang Yujie, a respiratory therapist, was instructing a patient in breathing exercises on the morning of January 5th. The patient was an 80-year-old man whose oxyhemoglobin saturation was only 85% when he was initially admitted to the hospital. At that time, nurses offered him high-flow oxygen and advised him to breath while lying flat on his stomach, but the elderly man could not breathe well and he refused to do so. The elderly man eventually agreed after Jiang Yujie's patient persuasion. At present, his condition has improved and he is more willing to receive treatment.

“目前,这3名患者的呼吸情况非常平稳,15床患者的血氧饱和度情况需要重点关注……”1月4日下午3时,武汉市中心医院后湖院区呼吸与危重症医学科护士、呼吸治疗师徐俊杰正在包点包片的肿瘤科病房进行巡查,了解患者的呼吸、血氧饱和度情况,指导肿瘤科的护士使用、调节呼吸机。

“At present, these three patients can breathe steadily, but we need to keenly focus on the oxyhemoglobin saturation level of the patient on bed No.15...” said Xu Junjie a nurse and respiratory therapist in the Department of Respiratory and Critical Care Medicine at the Central Hospital of Wuhan, Houhu Lake Campus, who was conducting a ward inspection at 3 p.m. on January 4. Xu was patrolling the oncology ward in his charge to monitor patients' breathing and oxyhemoglobin saturation level and instruct the nurses of the Department of Oncology on how to operate and adjust the ventilator.

“徐护士,你来了啊!我现在呼吸比之前好多了。医生说我目前的治疗效果也越来越好,没有你的鼓励,我不会有现在的状态。”看到徐俊杰来到病房查看情况,74岁的胡老先生(化姓)握住他的手不停感谢,并伸出大拇指点赞。

“Nurse Xu, good to see you! I can breathe much better now than before. The doctor says I am improving with each treatment. But for your encouragement, I would not recover to such state” said by the 74-year-old patient Mr. Hu (with surname changed) who expressed his gratitude to Xu, holding his hands tightly and giving Xu a thumbs-up as soon as he saw Xu Junjie enter the ward to assess his condition.

胡老先生是肿瘤科的老患者,一周前因感染新冠病毒导致肺部感染住院。“入院后出现急性呼吸衰竭,血氧饱和度只有82%。我赶到病床前给予氧疗时,他的情绪十分焦躁,不停地说好难受……”徐俊杰回忆,“当时我一边安抚,一边布置氧疗装置,给予经鼻高流量氧气,然后握住他的手说不要怕,直到他血氧饱和度升到正常值。”

Mr. Hu, an elderly patient with a history of oncological diseases, was hospitalized a week ago after contracting a lung infection due to COVID-19. “He showed acute respiratory failure after admission, with an oxyhemoglobin saturation level of only 82%. As I rushed to his bed to administer oxygen therapy, he was very anxious and kept saying that he was very uncomfortable...” Xu Junjie recalled, “At that time, while comforting him, I set up the oxygen therapy equipment, delivered high-flow nasal cannula oxygen therapy until his oxyhemoglobin saturation rose to normal values, and then continuously held his hands and said, ‘Don't be afraid’ .”

The next day, Xu Junjie found while patrolling the wards that Mr. Hu could breathe better, however he was still very anxious and low in spirits. Xu Junjie continued to comfort and encourage him:“Mr. Hu, don't be afraid, do cooperate with our treatment actively, let's overcome the challenges together!” Through Xu Junjie's encouragement, Mr. Hu gradually gained confidence, and improved by actively cooperating with the doctor's treatment.

“心血管内科监护室5床需要插管上机,请马上过来。”这段时期,武汉市中心医院的呼吸治疗师们常接到临床来的电话,需要他们为患者提供呼吸支持。一旦接到临床科室的求助电话,他们会立刻奔赴到患者床边,上呼吸机、上高流量氧疗、观测呼吸机参数……直到患者血氧饱和度逐渐恢复平稳。

“Bed 5 in the CCU requires intubation, please come here immediately.” Recently, respiratory therapists from Central Hospital of Wuhan have been frequently called to the clinic to provide patients with respiratory assistance. Upon receiving the calls from the clinical department, they head straight to the patients' bedside to put on the ventilators and administer high flow oxygen therapy. They then monitor the parameters of the ventilators until the patients' oxyhemoglobin saturation gradually stabilizes.

据了解,武汉市中心医院这支12人的呼吸治疗师团队由呼吸与危重症医学科、重症医学科、神经外科、急诊科、新生儿科的护士组成。在这个团队里,超过一半的是90后,他们每天如同脚踩“风火轮”,在全院监护室和病房来回奔走,每日定时巡查和24小时电话随时接诊。

According to information obtained, the 12-person respiratory therapist team at the Central Hospital of Wuhan is composed of nurses from the Department of Respiratory and Critical Care Medicine, the Department of Critical Care Medicine, the Department of Neurosurgery, the Emergency Department and the Department of Neonatology. On this team, more than half were born after 1990. They are constantly busy as those on “wind fire wheels”, racing back and forth in the hospital's intensive care units and wards, performing routine inspections and being on call around-the-clock.

“呼吸治疗师的主要工作,是为呼吸功能不全的患者提供各类呼吸支持治疗和气道管理等各种与气道、呼吸相关的工作,在对重症患者的救治过程中,12人的呼吸治疗师团队克服重重困难,守护着全院上机的感染患者,关键时刻提供了生命支持。”该院护理部主任付阿丹如此说。

“The primary responsibility of respiratory therapists is to offer patients with respiratory dysfunction various respiratory treatments, airway managements and other airway-related treatments. While caring for critically ill patients, the team overcame several difficulties in order to protect the infected patients in the hospital, thus providing life support when it was most needed by them.” said Fu Adan, head of the hospital's nursing department.
